What is a healthy community?

Participants answered the question “What is a healthy community?”

They were encouraged to think beyond their current conceptions of healthcare to their ideal visions of what it means to be healthy in all aspects of life. A selection of key words from the responses are arranged below. The size of a word corresponds to the frequency of its use in the combined responses.

How Health is Evaluated in Dodge County

+ County Health Rankings, which are destermined by:

- Health Outcomes: morbidity and mortality, health related quality of life and percent of low birth weight infants.) - Health Factors: health behaviors (smoking, diet, exercise, alcohol use), clinical care, socioeconomic factors and physical environment.+ Assessing Residents' Primary Concerns About Health (as gathered through community surveys): - Lack of exercise - Obesity and overweight adults - Unhealthy eating habits - Obesity in kids - Tobacco use in adults and youth+ State Health Improvement Program (S.H.I.P.) grant designed to promote health in the following areas: - Physical activity , nutrition, healthy weight, and healthy activity in: ~ Schools ~ The community ~ Work sites ~ Healthcare institutions+ S.H.I.P. funded the following ventures: - Snack carts in schools - Free swim days - Healthier school lunches - School gardens

+ Planning for new clinic started 15 months ago; interest in understanding what health truly means to this community.

+ Physical spaces do not define healthcare. The majority of " health " happens outside of clinic walls.

+ "How can we [ healthcare professionals] help you to stay healthy, [before you get sick]?"

+ We'd like to know how we can partner with members of the community to help create a healthy community.

+ So far there are no dates, timelines, locations, bulding plans etc.

+ To reform health[care], is to have a bigger vision of what health means, and what we are going to do to improve health for everybody.

+ " The cost of healthcare is everything a community uses in a year multiplied by the price of those things. Everything we do reflects that. When we don't eat well, for instance, that may create trouble or aggravate [health] problems."

+ Health and healthcare, in the future, are not a reflection of a clinic or hospital.

+ Healthcare professionals can make more of an impact outside of the clinic or hospital.

+ Ask yourself " What would we like to do in Dodge County that would improve the health of the community and improve our experience with 'the [healthcare] system'? "

+ In C.F.I. research, people understood health to mean that they could do everything they had to do in their day-to-day lives.

+The Triple Aim of new healthcare delivery systems is to: 1. Improve individual health 2. Improve community health 3. Reduce overall costs.

+ Overall, the costs of healthcare are rising. Without solutions to reduce the cost of healthcare, we may have to make choices in the future about whether to pay for healthcare or for education, for example. We would rather it be the communities in question that provide their own solutions, rather than have someone else tell them how it should be done.

Groups discussed the questions for some time and then each group elected a representative to share their ideas out loud with the other groups.

What can the Kasson Clinic do to promote and contribute to a healthy community?+ Walking and bicycle trails

+ Ask about social factors in doctor's appointments. -" Who are your close neighbors or family, Mr. Smith?"

+ Educational videos distributed throughout the community through cable television and local cinemas. They may be played preceding screenings of family-friendly movies.

+ Collaborative partnerships with schools, parks and recreation departments, and local stores and businesses, including markets. This may include "local" or "heart healthy" designations on food items, or it may entail a healthy eating rewards club.

+ Vaccinations at school.

+ Community education programs. Cooking classes taught by a nutritionist, for example.

+ Volunteer neighbors trained to be able to answer basic health questions and have contacts to local healthcare professionals.

What new wellness services shouldthe Kasson Clinic add to its offerings?+ Healthy living demonstrations and educational classes for youth and adults. These could address existing conditions (diabetes) and prevention of conditions (falls by seniors, vaccinations and autism, men's health, etc.)

+ Child-friendly environments to make a trip to the clinic more fun!

+ A wider selection of specialized tests and medical services: (mammograms, E.K.G. tests, C.T. scans, physical therapy, etc.)

+ Community library to encourage learning.

+ Work site and home physicals and vaccinations.

+ Focus and outreach to minority population in Dodge County.

+ Accessibility to healthy food options (local, organic, farmer's market, low fat, low-cal diabetic, gluten free, etc).

+ Innovative partnership with the school district, including nutrition education.

+ More opportunities for affordable or free physical activities ( outdoor spaces that are accessible to all, senior exercise center, after school physical education for children with working parents, swimming pool, etc).

+Community-wide prevention care program.

+Accessibility of care.

+ A "quick clinic" for common or non-urgent medical issues.

+ Having to drive to Rochester for anything more serious than the common cold. + There is a need for an urgent care or longer hours at the clinic on certain days.

+ Affordable [health and healthcare] options for all.

+ Culturally competent and sensitive care for new or culturally different residents.

+ The use of community health workers to reach out to specific populations within the community.
