1.0 INTRODUCTION

Nova Consulting Group, Inc. (Nova) provided on-site air monitoring during the removal ofasbestos-containing materials located in the First Floor West Hallway at the MaxfieldElementary building, 380 North Victoria St., St. Paul, MN 55104.

Removal was conducted using full enclosure and glove bag asbestos abatement techniques. Allasbestos-related work was performed in accordance with Federal, State, and Local asbestosregulations.

Asbestos-related work began on August 2, 2004 and was completed on August 16, 2004. MavoSystems, License #AC200, conducted the asbestos abatement.

Sample collection and analysis was completed by Megan Broberg, License #AS8988, andMichael Muff, License #AS8372, of Nova Consulting Group, Inc., 1107 Hazeltine Boulevard,Suite #400, Chaska, MN 55318.


2.0 ENCLOSURE SUMMARY

3.0 AIR MONITORING AND SAMPLE ANALYSIS

On-site air monitoring was provided by Megan Broberg, License #AS8988, and Michael Muff,License #AS8372. PCM sample collection and analysis was completed in accordance withNIOSH Method 7400. Quality control procedures were followed as required by this method.Air sample analysis results can be found on the air sample summary table located in theappendices.


4.0 WASTE TRACKING

All asbestos waste containers were labeled with the required EPA, DOT, and OSHA labels. Thefollowing waste containers were removed from the project site:

tiles. Nova signs off on the list.9:15 AM The supervisor states that there will be plywood laid above the black iron above the ceiling for additional support

to prevent ceiling plaster damage.9:30 AM Two decon cubes have been constructed, but not connected.10:30 AM All decon cubes, including the shower unit, have been constructed and attached to the containment.11:00 AM The first poly floor has been completed. The crew takes lunch.12:00 PM The crew returns from lunch. They begin laying the second poly floor.2:00 PM The portion of the containment below the plaster ceiling is completed. precleaned as the other two continue precleaning, working from North to South.11:00 AM The crew takes lunch break. The supervisor estimates they will be precleaning and fitting plywood above the

ceiling well into tomorrow.12:00 PM The crew returns from lunch, resuming precleaning and laying plywood.1:15 PM The containment above the ceiling is now precleaned. Two of the crew are now beginning to put up criticals on

one side while the supervisor continues to lay plywood.3:15 PM The crew continues criticaling and laying plywood. The supervisor informs Nova that various pipe runs that are

horizontal to the hallway will also be abated. Much of this will be outside of the containment and will abatedwith glove bags. Approximately half the plywood has now been laid. The crew begins cleaning up the area.

3:30 PM Mavo and Nova leave site.

been performed, but Nova estimates it to be at least another 100 LF.11:00 AM The crew takes lunch.12:00 PM The crew returns from lunch. Two of the crew resuit up with Tyveks and respirators to continue criticaling.

The supervisor now estimates that it may be Tuesday until they can begin abatement.2:00 PM The laying of the plywood above the ceiling is nearly complete. Critcaling of the west side of the containment

continues.2:45 PM The supervisor informs Nova that abatement of the pipe runs perpendicular to the hallway will be flush with the

hallway itself. Due to construction of the ceiling, separate containments would be needed to go beyond thesepoints.

3:15 PM The crew begins cleaning up.3:30 PM Mavo and Nova leave site.

he is to receive two new crewpersons on Monday.10:00 AM A three stage bagout is being attached to the south side of the containment.11:00 AM The crew takes lunch. They have mostly completed the inside and freestanding criticals above the ceiling.12:00 PM The crew returns from lunch. They continue working on the criticals.2:00 PM The crew has finished the wall criticals above the ceiling and is beginning to lay the floor critical.3:00 PM The floor critical is nearly half laid. The crew begins to clean up.3:30 PM Mavo and Nova leave site.








Log EntriesTime Description7:30 AM Nova arrives on site. Mavo now has a crew of five, with two new people. They are continuing to prep the

containment. The first floor above the ceiling is approximately half completed. One person is constructing glovebags above the ceiling on the north end of the hallway.

8:00 AM Tubing is being hooked up to the HEPA filters to exhaust externally into in inner courtyard on the north side of the containment.

8:30 AM Five glove bags are being constructed above the ceiling on the north side of the containment, outside of thecontainment proper. All are in the same proximity. Nova sets up equipment to run adjacent samples.

9:00 AM The five glove bags are inspected by Nova. The corner of one was found to have fallen and was asked to berepaired. This was done. Adjacent samples are started and a crew person begins abatement. Poly flooringhas already been laid below the glove bag areas. The crew person working on the abatement is donning aTyvek and respirator. The decon unit is now fully functional.

9:15 AM Nova requests a danger sign posting for the hatch entrance to the glove bags. This is done.11:15 AM All five glove bags have been completed. Nova inspects each, with no problem found.11:25 AM Adjacent samples for the glove bags are pulled.11:30 AM The crew takes lunch. Approximately half of the first floor for the enclosure area above the ceiling has been12:30 PM The crew returns from lunch. The glove bag adjacent samples are analyzed, with both reading at .01 f/cc.1:00 PM The Mavo supervisor reports that the first poly floor above the ceiling has been laid. They have now started

laying the second poly floor.3:00 PM One personal sample of Mavo's was analyzed and returned. The crew has completed laying the second floor

and is now putting in the walls on both sides above the hallway.3:15 PM The supervisor predicts they will be ready to begin abatement around noon tomorrow. The crew completes

final cleaning.3:30 PM Mavo and Nova leave site.

A manometer and viewing window have been attached.9:15 AM Nova is informed the containment is ready for the pre-abatement visual. No problem is found.9:30 AM The manometer is activated, along with the two HEPAs. The initial reading is -.033. The crew begins abatement.9:45 AM Nova is asked to look at some window sill insulation and caulk on the second floor of the west wing. They

were told that these do constitute suspect materials. The window insulation, located under metal frames,contains black tar paper pieces with a brown wooly material attached. They decide not to install any windowsuntil the suspect materials are analyzed.

10:00 AM Bulk samples of the caulk and window insulation are taken by Nova and processed as bulk samples to besubmitted.

The crew has already removed all the wrap and cut piping from the containment.1:50 PM The first set of adjacent samples is pulled and replaced with a second set.2:15 PM The crew begins bagging out waste. It is observed to be double-sealed, wet, and goosenecked. They have

decided to store the waste in the gymnasium on top of poly sheeting. The supervisor states that at the end ofthe day, they will also put poly over the bags.

2:45 PM The first set of adjacents is analyzed, with both reading at under .01 f/cc.3:20 PM The crew begins cleaning up. They reportedly have about half the pipes abated. Nova pulls the second set of

adjacents.3:30 PM Mavo and Nova leave site.

All waste is being stored on top of poly sheeting in the gymnasium until

tomorrow. The crew begins final cleaning.10:00 AM A meeting is held on site involving Ted Sherman of St. Paul Schools, Nova Project Manager Brian Meyer, and the

window contractor representatives. The three window caulk bulk samples submitted the previous day takenfrom the second floor of the west wing were found to contain asbestos. The cellulose and tar paper insulationinside the window frames was found, however, to be negative.

10:30 AM The Mavo supervisor was asked by Ted Sherman if they could take out four corner strips of caulking on the first and second floors of the west wing. This was agreed to. An additional bulk sample is being submitted oflinoleum and mastic on top of the perimeter vents near the windows. If this tests positive, they may have to beremoved under full containment.

10:45 AM Meeting personnel leave site. The first set of adjacent samples are pulled.11:00 AM The crew changes out for lunch. The supervisor states that they will not be ready for a final visual until well

after lunch, and thus will not have time to run finals until tomorrow morning.11:15 AM The crew takes lunch. The first set of adjacent samples are analyzed, with both reading at well under .01 f/cc.12:15 PM The crew returns from lunch. The second set of adjacent samples is started.2:00 PM Mavo indicates they are ready for a final visual.2:10 PM Nova enters the containment for the final visual.2:30 PM Asbestos splatter is found on the ceiling on the north end of the containment. The crew immediately sets upon

cleaning it up. Debris was also pointed out at many spots along the length of the containment above and belowthe ceiling on the floors. A few holes in the walls and floors were also pointed out and repaired. A few pipethreads were also found to need further cleaning.
