WHERE TO PERFORM ENDOVENOUS TREATMENT.
WHY OPERATING ROOM IS SAFER ?

FRANCE
Actual situation in FRANCE
Except phebectomies under local anesthesia and the evacuation of a superficial venous thrombosis, all the acts of superficial venous surgery must be made in an operating room (OR) with a high level of level sterilization environment (level IV as in arterial procedure with implantable prosthesis).
These acts generate an act of anesthesia coded, coveraged by social health care.
They generate a GHS for the health care establishment.
Actual situation in France
• At the national level the percentage of realization in ambulatory unit of the varicose veins is 55-60 %.
• Since December, 2010: the second wave of MSEP was organized targeted at the establishments the % of which was lower than 60 %.
• All the acts of venous surgery are concerned.
• The objective is to exceed the 80 % of ambulatory treatment
AVANTAGES
The advantages of the OR• Sterilized area and procedure• Security environment: anaesthesiologist,
Nurse• Recovery room• Monitoring per and operating • Available surgical material
AVANTAGES
The advantages of the ORComfort of the patient: feeling of safetyComfort of the Surgeon: everything is to be carried of the hand and very quickly if necessaryPossibility to modify the type of anesthesia according to the tolerance of the operated patient
IMMEDIATE FUTURE
• AMBULATORY PROCEDURE (+++)• Under tumescent anesthesia +/-neurolept
analgesia.• In operating sector (recommandation of HAS
04/2008)• At this time operating sector is equivalent of
an OP
THE FUTURE
• Considering the preferential use of tumescent anesthesia, the presence of a anaesthesiologist is not necessary
• A level block IV also is not necessary any more
DEMAIN
• 4 Levels: level 0 = doctor's office, Niveau III = OR)
• Level I: Doctor's office + technical cabinet:
• - Treatment room separated from the office• - With a water source and a sufficient lighting • - With a space being enough for allowing the provision of the
material and the necessary instruments• - Respect for the elementary rules of hygiene and asepty• - Non nurse and no assistant
CONCLUSIONS
The treatment endoveineux of the superficial venous incapacity allows and entails a radical modification of
the coverage of the patients requiring a fast adaptation of the vascular surgeons and the health
care establishments
CONCLUSIONS
But at this time the only legacy possibility to perform endovenous
thermal ablation of VV is for all procedures to perform these in an
operating room
