Town Clerk Sylvia A. Cabana is enhancing the way you can apply for your “One-Stop” Passport.

Passport office hours are Monday- Friday, 8 a.m.-3:45 p.m. with extended hours Thursday, 8 a.m.-6 p.m. For further information visit www.travel.state.gov or call 1-877-487-2778.

Passport Application RequirementsAn average application will require: a completed DS-11 application;

evidence of citizenship; proof of identity; a passport photo; check or money order for passport fees, payable to the U.S. Department of State; and

cash, check or money order for the execution fee, payable to the Town of Hempstead. All applicants, including minors, must appear in person.

Passport RegulationsAll persons, including minors are required to

obtain passports in their own name for travel to foreign countries. Proof of citizenship and primary identification are required. Full details can be found on www.travel.state.gov.

It is recommended that 16- and 17-year-old individuals have at least one parent present.

It is recommended that children under 16 be accompanied by their parent(s)/guardian(s) to authorize the issuance of their passport. If their parent(s)/guardian(s) cannot appear in person, additional documentation will be required. Please refer to travel.state.gov for more information.

Passport FeesPassport fees are set according to federal guidelines. For applicants

16 years and older, a check or money order payable to the U.S. Department of State in the amount of $110, for the passport book and/or $30 for the passport card. For applicants under age 16, a check or money order payable to the U.S. Department of State in the amount of $80 for the passport book and/or $15 for the passport card. For all applicants an execution fee of $35 payable to the Town of Hempstead in cash, check, money order or credit card is required, and an additional $10, if you choose to take a passport photo at our facility. Credit cards will incur a $1.50 service fee, and charged order over $62.50 incur a 2.4% service fee.

Optional ServicesExpedited service is available for an additional fee of $60 paid

to the U.S. Department of State. Overnight mailing service from the Department of State

for $15.89 paid to the U.S. Department of State.Overnight mailing service from the Town of Hempstead for $24.70 paid

to Town of Hempstead. For processing times, please visit travel.state.gov.

Recent Passport Service Upgrades The town has developed a Spanish passport reference guide available to assist

Spanish-speaking applicants with the completion of their U.S. passport applications. The outer office reception area and office space were improved to make them more

accessible for constituents and in accordance with Americans with Disabilities Act (ADA).‘Fast Forward Fridays’ for veterans and seniors: 8:30 a.m. to 12 noon.

All minors, first time applicants and those who are not eligible to use the DS-82 (U.S. Passport

Renewal Application for Eligible Individuals).• Must appear in person with valid picture identification and proof of citizenship, for examples please visit travel.state.gov• Must answer all questions, #1-21, on the form.• Print legibly using BLACK INK ONLY.• Leave NOTHING BLANK.• NO WHITE-OUTS.• DO NOT SIGN THE APPLICATION UNTIL INSTRUCTED.• Select the document(s) for which you are applying: o U.S. Passport Book. o U.S. Passport Card.
