1. มความรความเขาใจเกยวกบระบบการนากระแสสญญาณไฟฟาหวใจ
2. สามารถอธบายขนตอนและทาการการตรวจคลนไฟฟาหวใจไดอยาง
ถกตอง
3. มความรความเขาใจและสามารถอานแปลผล ECG ไดอยางถกตอง
4. สามารถใหการพยาบาลผปวยทมภาวะหวใจเตนผดจงหวะทสาคญได
อยางถกตองและเหมาะสม
บทนาและความสาคญ
การอานคลนไฟฟาหวใจ เปนการพยาบาลทสาคญแสดงใหเหนองคความร
ความสามารถ (Competency) ของการแปลผลคลนไฟฟาหวใจ นาสกระบวนการพยาบาล
ทเหมาะสม เพอความปลอดภยของผปวยเปนสาคญ เพราะพยาบาลเปนบคลากรในทม
สขภาพทอยใกลชดผปวยตลอด 24 ชม. หากพยาบาลขาดความรในการประเมน
คลนไฟฟาหวใจจะสงผลใหการชวยเหลอผปวยเกดความลาชา อาจนาไปสภาวะหวใจเตน
ผดจงหวะทรนแรงหรอหวใจหยดเตนได
คลนไฟฟาหวใจชวยบอกอตราและจงหวะการเตน ชวยใหเขาใจกลไกและชวย
วนจฉยภาวะของโรคหวใจและหลอดเลอด ภาวะหวใจเตนผดจงหวะชนดตางๆ ผลของ
ยา ภาวะเสยสมดลเกลอแร เปนตน ดงนนการประเมนคลนไฟฟาหวใจ เชอมโยงให
พยาบาลเขาใจในพยาธสภาพของโรค สงผลใหการพยาบาลไดอยางรวเรวและมคณภาพ
เพอใหผปวยมความปลอดภยมากทสด

ความหมาย EKG• EKG หรอ ECG คอ การบนทก Electrical impulses ทไป
กระตนหวใจใหทางานซงจะมกระแสไฟฟาเกดขนขณะหวใจบบตว
และจะแผกระจายไปยงผวหนง การนา Electrode ไปวางทผวหนง
แลวตอเครองมอทพฒนามาใชบนทกคลนไฟฟาหวใจ โดยเฉพาะจะ
สามารถบนทก คลนไฟฟาหวใจ ทเกดขนได

หวใจกบการควบคมโดยระบบประสาท
• ระบบประสาท Sympatheticมาจาก
• Cardiac, Pulmonary และ coronary plexus
• มาเลยงท SA node, AV node และกลามเนอหวใจ
– ผลการกระตน ทาใหกลามเนอหวใจบบตวแรงขน (positive inotropic)
– อตราการเตนของหวใจเรวขน (positive chronotropic)
- Positive dromotropic (นาไฟฟา)
- คลายตวเรวขน

ระบบประสาทพาราซมพาเตตก
• เสนประสาทเวกสขางขวาไปท SA node
• เสนประสารทเวกสขางซายไปท AV node
– ผลการกระตน ทาใหกลามเนอหวใจหดตวนอยลง
(negative inotropic) มผลตอกลามเนอ Atrium เปนสวนใหญ
- ทาใหอตราการเตนของหวใจชาลง (negative chronotropic)
- Negative dromotropic (นาไฟฟา)
- กระตน Vagus ทาใหผลของประสาท sympathetic ลดลง

Acute Coronary Syndromes
1. IschemiaRepresented by symmetrical T wave inversion(upside down).
The definitive leads for ischemia are: I, II, V2 - V6.
2. Injury
Acute damage - look for elevated ST segments. (Pericarditis
and cardiac aneurysm can also cause ST elevation; remember
to correlate it with the patient.
3. Infarct
Look for significant "patholgic" Q waves. To be significant, a Q
wave must be at least one small box wide or one-third the
entire QRS height. Remember, to be a Q wave, the initial
deflection must be down; even a tiny initial upward deflection
makes the apparent Q wave an R wave.

Symmetrical T wave inversion
IschemiaRepresented by symmetrical T wave inversion (upside down).
Look in leads I, II, V2-V6.
Injury Acute damage -- look for elevated ST segments.
Infarct"Pathologic" Q waves. To be significant, a Q wave must be at
least one small square wide or one-third the entire QRS height.
Myocardial Ischemia
ST depression = depression of the ST segment level >0.1mV compared with the baseline ST level for at least 1 minute

4.รปรางและตาแหนง (Configuration and location)

7–step process for EKG interpretation
1. Calculate the heart rate.
2. Determine whether the rhythm is regular or irregular.
3. Ensure that there is a P wave before every QRS complex and that
all the P waves are the same shape.
4. Measure the PR interval to determine if they are between 0.12 and 0.20
second.
5. Ensure that a QRS complex follows every P wave.
6. Verify that the QRS complexes are all the same size and shape and
that they measure between 0.04 and 0.10 second.
7. Determine the overall appearance and interpret the rhythm.

The QT interval and U wave
Normally QT interval : Duration < 0.36 sec. (9 mm.)Normally U wave: not seen, Although the U wave, which follow the T wave, has been associated with hypokalemia (which cause its accentuation) it may be found normally. It is often seen best in the midprecordial leads (V3 and V4, as above), and it has the same orientation as the T wave.

Paroxysmal – หวใจเตนผดจงหวะทเกดขน และหยดทนท
เปนพก ๆ
ความหมายเฉพาะ:
Sinus bradycardia - sinus rate < 60 ครง/นาท.
Sinus tachycardia - sinus rate 100-180 ครง/นาท ซงอาจเกดขณะออกกาลง หรออยในสภาวะทมการเพมการปลอยประจาของ SA node.
Sick sinus syndrome - การทางานของ SA node ทผดปกตขณะออกกาลงเปนผลใหมการเตนของหวใจเปลยนแปลงอยางมาก เชน bradycardia และ tachycardia
ภาวะหวใจเตนผดจงหวะคาจากดความทควรรจก
มลกษณะ1. Rate ปกต คอ 60-100 ครง/นาท Atrial rate = Ventricular rate
2. จงหวะการเตนสมาเสมอ PP และ RR interval มคาคงท
3. P waveรปรางปกตและเหมอนกน นาหนา QRSทกตว และหวตงใน Lead
I,II,aVF4. PR, QRS ปกตทงระยะเวลาและรปราง
Normal sinus rhythm
1. ภาวะหวใจเตนชาผดปกต (bradyarrhythmias)
2. ภาวะหวใจเตนเรวผดปกต (tachyarrhythmias)
2.1 Narrow QRS complex tachycardia

Sinus Bradycardia
- rate 40-60 ครง/นาท จงหวะการเตนสมาเสมอ
- P wave รปรางปกต นาหนา QRS complex ทกตว
-PR interval, QRS complex, T waveปกต เหมอน normal sinus rhythm พบ
ใน คนปกตทแขงแรง นกกฬา สงอาย โรคหวใจ หรอไดรบยา B-blocker, amiodarone, digitalis
What is arrhythmia?
Arrhythmia literally means “ out of rhythm” and is the term used to describe a condition where the heart beat has become irregular. Clinically, arrhythmia includes all rhythms of the heart other than its normal sinus rhythm.
• Irregular heart beat • Excitation from ectopic foci other than the sino-atrial
node causing the heart to contract.• Disturbance of conduction of excitation.• Rhythms faster than the regular sinus rhythm and of
more than 100 beats/min (tachycardia).• Rhythms slower than the regular sinus rhythm and of
less than 100 beats/min (bradycardia).

• 1. Direct factors; Ischemic heart disease, Myocardial infarction, Pericarditis, Valvular heart disease (Mitral valve stenosis), Congestive heart failure, Post-op Open heart etc.
• 2. Indirect factors; Electrolyte imbalance, Respiratory acidosis or alkalosis, Metabolic acidosis or alkalosis, Hypoxia, Shock, Hyperthyroidism, Automatic nerverse system abnormalities, Medication, Food, Tea, Coffee, insomnia etc.
Factors related to arrhythmia
1. Abnormally slow heart beat
• Electrical impulse from SA node < 60 beats/min
• - Sinus bradycardia (approximately 40 – 60 beats/min )
• - SA block
Abnormally of SA node
Sino-Atrial block
• เปนความผดปกตของ sinus node จาก EKG จะไมเหน P-wave แบง
ออกไดเปน
• 1. Sinus pause หรอ Sinus arrest
• 2. Sino-atrial (SA) exit block
• 3. Sick sinus syndrome
Causes of Sinus Arrest• Hypoxia,Myocardial ischemia or infarction, Hyperkalemia, Digitalis toxicity.
• Reactions to medications such as Beta-blockers, and Calcium channel blockers.
• Carotid sensitivity,or increased vagal tone.
Intervention for Sinus Arrest
• Signs of hemodynamic compromise such as
weakness,lightheadedness, dizziness,or syncope.
• If the episodes are transient and no significant signs or
symptoms,the patient is observed.
• If hemodynamic compromise is present,IV Atropine may
be indicated. If the episodes are frequent and/or
prolong(>3 sec.), temporary pacing or insertion of a
permanent pacemaker may be warranted.
Sino-atrial (SA) exit block
• เกดเนองจาก SA node สงคลนไฟฟาออกมาชา และในทสด
เกดการ block conduction ของคลนไฟฟาท SA junction
(บรเวณท SA nodal fiber ตอเชอมกบ intra-atrial tract) SA
block แบงไดเปน
• - First degree SA block
• - Second degree SA block type I (Sinus Wenckebach)
• - Second degree SA block type II
• - Third-degree SA block (type III)
Intervention and nursing care for SA Block
• If the episodes of SA block are transient and there are no
significant signs or symptoms, the patient is observed.
• If signs of hemodynamic compromise are present and are the result
of medication toxicity, the offending agents should be withheld.
• If the episodes of SA block are frequent, IV Atropine, temporary
pacing, or insertion of a permanent pacemaker may be warranted.
Causes of SA Block• Relatively uncommon but may occur as a result of acute MI,
Digitalis, Quinidine, Procainamide, or Salicylate. administration,
CAD, Myocarditis, CHF, Carotid sinus sensitivity, or increase vagal
tone.
Abnormally of SA node
2. Abnormally rapid heart beat
Electrical impulse from SA node > 100 beats/min
- Sinus tachycardia (HR 101 – 150 beats/min)
- Supraventricular tachycardia (SVT)
(HR > 150 beats/min)
Sinus Tachycardia
- Rate มากกวา 100 ครง/นาท จงหวะการเตนสมาเสมอ
- P wave นาหนา QRS complex ทกตวใน อตรา 1:1
- PR ปกต และ คงท
ลกษณะ
• Symtoms: Palpitation (normally not dangerous) but in the patients have heart disease observe the symtom.
• Treatment: Detect of main causes example; exercise, emotional of exciting, insomnia, hyperthyroidism etc.
Nursing care
- Check pulse and vital sign and Past history.
- Detect of factors related to arrhythmia.
- Detect of medication the patient have; Aminophylline, Dopamine,Epinephrine etc. if hemodynamic change notify the doctor.
• Management of SVT
- Conversion SVT to NSR
• Depend on hemodynamic
• Unstable
- Electrical cardioversion (state with 50-100 J Biphasic)
- Adenosine
• Stable
- Correct precipitating cause
- Vagal manuvers
- Antiarrhythmic agents
- Overdrive pacing
Sinus Arrhythmia
- จงหวะการเตนไมสมาเสมอ หายใจเขาอตราเรว หายใจออกอตรา
ชาลง PP, RRไมคงทตางกน>0.16วนาท
-พบไดในผสงอาย หรอ โรคกลามเนอหวใจดานลางตายเฉยบพลน
- P wave นาหนา QRS complex ทกตว และPR ปกต
Sinus arrhythmia
• The impulse of SA node is irregular. Sinus arrhythmia meets all
the criteria as described under normal sinus rhythm, except for
the variation in rate, often associated with the respiratory
cycles. It is commonly seen in the young. The PR intervals are
constant, but the RR intervals are continually changing.
• May occur because of emotional stress, CHF, Myocardial ischemia
or injury, mental and physical fatigue, atrial enlargement, digitalis
toxicity, hypokalemia, hypomagnesemia, hyperthyroidism, and
excessive intake of caffeine, tobacco or alcohol.
Causes of PACs
• Do not require treatment if they are not frequent.
• Frequent PACs are treated by correcting the
underline cause.
• If needed,frequent PACs may be treated with
beta-blockers,calcium channel blockers,and/or
antianxiety medications

Intervention for Atrial Flutter
• Atrial flutter associated with a rapid ventricular rate,treatment is first directed toward controlling the ventricular response.
• Cardiac function is normal-may use Calcium channel blockers or Beta-blockers.
• If cardiac function is impaired-may use Digoxin, Diltiazem, or Amiodarone.
• If the patient exhibits serious signs and symptoms -Synchronized cardioversion. (cardioversion 50-100 J Biphasic)
Multifocal atrial tachycardia (MAT)

alcohol,carbon monoxide), hypokalemia, and hyperthyroidism.
• Paroxysmal A-fib has been associated with excessive alcohol

First degree atrioventricular (AV) block represents a delay in the transmission of impulses from the atria to the ventricle. The delay commonly in the AV node but may occur below the bundle of His. A prolonged PR interval (greater than 0.2 second) is seen on this eletrocardiogram.
Second-degree AV nodal block the conduction velocity is slowed some

this can result in p-waves that are not followed by QRS .

1.Second Degree AV Block Mobitztype I or Wenckebach Block
In Mobitz 1 block ventricular beat are “ drop” in cyclic manner.
The PR interval is less prolonged at first but becomes
progressively longer, until a atrial contraction no longer
initiates a ventricular response. The cycle is then resumed. In
the above electrocardiogram, the first PR interval is shorter
than the second, which is shorter than the third. The fourth P
wave does not conduct and is therefore not followed by a QRS
complex. The fifth P wave starts the cycle again.
Second - degree AV block : Mobitz I (Wenckebach)

2.Second Degree AV Block Mobitztype II
Mobitz II blok is classified under second degree AV block,
since P waves are perioddically not followed by QRS
complexes (arrow). Electrophysiologic studies have shown
that the site of block is usually below the bundle of His and
may be a warning of future complete AV block.

1. QRS complex suddenly missing
2. PR intervals unchanged

Third Degree AV BlockIn third degree, or complete AV block, there is no relationship between the
atria and the ventricles. The atria, remaining under the control of the SA
node and are completely dissociated from the ventricle. Depending on the
site of impulse formation, The QRS complex may be of normal duration,
as above, with the pacemaker in the AV junction, or quite wide and
bizarre, with the pacemaker low in the ventricles.
There will be complete asynchrony between p-wave & QRS
complexes.
Atrial rhythm may be completely normal
Ventricular rhythm will be greatly reduced depending upon the
location of the site generating the ventricular impulse.
Ventricular rate typically range from 30-40/min.
Third-decree AV nodal block 4.
Third Degree AV BlockA patient with complete AV block, as seen here, may present with
symptoms of cerebral insufficiency, such as dizziness or clouded
mentation, or may actually have lost consciousness as a result of
the low heart rate and poor cardiac output or of transient
ventricular standstill or fibrillation. The present-day treatment is
electrical pacing to maintain a proper rate and good cardiac
output. The arrows point to pacemaker impulses.
3rd degree AV block : Complete Heart Block

• Treatment of PVCs depends on the cause, patient’s signs and symptoms.
• Most patients experiencing PVCs do not require treatment with antidysrrhythmic medications.
• The use of prophylactic lidocaine is no longer recommended. Although lidocain reduces the incidence of VF.
• Treatment of PVCs seen in acute MI should be directed at ensuring adequate oxygenation, relief of pain, and rapid identification and correction of hypoxia, heart failure, electrolyte or acid-base abnormalities.
