16. SPECIAL PROFESSIONAL ACTIVITIES
1. Participation in the "Save a Child’s Heart (SACH) " delegation to Ethiopia on
September 17, 2000, for 10 days. The Israeli team consisted of 14 members, two
cardiac surgeons, two operating room nurses, an intensive care unit (ICU) doctor, two
ICU nurses, a pediatric cardiologist, an anesthesiologist, two dentists, a medical
technician and an education coordinator.
Dr. Haim Renart and I, organized the donation of equipments, antibiotic and dental
materials from several companies in Israel, for the patients in Ethiopia.
During this periods we evaluated and treated 79 children before and after heart
surgery at the dental clinic on the first floor of the Zewditu Hospital. We also
performed six procedures in the Operating Room under general anesthesia, and
lectured to local dentists.
2. I regularly treat free of charge children from low socio-economic status,. The
patients are referred by the welfare department in Petach-Tikva.
3. I am the pediatric dentist counselor for Loutus dental clinics, and a
supervisor at the dental clinic of "help and medicine" of the rabbi Firer – free of
charge.
RESEARCH ACCOMPLISHMENTS PAGE
My clinical and basic research complements each other and deals in several main areas in
dentistry for children: early diagnosis and prevention of periodontal disease, prevention of
pain and anxiety and traumatic injuries.
Prevention of periodontal disease in children
In several basic studies we tried to understand the mechanism of the most serious
periodontal disease in children: aggressive periodontitis (Ref 4-5). In clinical study we
have found that the prevalence of aggressive periodontitis among 645 young recruits is
5.9%, 4.3% suffered from localized form and 1.6% from generalized form. Smokers were
at higher risk (ref 20). In a review about aggressive periodontitis by Demmer RT and
Papapanou PN published in Periodontology 2000 Volume 53, Issue 1, pages 28–44, June

Malka Ashkenazi, DMD
2010 this study was cited as "Only a single study fulfilling the inclusion criteria of our
search presented data on aggressive periodontitis ". In order to prevent gingivitis and
periodontitis we evaluated the effectiveness of tooth-brushing by different designs of
toothbrushes and developed a novel index (Ashkenazi index) to evaluate correct tooth
brushing (Ref-19, 31, 32, 36,37,44,45). This index was shown to be easy to implement, to
correlate negatively with plaque index, and to have high reliability In addition we
evaluated the knowledge compliance of children to dental prevention measures and the
reasons for non-compliance (Ref 15, 24, 29,46).
Prevention of pain and anxiety
Pain in dentistry can arise from pain during injection, during operative treatment and post
operatively. We evaluate all of these three components for a new developed technique of
local anesthesia (in which, the local anesthesia is delivered intra-sulculary via a
computerized delivery system) (Ref 16,17,21, 23, 27,43). We have found that intra-
sulcular anesthesia induces low pain and anxiety during injection, high effectiveness for
performing operative treatments and low post operative pain, without the numbness of the
surrounding oral mucosa. More over in a pioneer study we have shown that this technique
does not disturb the development of developing teeth. The reviewer of this manuscript
wrote "As the reviewer and someone who is current in this area of the dental literature on
this particular subject matter, it is my humble opinion this article will become a classic
study on the subject ".
To increase the effectiveness of mandibular block anesthesia we evaluated the Age-
associated changes of the mandibular foramen position in antero-posterior dimension The
MF moves anteriorly and the gonial angle decreases with age. and of the mandibular
angle in dry human mandibles. We found that the MF moves anteriorly and the gonial
angle decreases with age. This study was published in the prestige journal "The anatomic
records" (Ref 12). To further evaluate the clinical application of these finding we
conducted and published the article titled" Mandibular Block Success Rate in Relation to
Needle Insertion and
Location– A Self Report Survey . (48).
Traumatic injuries. We developed a method to evaluate the quality of storage solution
for teeth after avulsion and found a correlation between the clonogenic capacity of the
PDL-cells after storage and the clinical outcomes of the avulsed- teeth after storage in
41
Malka Ashkenazi, DMD
different media (Ref-6-7). Moreover, we have found that adding growth-factors such as
platelet-derived growth factor and insulin growth factor cannot increase the quality of the
storage media unless the cells are stored as long as 24 hours (Ref-8). In a different study
we have found that EMDOGAIN decrease the clonogenic capacity of the cells probable by
increasing their differentiation, and therefore is not effective as an additive to the storage
media. This basic study explains why emdogain is effective in treatment of periodontal
disease but ineffective in treating avulsed teeth (Ref-10).
In a clinical research, we have shown that soldier in a combat training are in higher risk
for dental trauma and a rubber-covered ceramic weapon reduces the incidence of dental
trauma in recruits during combat basic training (Ref 25,30).
To diagnose radiographically correctly development of dental infection associated with
traumatized primary incisors, we set novel radiographic criteria for diagnosing infection in
primary incisor (Ref 13).
Prevention of infection in dental clinic
In two studies we evaluated the prevalence of antibodies against hepatitis A among
general dental practitioners and specialists. We found that pediatric dentists are prone to
be infected by hepatitis A more than the general population, and therefore they should be
vaccinated. Regarding hepatitis C virus, we have found that dentists in Israel are not in
higher risk that the general population, probably because their compliance to infection
control means. (Ref 9, 11, 28).
