CAREER SYNOPSIS I trained at St Bartholomew's Hospital, London. My research training was
mentored at the Royal Postgraduate Medical School, London in the prevention of
venous thrombosis and pulmonary embolism. At St Thomas' Hospitals I
continued my training as an Orthopaedic Surgeon and developed a “living”
carbon fibre hip replacement which spontaneously resurfaced with living

to the Royal Navy, my substantive NHS appointment in Rochdale required that I
find a solution to the problems peculiar to failed back surgery. Rather than
pursue the same methods of treatment I introduced keyhole percutaneous
discectomy and KTP532 laser disc decompression into the UK in 1990. This led me
to pioneer aware state endoscopic transforaminal spine surgery and the use of
laser ablation of tissues and bone adjacent to nerves. This involved the
development of a side-fire laser probe. Overwhelmed by NHS referrals the Spinal
Foundation Charity was formed in 1994 and I became the Medical Director to
date.
The aware state patient feedback research defined the limitations of existing
techniques and led to the development of biportal (bilateral) flexible endoscopic
intradiscal discectomy with laser assistance. This expanded the application of
keyhole surgery for the primary treatment of Degenerate Disc Disease. However,
whilst this technique allowed larger or extruded discs to be addressed, there still
remained the need to address the wider gamut of spinal degeneration and failed
back surgery by keyhole means. This led to the development of the uniportal
technique of Transforaminal Endoscopic Laser Decompression & Foraminoplasty
and its instrumentation for the treatment of chronic lumbar spondylosis, lateral
recess stenosis, instability, spondylolitic spondylolisthesis, failed back surgery as
well as disc protrusions and extrusions / sequestra. To achieve these steps I set up
a company to fabricate suitable instrumentation because commercial concerns
did discern sufficient payback at that stage.
I have submitted multiple publications and performed many presentations and
educational workshops. In 2014 our 10 year independently reviewed outcome
study was published after peer review with a 72% - 80% success rate in patients
with and without failed back surgery.
The technique has proven very safe and successful over the last two decades1 and
now forms the platform for my on-going research and development of Gelstix
Disc Augmentation, stem cell disc reconstruction and robotic assisted surgery and
transforaminal fusion.
The research and clinical experience in the lumbar spine has been paralleled by
similar laser and endoscopic developments in the cervical spine.
My objectives are:

• to train more UK surgeons in the various stages of endoscopic surgical
complexity, (Currently over 2.000 surgeons perform these techniques
world wide). • to develop a further extrapolation of endoscopic instrumentation based
upon a unique pan-discipline MedusascopeR design. The design has been
patented and prototype design has been completed and requires to be
developed. • to combine disc height restoration with mesenchymal stem cell
regeneration effected endoscopically based upon a current development
programme.

INSTRUMENT DEVELOPMENT
When I commenced developing endoscopic spinal surgery, applicable instrumentation was not available. I developed a small company to make a novel design endoscope and instruments which sold to surgeons around the world. In an effort to expedite mesenchymal stem cell disc reconstruction the company merged with a German tissue replicant company but regrettably they shelved the spinal division. Recently I have sought to fill the gap in the market by building a group to provide a holistic entirely disposable endoscopic platform. This will assist surgeons in multiple disciplines and improve access and accelerate precision and reduce the need for surgical portals. This has reached the working prototype stage and now

NON SPINAL LABORATORY RESEARCH
Orthopaedic Prostheses
I have carried out a series of bio-compatibility and bio-engineering studies on a number of variants of Carbon Fibre Reinforced Carbon, in collaboration, this has lead to the development of a system of hip, knee and bone implants which are nearing the stage of clinical study.
Studies Include: Histological and the systemic evaluation of the response to Carbon Fibre Reinforced Carbon and other potential prosthetic substance in animals. Immunological and cell culture studies of the Carbon Fibre Reinforced Carbon.
The development of an animal model designed to reproduce the clinical and microbiological features of infection arising around prostheses and subsequent comparative studies with Carbon Fibre Reinforced Carbon variants and metals. The evaluation of surface tissue egress through Carbon Fibre Reinforced Carbon implants
An evaluation of the bio-physical properties of Carbon Fibre Reinforced Carbon in collaboration (with E J Walker, Atomic Weapons Research Establishment and with Dr J Coutts, Ph.D, Dept. Mechanical Engineering University of Leeds, and with Dr J Coutts, Ph.D., Dept of Engineering, The City University, London).
The development of a 3-Dimensional Finite Element Analysis model for the design of anisotropic Carbon Fibre Reinforced Carbon human prostheses. The design of surface joint replacements fashioned from Carbon Fibre Reinforced Carbon and their evaluation by introduction to animals.
Fracture Healing I have developed a physiological and quantitative model of fracture healing to evaluate the role of drugs and local hormones upon fracture healing. This model determines the rate of fracture healing by computer plenimetry, radioisotopes and biochemical dye analysis. This has been followed by a pilot study of the effect of the inhibitory properties of Diflunisal and Indomethacin, prostaglandin synthetase inhibitors upon fracture healing.
