REGISTERING FOR THE PATIENT PORTAL

• Type www.corhealthcare.com into the Search bar.

• Click on the Patient Portal link in the top bar.

• For first time users, click Register at the top left corner.

• Click “I accept” if you accept the Registration Consent Form.

• Fill out the blank fields with your personal information to create a new account.

• Once you have filled in all of your information, click Save.

• Click the answer that best describes your patient relation to COR Healthcare Medical Associates.

• Click Next.

• Fill out the New Patient Enrollment information. Boxes marked with a red asterisk * are required.

• After filling in all your information, click Next.

• Fill out Insurance information and click Next.

• Fill out Address information.

• Click Next.

• Fill out Employment Status and then click Review.

• Review information to check for any mistakes. If everything is correct, click Submit.

• Your enrollment information has been submitted to our office. You should hear back from clinic staff within 3 days. Thank you for enrolling with COR Healthcare.
