Mission Statement The Mission of the Department of Otolaryngology–Head and Neck Surgery, College of Medicine, Faculty of Health Sciences, University of Manitoba is to provide professional leadership in all aspects of Otolaryngology – Head and Neck Surgery through its teaching,

research, and by providing exemplary clinical care. Clinical Care The clinical mission of the Department:

Provide Manitobans with compassionate, evidence based care with the best possible clinical outcomes in the clinical areas of general otolaryngology, neurotology, pediatric otolaryngology, rhinology, communicative disorders; sleep disordered breathing, facial plastic and reconstructive surgery, and head and neck oncology.

Play an active role in health care management and advocacy as they pertain to the specialty.

Provide care to remote communities in Northern Manitoba and Nunavut through the Northern Medical Unit.

Education The general educational mission of the Department:

- Provide fundamental skills and a basic understanding of otolaryngology to undergraduates, in-depth training of postgraduates, tailored education for allied health care professionals, and an environment of continuing medical education in Otolaryngology – Head and Neck Surgery.

The primary undergraduate educational goals:

- Provide fundamental knowledge and skills to medical students such that they are prepared to enter postgraduate training in the field of their choice and to introduce them to Otolaryngology – Head and Neck Surgery as a potential career.

The primary postgraduate educational goals:

- Provide residents with broadly based, in-depth training that will enable them to enter general Otolaryngologic practice, or enter advanced training in any of the

Otolaryngologic subspecialties. The Department provides a safe, collegial, academically stimulating environment for staff and trainees. Educational goals will be accomplished within the framework of the College of Medicine, Faculty of Health Sciences, University of Manitoba mission statement, and the accreditation standards of the Royal College of Physicians and Surgeons of Canada.

Research The research mission of the Department is to produce scientific work that improves outcomes for our patients, improves the educational process for our trainees, and improves the administration and delivery of health care

Department Head Report Dr. P. Kerr . It gives me great pleasure to report on our department’s activities for 2015! The Dept. of Otolaryngology-Head and Neck Surgery, University of Manitoba continues to provide top level care to Manitobans, including hearing restoration through cochlear implantation; management of nasal and sinus disorders with image guided endoscopic surgery; treatment of tumours of the head and neck with minimally invasive techniques such as endoscopic skull base surgery & laser micro-surgery; correction of voice and swallowing disorders using the latest techniques and materials; and advanced multidisciplinary care of pediatric ear nose and throat disorders. We provide leadership to the Medical College, University of Manitoba in advancing medical student education, and continue to train surgeons at the highest levels in a fully approved Royal College of Physicians and Surgeons of Canada residency training program. Our areas of research activity include functional outcomes in head and neck cancer, management of hearing loss and dizziness, and advancement of clinical training through our Robotics and Simulation Laboratory. I’d like to extend the warmest of thanks to all my colleagues and our support staff for their outstanding efforts over the past year. Some of the highlights for 2015 include: Clinical Program

Dr. Andrea Darnbrough was recruited after she completed a fellowship in Laryngology at McGill University, Montreal. Through her leadership and expertise we now offer a variety of minimally invasive procedures to patients with voice and swallowing disorder. Drs. Darnbrough and Donna Sutherland have established the Tracheostomy Service at the Health Sciences Center. This service now offers support in the management of complex airway disorders, and has added the capability to perform percutaneous bedside tracheostomy in the ICU setting.

Dr. Donna Sutherland established 7-Oaks Hospital as a satellite site for the surgical care of parathyroid disease in dialysis patients. This has permitted these patients to get the care they need in their own dialysis unit under the care of their usual Nephrologist, thus improving continuity of care.

The Surgical Hearing Implant Program celebrated its 100 cochlear implant in fall of 2014 and the Cochlear Implant team has had continued success in 2015, completing the requested volume of cases under the requested time and under budget. Our department has asked for this program to be expanded to reduce the wait list for patients with profound hearing loss to acquire implantation. Congratulations to Justyn Pisa who administers the SHIProgram, and to Drs. J. Hochman, L. Garber, and D. Leitao.

Drs. Eric Meen, F. G. Osler, and Richard Gall have consolidated all the advanced endoscopic sinus surgery clinics and surgery at the St. Boniface General Hospital with the support of the hospital, and WRHA. This helps in improving efficiencies and maintaining quality within this program. Thanks to all involved.

Dr. Jodi Jones has commenced endoscopic salivary gland surgery in children and adults, making Winnipeg one of the earliest adopters of this technique in North America.

Postgraduate Medical Education

The Head and Neck Surgical Oncology Fellowship within our Department graduated its first surgeon Jun. 30/2015. Congratulations to Dr. M. Alessa who has since returned to Riyadh, Saudi Arabia, where he has joined the King Saud University Head and Neck Surgical Oncology team.

Undergraduate Medical Education

Dr. Adrian Gooi continues to provide leadership within the Department, the College of Medicine, and the Canadian Society of Otolaryngology - Head and Neck Surgery, advancing the education of medical students. He continues to be recognized by the students as one of the top educators in the College.

Research

Dr. Brian Blakley continues to serve as our Director of Research. Our primary areas of focus remain hearing loss, functional outcomes after management of H&N cancer, and robotics and simulation.

Dr. Jordan Hochman continues to publish and establish inter-institutional collaboration with his Robotics and Simulation Laboratory at the HSC.

Dr. F. G. Osler chaired the Local Organizing Committee of the Canadian Society of Otolaryngology – Head and Neck Surgery, culminating in one of the most successful national annual scientific meetings in the CSO-HNS’s history. Through her organization, our Department, Winnipeg and a variety of venues such as the Canadian Human Rights Museum, Assiniboine Park Pavilion Events Center shone. Local author Frank Albo provided an enthralling historic backdrop to the meeting. Local athlete Jennifer Botterill provided the injection of inspirational speaking to cap off the meeting. Wow, what great exposure for our program, the University of MB, and Winnipeg!

Educational Reports Postgraduate Director Report Dr. D. Sutherland The Otolaryngology Head and Neck Surgery Residency Program at the University of Manitoba remains fully accredited by the Royal College of Physicians and Surgeons of Canada. There are ten active residents in the Program, with three senior residents set to write the Royal College Examinations in the spring of 2016. The Annual Resident Research Day was held in May 2015 with Dr. Sam Daniels as the visiting professor. Residents also participated in the Annual CSO Conference which was held in Winnipeg this past June. For the next academic year we will be accepting one visa trainee into the Residency Program along with one physician which will be filled through the CaRMS interviews that are scheduled for February 5th, 2016. The University of Manitoba continues to develop their online scheduling and management system, Ventis. Otolaryngology is positioned to move to completing all assessments online through Ventis in the near future. The other ongoing development is the switch to Competency Based Medical Education under the direction of the Royal College of Physicians and Surgeons. A Committee has been struck and has met three times in Ottawa to develop the structure and evaluations required for Competency Based Medical Education. This Committee has consisted of the Nucleus Committee and Program Directors with additional stakeholders. Doctors Kerr, Sutherland and Osler have represented Manitoba at these meetings. The next meeting is scheduled for the spring of 2016 and the role out of Competency Based Medical Education is planned for the 2017 academic year.

Undergraduate Director Report Dr. Adrian Gooi

2015 was an event filled year with lots of activity in the following areas: Education

Transition to new UGME curriculum, all 4 years are now the in new curriculum

Incorporation of ENT into “Clerkship Academic Half-days” with very positive feedback, however, this might be removed in the future with the new curriculum.

ENT face-to-face time continues to be reduced, thus requiring different educational delivery methods

Full use of flipped classroom and interactive learning in instructional planning.

ENT continues to be a leader in innovation and online learning in UGME Innovation

Examined student and faculty attitudes towards live streaming

Examined best modality for assigned studies

Interprofessional initiatives with education sessions for Rural Family Medicine, Urban Family Medicine, Northern Family Medicine, Emergency Medicine, Dentistry, and internal Medicine. Very positive feedback on all sessions.

Scholarship

Published and presented on e-learning, student-generated question bank, Medical School 2.0, audience response systems, and point-of-view cameras for medical education.

Our Department continues to play an important, innovative role in undergraduate curriculum reform I the University of Manitoba. Thank you to all of those who contributed to Undergraduate Education in the Department in 2015!

Research Director Report Dr. Brian Blakley The Department’s Annual Research Day was held May 9, 2015. There were sixteen presentations, 3 students and 13 residents’ presentations. Our visiting professor was Sam Daniel from Montreal. The research output of the Department has been stable but at a low level for the past several years. As a result the Research Committee, in coordination with other Departmental Committees, has made attempts to improve the quality and quantity of research in the Department although Otolaryngology is still primarily a clinical department. Some of these efforts that I am aware of include:

More rigor for supervision of resident research projects.

Residents must identify a project and mentor in October

Research projects must be approved well in advance of Research Day. It is not clear at present if these new initiatives have impacted the residents’ enthusiasm for research.

Dr. Srinathan (Viallet) Improvements in research output are seriously impaired by growing need for forms and deadlines from various sources. There are hopes that a Research Assistant or similar position can be established for the Department to cope with these needs. The main research in the Department is currently being conducted by Drs. Hochman, Gooi and Blakley although Dr. Viallet has started several new projects.

Educational Reports

Director of Continuing Professional Development

Dr. Jodi Jones This year continued to be a great year for CPD in our department. The CSO-HNS Annual Meeting was held in Winnipeg in June, 2015. It was very well attended and was a great success. The theme of the meeting was “Taking care of your patients and yourself – work/life balance.” The Department of Otolaryngology at the University of Manitoba was well represented with numerous posters, podium presentations, workshops and plenary sessions. An airway simulation course, as well as a Cosmetic Botulinum Toxin workshop was also run with the conference. Congratulations to all in the Department for an incredibly successful meeting. Dr. Sam Daniel, Pediatric Otolaryngologist from McGill, was our guest professor for Research Day and he presented some excellent talks on Sialendoscopy and management of salivary gland disease. We also conducted a hands-on Sialendoscopy workshop for the residents. In view of the CSO-HNS meeting being held in Winnipeg, we did not hold the Prairie Otolaryngology Weekend this year. We are in the early planning stages for the 2016 meeting to be held at Elkhorn Resort, Spa and Conference Centre in Onanole, MB. We have been very fortunate to receive many unrestricted educational grants to continue to run this event. Dr. Derek Puddester from the University of Ottawa visited our Department on September 24, 2015 to do a workshop on Time Management with the resident group. Dr. Joseph Dort, Otolaryngologist from the University of Calgary, visited our Department on September 17, 2015, to present “Transforming Healthcare Delivery in Head & Neck Surgery.” Dr. Dort is a former graduate of the University of Manitoba Otolaryngology-Head & Neck Surgery Residency Program and was the recipient of the Royal College of Physicians and Surgeons Janes Travelling Fellowship.

St. Boniface Hospital

Section Head Report

Dr. F. Gigi Osler The Section of Otolaryngology-Head and Neck Surgery (Oto-HNS) at St. Boniface Hospital (SBH) provides care to patients in the emergency department, outpatient clinics, and inpatient units. In 2015 there were over 4,000 patient visits to the Oto-HNS outpatient clinics and over 1,100 patients treated in the main operating rooms for Oto-HNS procedures. SBH has the province’s only CT-navigated sinus surgery system and the rotation continues to provide outstanding Rhinology teaching to the residents under the expert guidance of Drs. Richard Gall, Eric Meen and Gigi Osler. Dr. Mark Barker continues to provide the residency program with Laryngology teaching and clinical expertise. Drs. Kerr, Sutherland and Viallet continue to provide excellent head and neck surgical oncology teaching with many of Manitoba’s head and neck surgical oncology procedures performed at SBH. The hospital was first established in 1871 and has a long tradition of excellence and caring. SBH strives to provide Manitobans with the highest quality health care possible by anticipating and responding to community needs. As a leader in patient care, research, and education, the hospital maintains the highest ethical standards, respects life and human dignity, and is committed to the populations it serves. St. Boniface Hospital is a tertiary care facility, a teaching hospital, and the second busiest hospital in Winnipeg. The hospital campus is comprised of more than 180 departments and services, which are located in seven separate buildings and situated on 20 acres of land. It also operates several satellite facilities and maintains close relationships with other health facilities as well as the health agencies of the Grey Nuns network.

Surgical Hearing Implant Program

The Surgical Hearing Implant Program had an exciting year. By the end of the fiscal year for 2014, we were able to perform the maximum number of surgeries allowed by our program, for both cochlear implants and bone anchored hearing aids. This year concluded the first negotiated contract by our program. Together with WRHA Logistics Department, a working group was formed to participate in a new RFP for hearing devices for the next 3-5 years. New contracts have been awarded, with a strong focus on value to patients. Improvements in technology and surgical technique continue to allow us to provide patients cutting edge technology through more minimally invasive techniques. This year also marked progress in the development of our Bone Anchored Hearing Program. Through collaboration with WRHA and Central Speech and Hearing Clinic, we are planning to consolidate services to provide outpatient clinic care in a community setting. This will improve patient wait times and access, and improve our ability to service patients’ needs. The program continues to work towards the development of a comprehensive database to allow us to track patient outcomes.

Through collaboration with the WRHA and Central Speech and Hearing Clinic, we are planning to consolidate series to provide outpatient clinic care in a community setting. This will improve patient wait times and access, and improve our ability to service patients’ needs. The program continues to work towards the development of a comprehensive database to allow us to track patient outcomes.

Children’s Hospital Section Head, Pediatric Otolaryngology Dr. D. Leitao The Section of Pediatric Otolaryngology is within the Division of Pediatric Surgery at the Winnipeg Children’s Hospital. Our section is comprised of 7 pediatric ENT surgeons. Outpatient clinical activity occurs both at Children’s Hospital, as well as in community ENT practices in a private setting. Outpatient ENT clinics are held at Children’s Hospital with Dr. Leitao and Dr. Jones four days per week, and are among the highest volume clinics at the hospital. In addition to consultation visits, the Pediatric ENT clinic hosts a large volume of TeleHealth consultations, providing remote areas with specialist assessment without the need for travel to Winnipeg. The Pediatric ENT service operates 5 days per week at Children’s Hospital. Low risk surgical procedures are also performed at Maples Surgical Centre, which allows us to improve our surgical wait times for children and their families. Over the past year, the Pediatric ENT service continues to bring surgical innovations and minimally invasive surgical techniques to Children’s Hospital. Our service has pioneered endoscopic ear surgery in Manitoba. Using specialized telescopes and dedicated endoscopic surgical instruments, we are able to perform complex operations completely through the ear canal, minimizing scars and improving patient comfort. These advances have also been seen in endoscopic airway surgery, where our surgeons are advancing the ability to reconstruct the pediatric airway without neck incisions. New surgical instruments dedicated to these tasks have recently been obtained, and have been integral to our ability to provide cutting edge surgical approaches to the population we serve. Universal newborn hearing screening is soon to be announced in Manitoba in 2016. Over the past year, we have continued to liaise with the various stakeholders, and have been a part of the discussions and planning with government as the process moves forward. Our service will continue to plan for supporting the wave of children that will be identified early with hearing loss. We will plan to ensure that the hearing loss assessments, counseling, and needs of patients, families, and health care providers are met.
