Overview on Data Submission
• Enroll your team and Organization
• Submit workbooks monthly by October 03, November 07, December 05, January 09 and Feb 06.
• Continue to submit monthly data until 6 months of consistently meeting goals.
• Share run charts with team and other stakeholders
Information Needed
• Team Leader:– First and last name; Phone #; Email;
Site/Location
• Key Organizational Contact and CEO or Administrator:– Salutation; First and last name; Contact Title;
Phone #; Email; Address; City; Province; Phone # and Fax #
– CEO/Administrator Signature
