It is an open spandrel arch bridge similar in design to the Rainbow Bridge. In 2002, a project to replace the existing structure was engaged, due to the structural deficiencies and narrow width (1 7' 11"). Over several years of public meetings and consultations with the Caltrans Bridge Department it was determined that the Orangevale Ave. Bridge is to be rehabilitated rather than replaced due to its historic significance. The rehabilitation project will include removal and replacement of the east abutment, spandrel columns and deck. Under the historic preservation guidelines, all elements of structure that are removed will be replaced to the existing dimensions and finish. Through the environmental process with Caltrans several environmental issues were identified in the technical studies. Dokken Engineering, having been previously retained for engineering design, assisted Caltrans with the technical studies which identified bats, swallows and the elderberry beetle as protected species to be monitored. The Dokken Engineering biologist will monitor the project interaction with the protected species and the engineers will provide design support and as-built plans for the project. POLICY/RULE Section 2.36.080 of the Folsom Municipal Code states, in part, that contracts for supplies, equipment, services, and construction with an estimated value of $54,883 or greater shall be awarded by the City Council. Environmental Policy Act (NEPA) Categorical Exclusion, a Request for Proposals for the environmental monitoring and design services was not issued.

Dokken Engineering provided a proposal for the environmental monitoring and design support services in the amount of $66,000. The proposal was reviewed by Public Works staff and determined to be reasonable.

FINANCIAL IMPACT

This project is funded by the Highway Bridge Program (HBP), which will reimburse the City 88.53% of the project costs. The total cost of the project is estimated at $2,763,419, a breakdown of which is listed below.

The Orangevale Avenue Bridge Rehabilitation was included in the FY 2015/16 Public Works Capital Improvement Plan budget with a budget of $429,354. Staff is requesting an additional appropriation be added to the Transportation Improvement Fund (Fund 446). An appropriation in the amount of $2,335,646 will be required and the revised project budget will be increased to $2,763,419. Sufficient funds are available in the Transportation Improvement Fund (Fund 446) for this additional appropriation.

ENVIRONMENTAL REVIEW

As part of the Caltrans review and authorization to begin construction, this project was deemed categorically exempt from the California Environmental Quality Act (CEQA) environmental review and compliant with a National Environmental Policy Act (NEPA) Categorical Exclusion prepared by Caltrans.

A RESOLUTION AUTHORIZING THE CITY MANAGER TO EXECUTE AN AGREEMENT FOR CONSULTING SERVICES WITH DOKKEN ENGINEERING FOR

ENVIRONMENTAL MONITORING AND DESIGN SUPPORT FOR THE ORANGE VALE AVE. BRIDGE REHABILITATION PROJECT, PROJECT NO. 9432

WHEREAS, the City of Folsom desires to rehabilitate the Orangevale Ave. Bridge; and

WHEREAS, Dokken Engineering is the Engineer of Record for the project; and

WHEREAS, the environmental monitoring and design support is a component of construction engineering; and

WHEREAS, the City of Folsom has received an Amendment Modification (E-76) from Caltrans, approving the use of $2,174,865 of Bridge Funding from the Highway Bridge Program (MP) funds; and

WHEREAS, the City of Folsom has requested an additional Amendment Modification (E-76) from Caltrans, approving an additional $675,854.02 of Bridge Funding from the Highway Bridge Program (HBP) funds; and

WHEREAS, funds are budgeted and available in the Transportation Development Improvement Fund (Fund 446), in the Orangevale Ave. Bridge Rehabilitation Project No. 9432 for FY 2016/17, and the remaining project budget will need to be appropriated in FY 2015/16, in order to begin the project in April 2016; and

WHEREAS, the agreement will be in a form acceptable to the City Attorney:

NOW, THEREFORE, BE IT RESOLVED that the City Council of the City of Folsom authorizes the City Manager to execute an agreement with Dokken Engineering, in the not to exceed amount of $66,000, for environmental monitoring and design support for the Orangevale Ave. Bridge Rehabilitation Project, Project No. 9432.

NOW, THEREFORE, BE IT FURTHER RESOLVED that the Finance Director is authorized to appropriate in FY 2015/16 Transportation Improvement Fund (Fund 446) the additional amount of $2,335,646 for a total project budget of $2,763,419 and a grant amount of $2,174,865.

Dear Mr. Reed:

Please find attached Dokken Engineering's proposal for design and envimnmental support during construction of the Orangevale Avenue Bridge Rehabilitation Project. This proposal allows for Dokken to support the City and resident engineer as the design firm of record during the construction phase of the project.

The proposed scope of services is described by task in the attached "Exhibit A, Scope of Work - Design & Environmental Support During Construction". These services include Requests for Information (including preconstruction support), Review of Shop Drawings, Preconstruction Surveys, Worker Environmental Awareness Training, ESA Fencing Mark-out, and Preparation of As-Built Plans. The budget for these services is estimated by task and probable staff level on "Exhibit B — Estimated Hours & Cost — Design and Environmental Support During Construction".

We are requesting a budget of $66,000 for these support services and we anticipate these services to be complete by April 2017.

We appreciate the City's resolve and determination to bring this challenging project to construction. It is certainly gratifying after all the hard work for many years to think about the construction being eminent. ORANGEVALE AVENUE BRIDGE REHABILITATION PROJECT CITY OF FOLSOM

EXHIBIT A

SCOPE OF WORK - Design & Environmental Support During Construction

Dokken Engineering (Dokken) will provide design and environmental support to the City of Folsom and the Resident Engineer throughout construction. These services will be billed hourly at the request of the City or the Resident Engineer.

DESIGN CONSTRUCTION SUPPORT

Task 1 Respond to Requests for Information (RFI's)

Dokken will provide on-going consultation and interpretation of contract documents, as requested by the City or Resident Engineer. Written responses will be provided to all requests for information (RFI's), and supplemental or revised drawings may be issued to provide clarity to the Resident Engineer and Contractor.

Attendance at a preconstruction meeting, as well as occasional site visits, is expected to be part of this task, Developing change orders or portions of change orders is also made a part of this task.

Task 2 Review Shop Drawings and Submittals

Dokken will review submittal drawings and stamp them as returned for corrections or approved. Any corrections will be clearly marked in red and returned to the Resident Engineer. The following submittals are anticipated to be reviewed by Dokken:

• Elevation Monitoring of the Arch Ribs • Sample of the Board Form Texture • Utility Hanger Devices • Bearings on Spandrel Columns

Please note that Dokken cannot (by insurance requirements) perform review of bridge falsework, demolition plan, temporary sewer structure, working platforms, crack injection and temporary bridge; unless we are performing the construction management. it is critical that the field engineers overseeing the work review these submittals and procedures.

Task 3 Prepare As-Built Plans

Using mark-ups received from the Resident Engineer and supplemental or revised drawings from addenda, RFI's, or change orders, Dokken will prepare the final record drawings. These drawings will be prepared electronically in Microstation format and provided on a CD and in half size copies on bond paper.

ENVIRONMENTAL CONSTRUCTION SUPPORT

The following is a list of environmental construction support tasks that Dokken will provide to the City of Folsom for the Orangevale Bridge Rehabilitation Project, prior to, during, and after construction.

Task 4 General Construction Support

Dokken will be responsible for general environmental construction support and monitoring as requested at the project site. This task involves periodic inspection of the construction site to ensure all environmental commitments are being adhered to by the City and its contractors. Monitored commitments will include

maintenance of Environmentally Sensitive Area (ESA) fencing, environmental awareness training for construction workers, ensuring revegetation methods avoid invasive/non-native species, etc. Dokken will maintain the Environmental Commitment Record and sign off on tasks once they are completed as well as ensure that measures identified in the 1602 Strearribed Alteration Agreement are implemented.

Task 5 Migratory Bird Treaty Act Compliance

Dokken will coordinate with the City and the Contractor to ensure migratory birds, their occupied nests, and their eggs, are protected as specified in the Migratory Bird Treaty Act. If any vegetation removal is necessary during the nesting season, a Dokken Engineering biologist will perform a nesting bird survey of the specified areas. Vegetation surveyed shall be removed by the contractor within 14 days. If any active nests are identified, a 100 foot no disturbance buffer shall be established around the nest, unless a variance approval is obtained from the California Department of Fish and Wildlife. The appropriate buffer will be maintained until the biologist has determined that the young have fledged_

Task 6 Bat Habitat Assessment and Initial Surveys

Dokken will perform day and night-time surveys as required by a provision of the 1602 Stream bed Alteration Agreement. If surveys are positive for bats roosting in the bridge, additional bat exclusion measures or other mitigation efforts may be required prior to the start of construction activities.

Task 7 1602 Streambed Alteration Agreement — Closeout Survey and Report

Dokken will prepare a closeout report to meet the requirements of Measure 3.3 of the 1602 SAA. This report will include a final survey and photographs of the completed construction site. The report will be submitted to CDFW and will notify them that construction has completed.

Task 8 Revegetation Plan (Optional)

Dokken will prepare a revegetation plan consistent with the requirements of the 1602 SAA and Section 12.16 of the City of Folsom's Municipal Code (relating to Tree Preservation). Implementation of the Revegetation Plan would be performed by the construction contractor.

The following is a list of environmental commitments that Dokken Engineering assumes will be the responsibility of either the City, the Resident Engineer, or the construction contractor.

• Obtain a Storm Water Pollution Prevention Plan (SWPPP) and implement BMPs consistent with the National Pollutant Discharge Elimination System.

• The City will be responsible for submitting an annual monitoring report each year for five years after completion of construction to update CDFW on the performance of revegetation mitigation as required by the 1602 Stream bed Alteration Agreement.
