CURRENT PRACTISE
Knee Specialist (Arthroplasty, Soft Tissue & Trauma)
My clinical duties involved six sessions of Orthopaedic Trauma and General Orthopaedics with emphasis on Lower Limb particularly Knee Surgery.
I run a comprehensive knee service unit, which involves performing almost all the Complex Revision Knee Replacement and Revision Ligament Reconstruction in the Department. I do a fair number of Primary Knee Replacements per year.
My routine theatre list involves at least two joint replacements together with ligament reconstruction case and at least two knee arthroscopic procedures per list.
My experience in knee arthroplasty involves the use of different types of primary prostheses. This includes total condylar, unicopartmental (fixed / mobile bearing) replacements, and also patellofemoral replacement.
I have a wide experience with different types of revision prostheses including posterior stabilised super stabilised rotating hinge and constrained prostheses.
During my work as an exclusive Knee Arthroplasty Surgeon I have dealt with wide range of different makes including Triathlon, Kinamax, Scorpio, PFC, Optetrak, Oxford and Avon and others.
I have some experience with Minimal Invasive Surgery (MIS) and Computer Navigation Surgery.

POSTGRADUATE ORTHOPAEDIC TRAINING
I did my basic Surgical Training at Guys Hospital Medical School in London.
I started my Basic Orthopaedic Training in the South East of England Kent & Canterbury rotation.
I moved to Bristol & Cheltenham as an Orthopaedic Registrar.
I then joined The MCh Orth Course (Master Degree in Orthopaedics) at The University of Liverpool, England.
I joined the North West Orthopaedic Specialist Registrar Training Scheme in Manchester, England. During that period I did a Fellowship in Spinal & Paediatric Orthopaedics for six months in Hong Kong at The University Duchess of Kent Children's Hospital working with Prof. J Leong and Prof. K Luk.
In my final year as a trainee I was a Knee Fellow at the Knee Unit at Hope Teaching Hospital with Mr J Noble.
I passed the Speciality Exit Exam (FRCS Tr & Orth) 1998, and obtained the CCST (Certificate of Completion of Specialist Training and joined the Specialist Register.

TEACHING
I am involved in Orthopaedic Undergraduate Teaching to students from Manchester School of Medicine.
I am actively involved in Post-Graduate teaching to junior doctors during ward rounds, out-patient clinics and theatre sessions.
I run regular teaching sessions and courses for the Orthopaedic Trainee locally and regionally.
This is part of a recognised Higher Surgical Training Scheme in the United Kingdom, which is approved by the Royal Colleges of Surgeons.
All through my training I have been involved in undergraduate and postgraduate teaching. During
my surgical training at Guys Hospital I taught the third year medical students on regular introductory clinical ward rounds.
The period which I spent at the other teaching hospitals, Hope Hospital, Withington Hospital and
Royal Manchester Hospital I had a regular commitments in teaching third and final undergraduate medical students which included four week modules of out-patient clinics, ward rounds and lectures.
I was also involved in postgraduate teaching. I delivered series of lectures in different orthopaedic topics to the Accident and Emergency medical staff at Withington Hospital. Organising a weekly journal club was my responsibility during that period.

1. Mechanical Properties of Articular Cartilage in Knees with Unicompartmental Osteoarthritis

We studied the mechanical properties of cartilage from the apparently unaffected compartment of knees with unicompartmental osteoarthritis (OA). Plugs of cartilage and subchondral bone, 8 mm in diameter, were obtained from the tibial plateau of seven patients treated by total knee replacement. Control specimens were obtained from eight cadaver knees of similar age.Specimens were loaded by a plane-ended indentor in a hydraulic materials testing machine; we measured thickness, "softness", rate of creep, and compressive strength of the articular cartilage.We found that the 'unaffected' cartilage from OA knees was significantly thinner and softer than control cartilage, and it was slightly, although not significantly, weaker. We conclude that apparently unaffected cartilage in knees with unicompartmental OA is mechanically inferior to normal cartilage, even although clinically, radiologically and morphologically it appears to be sound.

2. PATELLA COMPLICATIONS IN TOTAL KNEE ARTHROPLASTY

This is a retrospective study, which was carried out on patients who under went total knee arthroplasties, with or without patella resurfacing over a seven-year period.The numbers of knees reviewed were 177 knees in 150 patients. Most of the prosthesis implanted was of kinematic type. The total number of complications was 33. The complicated group was divided into: Pain Group (n=10) and Mechanical Group (n=23). the Mechanical Group was then divided into: Subluxation & Dislocation (n=14) and Others [(n=9), 3 Fractures and 6 Component Separation].No correlation was found between Complicated Group (n=33) and Normal Group (n=144) regarding age, sex, type of prosthesis, and thickness of patella and lateral release. The mean duration of follow-up, was longer in the Pain Group and Component Separation Group, the differences were statistically significant. The incidence of pain was found to be higher in knees without patellar resurfacing. The number of patients who have had lateral release in the complicated group showed no statistical difference compared to the normal group.There was an increase in the valgus angle postoperatively in the patients with anterior knee pain. The duration of follow-up was found to be statistically significant in the pain group and component separation group (longer period of follow-up).

I am a full time NHS Consultant in Orthopaedic and Trauma Surgery. My Orthopaedic career spans over 28 years.
My interest is Lower Limb and Orthopaedic & Trauma Surgery. I have a wide experience in General Orthopaedics including Spinal, Upper and Lower Limb Surgery.
My area of practice is primary and revision Knee Arthroplasty, Arthroscopic knee surgery, ligament reconstruction and other lower limb work including Hip, Foot & Ankle Surgery.
