Deep Fascia and Triangles of the Neck

DEEP FASCIA AND TRIANGLES OF THE NECK

A. STERNOCLEIDOMASTOID MUSCLE อยูที่ดานขางของคอ ทอดตัวอยูในแนวเฉียง สวนกลางแคบและหนา สวนปลายทั้งสองกวางและแบนกวา Origin มี 2 heads คือ 1. Sternal (medial) head เปน tendon เกาะที่ anterior surface ของ manubrium sterni 2. Clavicular (lateral) head เปนกลามเนื้อเกาะที่ medial one- third ของ clavicle Muscle fibers ทอดขึ้นบนเฉียงไปทาง posterior Insertion เปน tendon เกาะที่ lateral surface ของ mastoid process และ lateral half ของ superior nuchal line ของ occipital bone Nerve supply ไดจาก spinal accessory nerve (CN XI) และ ventral rami ของ C2 spinal nerve

Action - ถาทํางานขางเดียว จะดึง mastoid process ลงลาง เขาหาไหลขางเดียวกัน จึงทําใหคางหันขึ้นบนเฉียงไปยังขางตรงขาม - ถาสองขางทํางานพรอมกัน จะมีผลคือ - ทําใหศีรษะยื่นไปขางหนา และคางเคลื่อนขึ้นบน

- ถาอยูในทานอนหงายจะทําหนาที่ยกศีรษะขึ้น (flex the neck against resistance of gravity)

- ชวยในการหายใจเขา โดยยก clavicle และ sternum ขึ้น เพ่ือเพ่ิม thoracic cavity (forced



inspiration) B. DEEP FASCIA OF THE NECK (CERVICAL FASCIA)

Cervical fascia อยูลึกกวา platysma muscle บางแหงเรียงตัวหลวมๆ และบางแหงหนาตัวเปนแผน ทําหนาที่หุมกลามเนื้อ และอวัยวะอ่ืน ๆ ของคอ รวมทั้งหุมหรือเปนทางใหหลอดเลือด หลอดน้ําเหลือง และเสนประสาททอดผานไปไดโดยงาย นอกจากนี้ยังทําใหอวัยวะตาง ๆ ของคอเคลื่อนไหวไดโดยสะดวก เชนระหวางกลืนอาหาร และเอ้ียวคอเปนตน คําอธิบายเกี่ยวกับ cervical fascia แตกตางกันในรายละเอียด เพราะวาการเรียงตัวของมันสลับซับซอน และแตกตางกันไปตามวิธีการที่ใชศึกษา บางคนไดจากการชําแหละ บางคนไดจากการทํา section แลวตรวจดูดวยกลองจุลทรรศน บางคนใชวิธีฉีดของเหลวเขาไป และบางคนศึกษาจากการกระจายของการติดเชื้อ เพ่ือความสะดวกในการอธิบาย จึงแบง cervical fascia ออกเปน 6 ชั้น คือ 1. Investing fascia 2. Prevertebral fascia 3. Middle cervical fascia 4. Cervical visceral fascia - Pretracheal fascia - Buccopharyngeal fascia 5. Alar fascia 6. Carotid sheath



1. Investing Fascia เปน fascia รูปทรงกระบอกลอมรอบโครงสรางในลักษณะเหมือนปลอกคอ ใน cross section ของคอ, investing fascia จะแยกออกเปน 2 ชั้น เพ่ือลอมรอบ sternocleidomastoid และ trapezius muscles แตมีบางสวนที่ยังคงเปนชั้นเดียวซึ่งจะปกคลุม anterior และ posterior triangles ของคอ ที่ posterior midline, fascia น้ียึดติดกับ external occipital protuberance, ligamentum nuchae และ spine ของ cervical vertebra ที่ 7 สวนบนสุดของ fascia น้ียึดติดกับ external occipital protuberance, superior nuchal line, mastoid process และ inferior border ของ mandible จาก inferior border ของ mandible, fascia น้ีจะทอดตอเน่ืองขึ้นไปสูใบหนากลายเปน parotideomasseteric fascia ซ่ึงจะหุม parotid salivary gland และปกคลุม lateral surface ของ masseter muscle, สวนของ investing fascia ที่ขึงระหวาง styloid process กับ angle of mandible จะหนาตัวขึ้น form เปน stylomandibular ligament สวนลางของ investing fascia จะกลมกลืนไปกับเยื่อหุมกระดูกของ manubrium sterni, clavicle, acromion และ spine ของ scapula ที่ตําแหนงสูงกวา manubrium sterni ประมาณ 2 ซม. fascia น้ีจะแยกออกเปน 2 ชั้น ไปยึดติดที่ anterior surface และ posterior surface ของ manubrium sterni ระหวางชั้นทั้ง 2 น้ีเกิดเปน suprasternal space ซ่ึงภายในพบ - สวนลางของ anterior jugular veins - Jugular veneous arch - Sternal heads ของ sternocleidomastoid muscles - บางครั้งพบ lymph nodes ที่สวนบนของคอ, investing fascia ยึดติดที่ hyoid bone ดวย จากนี้จึงทอดไปยัง mandible โดยยึดติดกับ fascia ของ anterior belly ของ digastric muscle และปกคลุม submental และ submandibular triangles ใน submandibular triangle, investing fascia ยังเปน sheath ของ submandibular salivary gland 2. Prevertebral Fascia เปน fascia รูปทรงกระบอก ลอมรอบ cervical vertebrae และ associated muscle ของ vertebrae เหลานี้ Prevertebral fascia ทอดขาม midline และอยูหนาตอ prevertebral muscles (longus colli and capitis, rectus capitis anterioris and lateralis) แลวจึงยึดติดกับ tips ของ transverse processes จากนี้จึงทอดตอไปทาง lateral ปกคลุม muscular floor ของ posterior triangle (scalene muscles, levator scapulae, splenius muscle) และ deep cervical muscle อ่ืน ๆ ดังน้ันจึงเปน fascial floor ของ posterior triangle ของคอ ทาง posterior, fascia น้ีเกาะที่ external occipital protuberance, ligamentum nuchae และ spine ของ cervical vertebra ที่ 7



ทาง superior, สิ้นสุดที่ basilar part ของ occipital bone, carotid canal, jugular foramen, mastoid process และ superior nuchal line ทาง inferior ที่ base ของคอ, fascia น้ีทอดตาม prevertebral muscles เขาสู posterior mediastinum ของทรวงอก 3. Middle Cervical Fascia (Fascia of Infrahyoid Muscles) เปนปลอกลอมรอบ infrahyoid muscles, ประกอบดวย 2 ชั้น ซ่ึงใน cross section ของคอจะเห็นเปนรูปครึ่งวงกลม อยูที่ anterior half ของคอ Superficial layer ลอมรอบ sternohyoid muscle และ omohyoid muscle Deep layer ลอมรอบ sternothyroid muscle และ thyrohyoid muscle ที่ midline, middle cervical fascia กลมกลืนไปกับ investing fascia, และทาง lateral กลมกลืนไปกับ carotid sheath ทาง inferior, fascia น้ีทอดลงลางไปอยูหลัง sternum และในที่สุดทอดไปบน pericardium 4. Cervical Visceral Fascia เปน fascia รูปทรงกระบอก ลอมรอบ cervical viscera ไดแก pharynx, esophagus, larynx, trachea, thyroid gland และ parathyroid glands แบงออกเปน 2 สวน, สวนหนาคือ pretracheal fascia และสวนหลังคือ buccopharyngeal fascia 4.1 Pretracheal Fascia เปนสวนของ fascia ที่ปกคลุม larynx, trachea และแยกออกหุม thyroid gland ดังน้ันจึง form เปน sheath ของ thyroid gland ทาง superior เกาะที่ hyoid bone ทาง inferior หลังตอ sternum กลมกลืนไปกับ middle cervical fascia และตอเน่ืองไปกับ fascial sheath ของ aorta และ fibrous layer ของ pericardium ทาง lateral กลมกลืนไปกับ buccopharyngeal fascia 4.2 Buccopharyngeal Fascia เปน external covering สําหรับ upper alimentary tract ปกคลุม buccinator muscle และ pharynx และทอดลงลางไปบน posterior surface ของ esophagus ทาง superior เกาะที่ pharyngeal tubercle ของ occipital bone แลวจึง extend ไปทาง lateral ปกคลุม pharyngobasilar fascia แลวไปยัง medial pterygoid plate และปกคลุม superior constrictor muscle แลวจึงไปยึดติดกับ pterygomandibular raphe, จากนี้จึงแผตอไปทางขางหนาไปปกคลุม buccinator muscle ทาง inferior, buccopharyngeal fascia ปกคลุม middle และ inferior constrictor muscles ของ pharynx. Inferior extension ของ buccopharyngeal fascia หลัง esophagus ไมแนนอนเพราะวาลักษณะที่เปนแผนของ fascia น้ีคอย ๆ หายไป.



Pretracheal fascia และ buccopharyngeal fascia ตอเน่ืองกันตามตําแหนงที่เกาะของ pharyngeal constrictors (คือที่ hyoid bone, thyroid cartilage และ cricoid cartilage) และที่ posteromedial border ของ thyroid gland 5. Alar Fascia เปน fascia แผนบาง ๆ อยูใน retropharyngeal space ระหวาง prevertebral fascia กับ buccopharyngeal fascia, สิ้นสุดที่ประมาณระดับ cervical vertebra ที่ 7 โดยยึดติดกับ buccopharyngeal fascia ต่ํากวาระดับน้ีตลอดแนว midline ก็ยังยึดติดกับ buccopharyngeal fascia อีกดวย ทาง lateral, alar fascia กลมกลืนไปกับ carotid sheath 6. Carotid Sheath เปน connective tissue ที่คอนขาง loose, extend จาก base ของ skull บริเวณ jugular foramen และ carotid canal ลงลางไปยัง root ของคอ Carotid sheath มีลักษณะเปนปลอกรูปทรงกระบอกลอมรอบ structures ดังตอไปน้ี - internal และ common cartoid arteries (อยูทาง medial) - internal jugular vein (อยูทาง lateral) - vagus nerve (อยูทาง posterior ระหวาง artery กับ vein) Posteromedial ตอ carotid sheath พบ sympathetic trunk, fibers ของ carotid sheath ตอเน่ืองไปกับ deep cervical fascia อ่ืน ๆ ดังน้ันจึงเปนชองทางที่แฝงอยู (potential space) ที่สําคัญสําหรับการแพรกระจายของการติดเชื้อจากฐานกะโหลกศีรษะลงไปยังทรวงอกได C. FASCIAL SPACES OF THE NECK เปน spaces ที่แฝงอยูระหวางชั้นตาง ๆ ของ cervical fascia มีความสําคัญในการแพรกระจายของการติดเชื้อจากศีรษะและคอลงไปสู mediastinum ของทรวงอกได 1. Pretracheal (previsceral) space อยูลอมรอบ trachea ดานหนาของ esophagus 2. Retropharyngeal (retroesophageal / retrovisceral) space อยูระหวาง buccopharyngeal fascia กับ prevertebral fascia, extend จากฐานกะโหลกศีรษะลงลางเขาสู superior mediastinum

3. Danger space เปนสวนหนึ่งของ retropharyngeal space อยูระหวาง alar fascia กับ prevertebral fascia D. TRIANGLES OF THE NECK เม่ือสังเกตบริเวณดานขางคอจะพบเปนรูปสี่เหลี่ยม มีขอบเขตดังตอไปน้ี Superior : Lower border of mandible และเสนที่ลากระหวาง angle of mandible กับ mastoid proces s



Inferior : Upper border ของ clavicle Anterior : Anteromedian line ของคอ Posterior : Anterior border ของ trapezius muscle อาศัย sternocleidomastoid muscle แบงบริเวณดานขางคอ ซ่ึงเปนรูปสี่เหลี่ยมนี้ออกเปนสามเหลี่ยม 2 รูป คือ 1. Anterior triangle of the neck อยูหนาตอกลามเนื้อมัดนี้ 2. Posterior triangle of the neck อยูหลังตอกลามเนื้อมัดนี้ 1. Anterior Triangle of the Neck Boundaries Superior (base) : Inferior border of mandible และเสนที่ลากระหวาง angle of mandible กับ mastoid process Anterior : Anterior midline ของคอ Posterior : Anterior border ของ sternocleidomastoid muscle Apex : Sternum Roof : Investing fascia และ platysma muscle : Cervical branch of CN VII และ transverse cervical nerve อยูลึกตอ platysma muscle อาศัย digastric muscle, stylohyoid muscle และ superior belly of omohyoid muscle แบง anterior triangle ของคอออกเปนสามเหลี่ยม 4 รูป คือ 1. Submental triangle 2. Submandibular triangle 3. Carotid triangle 4. Muscular triangle

1. Occipital triangle มีขนาดใหญ และมีเสนประสาทที่สําคัญ คือ Spinal accessory nerve 2. Subclavian (supraclavicular / omoclavicular) triangle มีขนาดเล็ก สามารถบงชี้บนพ้ืนผิว

ของคอไดโดยตรงกับ greater supraclavicular fossa ภายใน triangle น้ีพบ external jugular vein ทอดขามอยางตื้น ๆ และมี subclavian vessels และ brachial plexus อยูลึก ๆ



อาศัย spinal accessory nerve สวนที่ทอดอยูบน floor ของ posterior triangle (โดยทอดอยูบน levator scapulae muscle) แบง posterior triangle ออกเปน

1. Carefree region 2. Careful region ซ่ึงใน region น้ี จะมี structures ที่สําคัญ ๆ เชน subclavian vessels, brachial

plexus
