We’re always working to make health care easier for you, so we’ve listed the top five things

you can do to get the most out of your Anthem

health care benefits.

1Number Call your primary medical provider (PMP)

Your doctor or PMP is the first person you call for your health care needs. He or she will help with things like: Checkups and vaccines.

Referrals to specialists.

Referrals for tests and services.

Admission to a hospital.

To make sure you get care right away, we’ll choose a PMP for you. If you want a different PMP, log in or register at www.anthem.com/inmedicaid, click on Change Your PMP and follow the steps to choose a new one. Or download the free Anthem Medicaid App from the App Store or Google Play to find doctors, view claims and more. If you need help, call us at 1-844-284-1797 (TTY 711).

2Number Complete the Health Needs Screening The Health Needs Screening (HNS) is very important. It’s a list of questions that helps us know what kind of care you may need. We’ll call you to help you take the HNS. Plus, we’ll reward you for it!

Once you complete the HNS, you’ll earn $10 as part of the Anthem Rewards Program for Walmart purchases.

If you’d like to complete the HNS on your own, you can:

Visit a Pursuant Health kiosk in a nearby Walmart pharmacy.

Go online to www.anthem.com/HNS.

Call 1-844-284-1797 (TTY 711).

Get preventive care Good health starts with preventive care. This is the care you get when you go to your PMP before you get sick.

You should see your PMP at least once a year for a wellness checkup. This checkup gives your PMP the chance to get to know your health care needs. You should also see your PMP for shots and exams, like mammograms for women or prostate cancer screenings for men. We’ll send you reminders about your wellness checkups and exams so you don’t miss them.

To get your reward, you’ll need to complete the HNS within 90 days of joining Anthem. So don’t wait! You can also earn other rewards in the future.

Use our ride service to get to your appointments Having trouble getting to the doctor should never stand between you and your health. We offer transportation assistance to help you get to your doctor’s office, as well as to: Anthem health education classes.

Renewal appointments. Women, Infants, and Children (WIC) office. Your pharmacy after leaving the doctor’s office.

Follow these steps to use this benefit.

1. Make the call Call LogistiCare at 1-844-772-6632 (TTY 1-866-288-3133) Monday through Friday, 8 a.m. to 8 p.m. Eastern time. Rides must be set up at least 48 hours in advance.

2. Set up your ride When you call LogistiCare, you’ll need to tell them: Your member ID number (on your ID card) or Social Security number. The date, time, address and phone number of where you’re going. Any extra help you may need. Do you use a walker or wheelchair?

3. Book your return trip When your appointment is over, call LogistiCare for an estimated time of arrival. The driver will try to pick you up within the first hour of your call.

5Number Make the most of your pharmacy benefits Your pharmacy benefits give you what you need to get and stay healthy, including:

Prescription drugs. Over-the-counter (OTC) items listed on the OTC Drug Formulary.

And more.

You can find the full list of preferred drugs in your plan at www.anthem.com/inmedicaid. When you’re ready, take your prescription to any Express Scripts pharmacy in your plan. Show your member ID card when you pick up your prescription.
