are clear about outcomes that matter we should be able to design better delivery systems

I agree, beds are seen as the rate limiting step but its not really the case in A&E flow, its always

organisational response and delays between process steps.

Once an admission occurs the pace of process slows further but given the lack of technology

employed to manage flow at the micro level, its largely invisible until bed pressures enforce

random actions to speed up discharge.

We find that patient discharge rates decrease after hours when our most senior clinicials are off

shift - how do we empower nurses and junior doctors to discharge patients when they are ready?

Re your point on costs of single rooms, the evidence is that it is not a big problem. Many time

wasting steps can be removed. Big opportunities for involving family/carers too.
