Connected Care
The Alliance plan includes an impressive network of
more than 23,000 physicians and specialists, and
more than 127 hospitals. And since these provider
medical groups have been recognized for offering
quality care, members will feel good about choosing
a primary care physician from one of the participating
medical groups.
The primary care physician will be the member’s
first source for care and coordinate additional
care they may need.
We also make it easy for members to access a doctor or
specialist who will fit their unique health needs.
Patient Care Continuum
The Alliance plan was built on the fundamentals of
patient-centered care, and enhancing the patient-
doctor relationship to promote health care quality at
lower costs. The patient care continuum represents
comprehensive coordinated health services designed
to improve the quality and value of care. Innovative
information systems help provide a singular view
of any patient’s care, helping providers effectively
coordinate and link resources to avoid duplicating
tests or services, facilitating seamless movement
between care settings.
The focus is on a patient’s overall health, from
preventive care to complex disease management.
UNITEDHEALTHCARE’S ALLIANCE* is an innovative health plan offering select physicians who collaborate on their patients’ treatment plans. As a distinct subnetwork of the UnitedHealthcare’s full HMO network, the new Alliance plan provides access to a local network of physicians and specialists. These physicians coordinate care and work with their patients to manage their health and health care costs.
DISTINCT ALLIANCE NETWORK With the Alliance plan, participating medical groups have been selected based in part on their technological sophistication and consistent level of quality performance. UnitedHealthcare has relied on the State of California’s Office of the Patient Advocate (OPA) quality report card for information on the participating medical groups. The quality report cards are published annually on the state’s website at www.opa.ca.gov. Members have access to this distinct network of quality-rated participating medical groups that are connected to their patients’ care. UnitedHealthcare’s Alliance plan, offering quality health care at a lower cost, is a smart choice for any business. The Alliance participating medical groups are
known for providing quality, connected care and
offering online tools that make it easier to review
your medical history and develop the right treatment
plan. The outcome: quality health results.
Preventive care is covered 100 percent
Preventive care services include routine wellness
exams, well-child exams, physicals, mammograms,
flu shots and other immunizations. As long as
Alliance members see network doctors, they don’t
even have to worry about copayments.
Integrating pharmacy solutions to provide better patient outcomes
Our pharmacy coverage focuses on clinical quality
and improving patient outcomes while promoting
the most appropriate use of medication. Our
prescription drug list and pharmacy programs
are built on evidence-based care and integrated
research data to keep treatment costs in check.
Alliance plan members can also receive
personalized communications to help them make
more-informed decisions. For additional savings,
outpatient prescription drugs can be obtained
through a mail order program or by accessing one
of many retail pharmacy locations.

Better information to help members make better decisions. Alliance includes a benefit
program that sends members messages on how to improve their own health and save money.
They will also receive a personalized health report based on their claims history. This report
provides information and best practice recommendations based on the member’s specific
health conditions.
Helping those with chronic diseases experience a better quality of life. Our disease
management programs can help members manage chronic diseases, such as asthma and
diabetes. We are committed to helping those living with an illness experience the best quality
of life possible.
Adding more value with our wellness programs. Health and wellness programs on nutrition,
fitness, smoking cessation, stress management and more are offered at no additional cost, so
members may be healthy and more productive at work and less likely to be absent because of
illness or injury.
