If any person suffers loss or damage which is proved to have been caused by any negligent act or omission of Det Norske Veri tas, then Det Norske Veritas shall pay compensation to such person for his proved direct loss or damage. RENEWAL SURVEY CLASS AND STATUTORY
SCOPE OF SURVEY
This is to confirm that the undersigned Surveyor attended onboard abovementioned vessel
while she was having drydocking and refit in Sembawang Shipyard and had carried out
surveys as follows:
- First Renewal Class, Hull
- Tanker for Oil, Complete
- Bottom Survey
- Tailshaft monitoring, Annual
- Oilfired Aux. Boiler, Renewal
- Exhaust Aux. Boiler, Renewal
- EO- Class, 5-Yearly
- Continuos MachinerySurvey
- Load Line Certificate, Renewal
- Safety Construction Certificate, Renewal
- Safety Equipment Certificate, Renewal
- Safety Radio Certificate GMDSS, Renewal
- IOPP Certificate, Renewal
In addition to the above surveys, as requested by the owner's representative the following
were carried out:
- Witness Load Test of Lifting Appliances ( see separate report form 40.9a)
- Survey for Change of Load Line Marks (see separate report 40.9a)

Surveys in connection with the new requirements of SOLAS Ammendments were also carried
out as follows:
- Installation of Aft and Fwd Emergency Towing Arrangement, SOLAS Ch.V,Reg.15-1(see
separate report Form 40.9a)
- Existing Safe Acces to Tanker Bows, SOLAS 1996 Dec. Amendments Ch.II-I, Reg. 3-3 (see
separate report Form 40.9a)
- Existing Cargo Tank Venting Arrangements, SOLAS 1996 Amendments Ch. II-2,
Reg.59(see separate report Form 40.9a)
FIRST RENEWAL CLASS, HULL
For the purpose of completing the survey, reference was made to the Enhanced Survey
Programme and Rule Pt.7 Ch.2 Sec..3 . The following tanks were inspected/tested and results
found as follows: (refer separate form no. 40.66a)
Centre Cargo Tanks (No. 1,2,3,4,5,6,7and 8)
All cargo tanks were examined. Coating condtion was generally found good. Partial close-up
examination was carried out. Cracks were found iwo welding in tank no.3,6,and 7.
- Tank no.3 : Longitudinal cracks at the intersection between shedder plate of lower stool to
tranverse corrugation and stbd. longitudinal bulkhead.
- Tank no.6 : Longitudinal cracks at the intersection between shedder plate of lower stool to
tranverse corrugation and port longitudinal bulkhead.
- Tank no.7 : Longitudinal cracks at the intersection between shedder plate of lower stool to
port longitudinal bulkhead and and tranverse coorugation.
Repairs were carried out by inserting new plates. New backing flat bar stiffeners were added
parrallel /adjacent to the shedder plates iwo ballast tanks. MPI carried out and no
discontinuities were found.(see report no. 98402)
Tank boundries facing ballast tanks were all tested. No indication of leaking or buckling were
found upon completion of testing.
Water Ballast Tank ( No. 1P&1S, No.2, No.3P&3S, No.4P&4S)
All water ballast tanks were examined. Tank coating was found in good condition. Complete
close up examination carried out. The following defiencies were observed:
- Water Ballast Tank No.2 : Web frame plating, its horizontol stiffener and lug iwo stbd bay
no.3 and no.4 counting from forward bulkhead and first stringer from bottom found

buckled. Web frame plating was partially renewed. Stiffener and lug were fully renewed.
- Water Ballast Tank No.4S : Pitting bottom plate adjacent to suction belmouth. Repair
carried out by filling it up with welding.
Slop Tank Port and Stbd.
Tanks were internally examined. Coating found to be in good condition. The following
defects were found:
- Slop Tank Port : Pitting iwo bilge well. Repair carried out by means of welding. MPI
carried out and no discontinuities found.
Fore and Aft Peak Tank
Both tanks were pressure tested. Tanks were internally examined . No coating failures and
srtuctural damages were found.
Void Stool Tanks
All void stool tanks were completley close-up examined. Coating was found in good
condtion. No stuctural defects were found.
Representative Fresh Water, Fuel Oil and Lubricating Oil
Above reperesentative tanks were pressured tested and examined. No indication of leaks and
structural damages were found.
Ultrasonic Thickness Measurement
UTM was found not necessary to carry out due to the following reasons:
- Coating of cargo tanks, ballast tanks ,void stool tanks and main deck as observed during
inspection was found generally in good condition.
- No any significant corrosions observed in those tanks and main deck
- No any major stuctural damages or thinning down were observed in those tanks and main
deck
- Centre cargo tanks were constructed without plate members. Deck tranverses can bee seen
fitted on top of main deck.
BOTTOM SURVEY
Bottom survey carried out with the inspection of sea suction, sanitary and overboard
discharge valves.

Rudder pintle bushes were found to be in good condition. Clearances measured as follows:
Upper pintle : 0.3mm ; Lower pintle : 0.6mm
OIL FIRED AUX .BOILER
Above boiler was surveyed. Internal and external fittings and components were examined. No
any indication of defects found.
Port and stbd. safety valves were tested. Both valves were set as follows:
Port : 8.1kg/cm2 ; Stbd. : 8.2kg/cm2
Instrumentations and alarms were also tested and found in good working order.
EXHAUST FIRED AUX BOILER
An internal inspection of the above boiler was carried out with satisfactory result. The
standard condition for testing of safety valves and instrumentatios was given as follows:

Given condition of class due 98.11.04
Ref.13 Exhaust Fired Aux. Boiler. Before taken into use, safety valves to be adjusted by Chief
Engineer. Results to be recorded in the engine log book. Confirmation to be forwarded to
DNV Head Office.
CONTINUOS MACHINERY SURVEY
The following defects were found while carrying out inspection:
- Groves (abt. 10mmL x 3mm depth ) were found in the internal surfaces of main engine
cylinder cover no.1 and no.2. They were send to engine's manufacturer(B&W) for repair.
- Intermediate shaft bearing was found wear down at the aft side. It was then remetalled and
reused.
- All batteries(12 nos.) for GMDSS radio cracked at the upper casing. The radio is now
fitted with new batteries.
- Combustion chamber for separate gas generator of inert gas system found cracked at the
casing. It was then reinstalled with new chamber. On completion of reinstallation , the
generator was satisfactorily tested.
SAFETY EQUIPMENT CERTIFICATE, RENEWAL
Vessel equipped with free fall lifeboat. The last launching was recorded on 98.02.18. While
vessel in the yard, lowering test was satisfactory carried out.

SAFETY RADIO CERTIFICATE GMDSS, RENEWAL
Radio survey was carried out by Radio Holland B.V. , Singapore in Sembawang Shipyard.
All radio equipment checked and found to be working in order. No any defiencies reported.
IOPP CERTIFICATE, RENEWAL
Condtion of class was postponed since vessel found not engaged in the trade of carrying crude
oil.

With regard to onboard inspection during discharge/COW operation.
RESULT OF SURVEYS
Upon satisfactory completion of all surveys and repairs, certificates endorsed and validity
extended to 1999-01-04.
