Abses gingiv

• Lesi inflamatori lokalisir,akut

• gingiva marginal dan interdental

• Infeksi plak, trauma, impaksi benda asing

Abses periodontal

• Untreated perio

• Moderate-deep pocket

• Kerusakan ligamen dan tulang

• Incomplete calculus removal

• Kondisi klinis lain

• (-)infl. : perforasi, fraktur

Abses perikoronal

• Inflamasi jaringan operkulum mahkota gigi erupsi sebagian

• Retensi plak, impaksi mknan, trauma

For some patients, periodontal abscess formation may be the first sign of the disease

Abses akut : kronis

Abses akut eksaserbasi dari lesi inflamatori kronisMerah,oedem, nyeri, halus/licin,ovoidEksudat: gentle pressure, gigi sensitif perkusi, demam, regional

lymphadenopathy

Abses kronis penyebaran infeksi setelah drainase spontan, respon host, terapi

(-) simtom, poket periodontal, inflamasi

Patient presenting with acute abscess complained of dull pain

and a sensation of tooth elevation in the socket. Signs of tissue distention and exudation are evident.

Abses periodontal : periapikal

Perawatan Abses akut Nyeri, kontrol penyebaran infeksi,

drainase abses

1. Drainase melalui poket• Anestesi topikal dan lokal• Dinding poket ditarik dengan prob

atau kuret untuk initial drainase dari dalam poket

• Gentle pressure dan irigasi • Skeling dan root planing

Pada lesi yg lebih besar, (-) drainaseDebridemen akar ditunda sampai tanda

klinis awal mereda antibiotik

Gentle digital pressure may be sufficient to express purulent discharge

2. Drainase dengan insisi eksternal• Isolasi dan keringkan dengan kassa• Anestesi topikal, lokal• Palpasi• Insisi vertikal, mulai dari mukosagingival melintasi daerah paling lunak ke tepi

gingiva• Dipisahkan dengan kuret atau elevator periosteal• Tepi luka ditekan ringan menggunakan kassa• irigasi

Pada abses dengan inflamasi dan pembengkakan yang parah, maka instrumentasi yang berlebihan sebaiknya ditunda antibiotik

Tidak indikasi antibiotik: instruksi kumur-kumur air garam hangat, aplikasi periodik chlorhexidin gluconate (kumur-kumur/ cotton-tipped)

Instruksi: istirahat, banyak minum air (komplikasi sitemik), analgetik Jika simtom tidak hilang maka instruksi sebelumnya tetap dilanjutkan selama 24 jam

Abses kronis

Dengan skeling dan root planing atau terapi bedah. Terapi bedah diindikasikan jika terdapat defek yang luas atau didaerah furkasi

(A) Deep furcation invasions area common location for the periodontal abscess

(B) Furcation anatomy often prevents the definitive removal of calculus and microbial plaque.

Instruksi;24 jam pertama (setiap 2 jam) kumur-kumur air garam hangat
