Vision and core functions

Vision: All children and young people with additional needs who require support from more than one practitioner should

experience a seamless and effective service in which one practitioner takes a lead role to ensure that services are co-

ordinated, coherent and achieving intended outcomes

Act as a single point of contact for the child or

family

Co-ordinate the delivery of

actions agreed by the practitioners

involved

Reduce overlap and inconsistency

in the services received

Common Assessment Framework (CAF)

The Common Assessment Framework (CAF) is a standardised method to assess the needs of children and young people. Principally it highlights that safeguarding and the well-being of children is everyone’s business.

CAF supports all those agencies and individuals who provide a service to children and young people, in essence, the entire children’s workforce.

It asks that, as professionals we identify concerns as early as possible and initiate action. Multi-agency working is encouraged as part of the assessment, to ensure the child, or young person is receiving the services they require.

When should I complete a CAF?

You should complete a CAF if:

You have concerns about the progress of a child or young person

The needs of the child or young person are not clear

The needs of the child or young person can not be met within your agency

What is a Lead Professional?

School Nurse

A Lead Professional is:

The most relevant practitioner to the child or young person’s action plan who has the most appropriate skills

Any of the practitioners who have come into contact with the child or their family (voluntary or statutory)

Not necessarily the first person involved, or who carried out the initial assessment

A practitioner who can be selected by the child or family

The skills of a Lead Professional

Understand implications of the child’s assessment, for example

in relation to risks and protective factors

Knowledge of local and regional

services for childrenand families

Convene meetings and initiate

discussions with relevant practitioners

Work effectively with practitioners from

a range of services

Support and enable child / family

to achieve their potential

Empower child/family to make

decisions and challenge when appropriate

Establish a successful and trusting relationship with

child/family

Strongcommunication skills; diplomacy; sensitivity

Understand boundaries of own

skills and knowledge

Team around the Child (TAC)

A team around the Child (TAC) meeting is a forum for multi-agency workers and parents/carers to discuss and identify the needs of the Child.

The meeting provides an ideal platform to ensure the Child receives the right support at the right time by:

Sharing informationDeveloping an action planNominating a lead professionalAgreeing a method of review

The meeting is centred on the needs andVoluntary engagement of the child/young personAnd parents/carers, taking into account their view
