ANY ENTRY AT THE UPHA CHAPTER V TO HOLD BLAMELESS ANY PARTIES CONNECTED TO THE SHOW FOR

ANY TYPE OF LOSS. EVERY CLASS OFFERED HEREIN, WHICH IS COVERED BY THE RULES AND SPECIFICATIONS OF THE CURRENT USEF RULEBOOK, WILL BE CONDUCTED AND JUDGED IN ACCORDANCE THEREWITH. ENTRY DEADLINE: All entries close April 23, 2018. Post Entries will be accepted based on stabling

availability. STABLING IS LIMITED and post entries will only be accepted if stabling is available.

JUNIOR EXHIBITOR JACKPOT STAKES: All divisions offered for junior exhibitors will NOT require qualifying

for the championship class. ONE OWNER PER ENTRY FORM: Entries must be completed on the entry forms furnished by the UPHA Chapter V Horse Show. PLEASE TYPE OR PRINT. HORSES MUST BE ENTERED BY THEIR REGISTERED NAME AND CORRECT OWNERSHIP. Copies of registration papers are required. YOUR SOCIAL SECURITY OR FEDERAL TAX ID NUMBER IS REQUIRED TO RECEIVE A PREMIUM CHECK OPEN ACCOUNTS: There will be no open charge accounts without an open check or credit card on file in the office. SIGNATURE: Every exhibitor, owner, agent or trainer must sign the entry blank and be accompanied by a check or credit card covering all fees and charges. ALL ENTRIES are required to have a Certificate of Equine Infectious Anemia Test (Coggins Test) performed within 12 months of show. Health papers are required on all out of state horses. VACCINATION REQUIREMENTS 1. Horses entering the grounds must be accompanied by documentation of Equine Influenza Virus and Equine Herpes Virus (Rhinopneumonitis) vaccinations within six months prior to entering the stables. Horses not in compliance with this rule may be required to leave the competition grounds upon request by Competition Management. FEDERATION ENTRY AGREEMENT By entering a Federation-licensed Competition and signing this entry blank as the Owner, Lessee, Trainer, Manager, Agent, Coach, Driver, Rider, Handler, Vaulter or Longeur and on behalf of myself and my principals, representatives, employees and agents, I agree that I am subject to the Bylaws and Rules of The United States Equestrian Federation, Inc. (the “Federation”) and the local rules of the competition. I agree to be bound by the Bylaws and Rules of the Federation and of the competition. I will accept as final the decision of the Hearing Committee on any question arising under the Rules, and agree to release and hold harmless the competition, the Federation, their officials, directors and employees for any action taken under the Rules. I represent that I am eligible to enter and/or participate under the Rules, and every horse I am entering is eligible as entered. I also agree that as a condition of and in consideration of acceptance of entry, the Federation and/or the Competition may use or assign photographs, videos, audios, cablecasts, broadcasts, internet, film, new media or other likenesses of me and my horse taken during the course of the competition for the promotion, coverage or benefit of the competition, sport, or the Federation. Those likenesses shall not be used to advertise a product and they may not be used in such a way as to jeopardize amateur status. I hereby expressly and irrevocably waive and release any rights in connection with such use, including any claim to compensation, invasion of privacy, right of publicity, or to misappropriation. The construction and application of Federation rules are governed by the laws of the State AMERICAN SADDLEBRED HORSE ASSOCIATION Every trainer and/or horse owner MUST be a current member of American Saddlebred Horse Association with copy of current card. CH POINT ACCUMULATION In order to accumulate points toward the ASHA’s “CH” status, all parties must be members of the USEF. USEF Motorized Vehicle Statement Minors who do not have a valid driver's license which allows them to operate a motorized vehicle in the state in which they reside will not be permitted to operate a motorized vehicle of any kind, including, but not limited to, golf carts, motorcycles, scooters, or farm utility vehicles, on the competition grounds of licensed competitions. Minors who have a valid temporary license may operate the above described motorized vehicles as long as they are accompanied by an adult with a valid driver's license. The parent(s), legal guardian(s), or individual who signs the entry blank as a parent or guardian of a minor operating a motorized vehicle in violation of this rule are solely responsible for any damages, claims, losses or actions resulting from that operation. Violations of this rule will be cause for sanctions against the parent(s), guardian(s) and/or trainer(s) who are responsible for the child committing the offense. Penalties may include exclusion of the child, parent(s), guardian(s), and/or trainer(s) from the competition grounds for the remainder of the competition and charges being filed against any of the above individuals in accordance with Chapter 6. Wheelchairs and other mobility assistance devices for individuals with disabilities are exempt from this rule. SHIP IN DATE: Sunday Afternoon, May 6, 2018 STALLS: Horse and tack stalls are $140 each for the entire show. Late Entry Stalls $155. NO FIRST BEDDING PROVIDED. All stalls are 10 X 10 temporary box stalls and equipped with swinging doors. STABLING IS LIMITED and post entries will only be accepted if stabling is available.

BEDDING: NO OUTSIDE BEDDING ALLOWED. All bedding must be ordered through Horse Show Office. See entry form. A fee will be charged for shavings removal if bedding is brought in by an exhibitor. CAMPERS: Camper Fee $15 per day – first come first served. TIME OUTS: Only one (1) 5-minute timeout may be requested per exhibitor per class. In case of casting a shoe or breakage of equipment in the paddock, the gate may be held no more than 3 minutes. There will be no delay after the first horse enters the ring, at which time the gate will be held for 2 minutes. THE HORSE SHOW COMMITTEE decision shall be final and shall decide all questions not covered in these rules and regulations. CONTROL OF HORSES: Horses must be kept under control at all times. At the discretion of the judges and/or steward, horses may not be allowed to compete. Owner will forfeit entry fees. SHARPS CONTAINERS: Competition management may fine any individuals including trainers, owners, exhibitors, or their agents up to $100 for improper disposal of needles or other sharp disposable instruments. See GR1210.4 RING DIMENSIONS: Show Ring 107 feet x 217 feet – sand /loam footing – Covered Show Arena. Covered Make up Area 90 feet by 180 feet –Sand/loam Surface. COURTESY TO OFFICIALS: Any act of discourtesy to the judges on the part of any exhibitor, owner, trainer or groom shall disqualify the horse and rider. The owner shall forfeit his entry fees and any prize money won at this show. During the time of judging, no person will be allowed in the ring except ring attendants, judge(s) of the class and when necessary the handler in charge of the horse being judged. Attendants or grooms will enter the ring in suitable attire only. Headers may enter the ring by the end gate only and must take no actions that will affect the performance of any animal in the class. CANCELLATION/SCRATCHES: Exhibitors canceling their entries after the official closing for any reason shall forfeit the accumulation of entry and stall fees. All scratching of horses at the show must be done at the show office one hour before the start of the session in which they are to compete. PLEASE MAKE SCRATCHES EARLY AND BE CONSIDERATE TO OTHER EXHIBITORS. CHAMPIONSHIPS: To be eligible horses must have been entered, shown and judged in any other class of its division or section. The Horse Show officials may exercise their rights to limit Championship/Stake Class entries to the ribbon winners from the qualifying classes. VETERINARIAN: The Official Veterinarian will be on call and will inspect such horses designated by the judge(s) or steward to be considered unsound and will pass on any question pertaining to the soundness of any horse before being excused from the class. Their decision will be final. They will be available at all times and telephone numbers will be posted on the door of the Horse Show office for after show hours. PAYMENT OF PREMIUMS: All prize money will be paid by check drawn to the order of the owner as listed on the entry blank. Checks will be forwarded by mail within 30 days after the show. SOCIAL SECURITY NUMBER OF THE OWNER MUST BE INCLUDED ON ENTRY FORM BEFORE PAYMENTS ARE MADE. RECEIVING YOUR BACK NUMBERS YOU WILL NEED – Social Security or Federal Tax ID number for owner. Back numbers will not be issued until all paper work is complete.

ACADEMY DIVISION * Academy Entry Fee: $30 per class Horses: Academy horses must be school horses regularly used in a lesson program, and must not be in training. Horses may cross enter within the academy Riders: A rider’s age as of December 1st will determine his/her age group. Riders are not allowed to participate in any other equitation class or performance class. Once a rider has competed in non-academy classes he/she is ineligible for further academy competition. Riders to wear jodhpurs, jod boots, and shirt with tie. Gloves, sweaters or vests optional. No riding suits. Equipment: Junior riders and drivers are required to wear protective headgear with chinstrap. Protective headgear is defined as any brand of headgear that meets or exceeds ASTM certification requirements and is specifically designed for equestrian activities. The failure to wear such headgear will result in the elimination of the entry. Protective headgear is highly recommended for all adult participants. Derbies, jackets and chaps are prohibited. All academy classes are Saddle Seat and require flat saddles. Any type bridle and any type martingale is permitted. No stretchers or chains. No blacking of hooves or braiding of manes. Classes: Classes will be divided at 6 or more entries, except driving, which will divide at 5 or more entries. Equitation classes are to be judged according to current USEF rules governing equitation. Horsemanship classes stress the rider’s ability to meet and overcome challenges presented by the horse. In driving classes, the Academy driver must be accompanied in the cart by the assistant driver who must be over 18 years of age with experience in driving. Juvenile academy drivers must wear protective headgear while in the cart. It is strongly recommended that Adult Academy drivers wear protective headgear while in the cart. Extended trot or back will not be called for, judged at the walk and trot only. Drivers to wear academy attire. In all Academy classes, at the discretion of the judge, headers will be allowed into the ring for the lineup. Missouri Horse Show Association (MHSA) Rules will apply to Academy Division Splits in Equitation class will be followed immediately by their horsemanship class
