The NRC staff has recently been engaged in a series of discussions with reactor vendors, EPRI, and the Steam Generator Owners Groups concerning steam generator operational problems. Central to these discussions is an accurate assessment of operational conditions and experiences as well as the programs aimed towards the resolution of these problems.
In order to ensure that both the NRC and the nuclear industry have available a comprehensive collection of operating data for steam generators to permit informed, timely decisions and actions, DOR is establishing a steam generator information system. The system will collect appropriate information from all PWR licensees which will periodically be published. It is presently anticipated that the initial publication of information will be in the earlyi part of 1978. You will be sent a copy of this and all future publications.
This information system will enable the NRC and each Licensee to draw from the operational experiences of the entire nuclear.industry when making any decisions concerning steam generators. This should result in both safety and economic benefits.
Enclosed is a questionnaire which we request that you complete for each of your operating PWR units. We believe that the questionnaire is self explanatory, however, if questions arise or any clarifications are necessary, please do not hesitate to contact your NRC Project Manager. Please include with your response any diagrams you may have available which illustrate the tube plugg-ing and/or the tube degradation patterns.
To enable us to maintain the information current, you are further requested to Subm:it in the same format indicated by the questionnaire, any changes or additions to your initial subittal to reflect the future operating experience with your steam generators, i.e., the results of future steam generator inspections. The questionnaire should be completed to the extent applicable and appropriate at this time, i.e. regardless of operating experience.

Gentlemen:
The purpose of this letter is to provide guidance regarding the information required on operator and senior operator license renewal applications relating to participation in the operator requalification program. We have found that vagueness or incompleteness in these applications has, at times, caused delay in their processing.
Section 55.33(a)(4) of 10 CFR Part 55 requires that the applicant provide "A statement that during the effective term of his current license the applicant has satisfactorily completed the requalification program for the facility for which operator or senior operator license renewal is sought." "Satisfactorily completed" means that the licensee has been administered the annual examinations and has participated in the lecture series, as required, that he has participated in the on-the-job training; has been evaluated by supervision regarding his performance during abnormal and emergency situations; and has participated in all the other portions of the approved requalification program.
Section 55.33(a)(4) was worded in this manner to preclude the need for the applicant to furnish the details of his participation in the program in .his application for license renewal, since it was expected that he would be participating in the program as submitted by the facility licensee and approved by the NRC.

In order to avoid this office having to request additional information in conjunction with renewal applications, we request that individuals who have completed all portions of the program use the term "satisfactorily completed the requalification program," in the renewal applications. Terms such as "actively engaged," "engaged in," "participating in," should not be used to indicate complete participation.
If the applicant has not completed all portions of the program, those portions not completed should be enumerated in the application for license renewal. In addition, facility management should indicate what action has been taken, or is to be taken, to compensate for the nonparticipation. We recognize that exigencies may arise that cause an operator to miss participation in some aspect(s) of the program and these will not necessarily bar renewal of the license without reexamination. However, all such events must be identified and justified.
Based on our evaluation of this information, and the other information required by Section 55.33(a), we will be better able to make the determination to renew the license with or without reexamination.

ACRS (16) By your application dated Augist 31, 1977, you submitted a document each for Oconee 1, 2, and 3 entitled Response to NRC Request for Information on Reactor Vessel Material Surveillance Program and requested that it be withheld from public disclosure pursuant to 10 CFR 2.790. Your letter also enclosed an affidavit by Mr. James H. Taylor of the Babcock & Wilcox Company (ONW) dated August 12, 1977, in support of the withholding request.
B&W's reasons for requesting that the data be. withheld from public disclosure were that the information was developed at considerable cost to them over a three year period; it is not available in open literature or el sewhere on the open market, and the market potential for this technology has a significant dollar value; therefore, its disclosure is likely to cause substantial harm to B&W's competitive position.
We have reviewed your application, the material, and the.affidavit based on the requirements and criteria of 10 CFR 2.790 and-have determined that the above-mentioned documents sought to be withheld contain trade secrets or privileged commercial or financial information.
We have also found at this time that the right of the public to be fully apprised of those plant-specific proprietary details regarding the reactor vessel raterial surveillance program does not outweigh the demonstrated concern for protection of B&W's competitive position. Considerable details on the subject program are available in the Public Document Room in Topical Reports BAW-10006 (Revision 3 dated October 1973) and BAW-10100A (dated February 1975) entItled "Reactor Vessel Material Surveillance Program."

We therefore approve your reauest for withholding pursuant to Section 2.790 of TO CFR Part 2 and are withholding the information on reactor vessel -material surveillance program from public inspection as proprietary.
Withholding from public inspection shall not affect the right, if any,' of persons properly and .directly concerhed to inspect the documents. If the need arises, we may send copies of this information to our consultants working in this area. We will, of course, assure that the consultants have signed the appropriate agreements for handling proprietary data.
If the basis for withholding this information from public inspection should change in the future such-that the information could then be made avail-able for public inspection, you should promptly notify the NRC.

In recent months we have observed that some NRC licensees are using a variety of addresses for items of correspondence directed to the Office of Nuclear Reactor Regulation. This nonconformity has the potential for causing administrative delays in the handling and disposition of incoming correspondence and therefore the processing of any licensing action indicated.
Your attention is invited to 10 CFR 50.30(a) which states, in part: "Each application for a license, including whenever appropriate a construction permit,.or amendment thereof, should be filed, for a nuclear reactor, testing facility or other utilization facility, with the Director of Nuclear Reactor Regulation, U. S. Nuclear Regulatory Commission, Washington, D. C. 20555". Part 50.4 requires the same address for communications and reports concerning Part 50 regulations unless otherwise specified. Regulatory Guide 10.1 "Compilation of Reporting Requirements for Persons Subject to NRC Regulations" specifies addresses and desired number of copies for most required reports other than those identified in 10 CFR 50.30(a).
It is hereby requested that all of your future correspondence directed to the Office of Nuclear Reactor Regulation and its component be addressed in accordance with 10 CFR 50.30(a), or as specified in the Regulatory Guide.
