Planning & Transformation
Talk to clinicians, patients and
commissioners to identify issues
Identify common issues
Map out a patient story that brings this
to life
Identify the common themes emerging Define as is and to be Define the benefits of
doing things differently
Validate with clinicians, patients and community of
interest
Publish each localities key challenges
Check for example scenarios
Publish developed storylines and gain feedback from the
community

“One of my patients came to see me last week, 2 stone lighter(!) and wielding her Mapmydiabetes
plan! She had started connecting the dots between her numbers and her lifestyle, had made some
changes, and had come to see me to discuss how to proceed with HER plan! I was delighted - she was so‘in charge’, and using me as an advisor, not the
person who she wanted to‘fix’things for her”

Lessons learnt from digital implementations
Oxfordshire (South) has successfully piloted e-consultation with very positive feedback from patients and clinicians Richmond has successfully implemented extended access to primary care via 4 Hub practices; the technology solution allows practices to
book Hub appointments and for Hub clinicians to have access (with patient consent) to patient records Peterborough has used technology solutions to enhance their unscheduled care pathway through creating a primary care walk-in centre to
triage patients prior to going into A&E and enabling NHS 111 staff to book primary care appointments. They have also offered GP telephone calls and e-consultation to reduce demand on A&E and improve access to primary care
Video consultation is being piloted to improve access to primary care with care homes. Peterborough is using the camera on a smart phone to promote e-consultation on dermatology cases
It is important to align the work linked to the development of Local Digital Road Maps with the creation of the Sustainability and Transformation Plans
To get the best out of your technology supplier / a procurement process, remember the key stages of this process!
Key lessons from pilot sitesThere are a growing number of examples of collaboration, for example where practices have collaborated to improve patient experience and access using technology to deliver primary care at scale, improve unscheduled care pathways and promote digital consultation
