Influenza Vaccine to Effectively Stop Cardio Thoracic Events and Decompensated Heart
Failure (INVESTED)
Influenza infection is known to be associated with increased risk of cardiovascular events, and
leads to significant morbidity and mortality each year. Annual influenza vaccination is
recommended by the Centers for Disease Control and Prevention, but the optimal vaccine
regimen that portends the most protection in patients with high risk cardiovascular disease is
unclear. The INVESTED study is a multi-center, randomized, active- control clinical trial of high
dose influenza vaccine compared to standard dose influenza vaccine for three seasons in adult
individuals with a history of myocardial infarction or heart failure who are at high risk for
cardiovascular events. The results of this trial have the potential to inform health care policy
regarding optimal influenza vaccination for individuals with high risk cardiovascular disease,
which may in turn reduce morbidity from this annual threat to health stability in patients with
cardiovascular conditions.

Pragmatic Trial Comparing Telehealth Care and Clinic-based Care for Uncontrolled High
Blood Pressure (Hyperlink-3) (Margolis)
This pragmatic trial is will be implemented in 20 HealthPartners clinics with little to no support
from the grant (to show that it is feasible for health systems to implement and sustain). Clinic
staff will recruit patients. Clinicians will need to be trained on the study protocols. The telehealth
intervention will be supported by HealthPartners MTM pharmacists and nurse practitioners who
will provide support over the phone.

Home Blood Pressure Telemonitoring and Case Management to Control Hypertension

The purpose of this proposal is to extend follow-up of the 450 patients enrolled in the HyperLink
intervention and usual care groups to achieve three specific aims: (a) provide new data on the
long-term durability (b) assess the cost-effectiveness of the Hyperlink intervention, and (c) use
mixed methods to identify critical factors for delivering the intervention successfully and
translating it into practice.

Blood Pressure Checks and Hypertension Diagnosis and Control (BP-CHECK) (Green)
Almost 1 in 3 Americans have hypertension—sustained high blood pressure (BP). Hypertension
is the leading risk factor for cardiovascular disease (CVD), the most common cause of avoidable
death and disability in the U.S. Our primary outcome is differences in mean systolic and diastolic
BP comparing clinic, home, and kiosk to 24-hour BP and to examine the knowledge,
preferences, and beliefs of providers (physicians, nurses, and MAs), managers, and
organizational leaders about BP diagnostic tests pre and post study; to identify implementation
barriers and facilitators for clinic, home, kiosk, and 24-hour BP monitoring. We hope to develop
a better understanding of the effects of treatment options for severe postpartum hemorrhage and
subsequent complications.

Kaiser Permanente & Strategic Partners Patient Outcomes Research To Advance Learning
(PORTAL)(McGlynn)
This study seeks to improve the ability and capacity to conduct widespread comparative
effectiveness research (CER) through the creation of a national electronic data infrastructure. In
Phase I of this initiative, the PORTAL Network developed a secure data infrastructure to
maintain patient information and track them over time. Further, it engaged patients, clinicians,
and other stakeholders in the research process. Phase II goals of PORTAL and PCORnet for
health system engagement includes the use of the Common Data Model, supporting embedded
clinical trials, and building sustainable collaborations. This will undoubtedly improve both
observational and experimental CER and develop much-needed resources that can be used for
both research and operations.

Depression RCT in ACS patients: Quality of Life and Cost Outcomes (Davidson)
Patients with an acute coronary syndrome (ACS) and comorbid depression have a 2-fold higher
risk for recurrent ACS and mortality, worse quality of life, and higher costs of care than
nondepressed ACS patients. Our specific aim is to determine the quality-adjusted life year
benefits and health care costs of following the AHA’s advisory for depression screening and then
referral for further diagnosis and treatment in post-ACS patients, if depression is found.

Prioritized Clinical Decision Support to Reduce Cardiovascular Risk (O’Connor)
The objective of this project is to develop and implement sophisticated point-of-care EHR-based
clinical decision support that (a) identifies and (b) prioritizes all available evidence-based
treatment options to reduce a given patient’s cardiovascular risk (CVR). This decision support
tool could be broadly used to both standardize and personalize care delivered by case managers,
pharmacists, and other providers in a wide range of care delivery configurations.

Diabetes, Metformin and Breast Cancer Prognosis (Luo)
The goal of the proposed study is to examine the causes for higher mortality in breast cancer
patients with diabetes compared with non-diabetic breast cancer patients, particularly, to
examine the impact of diabetes pharmacologic treatment on prognosis in breast cancer patients.
The study will take advantage of the relatively new linkage of the Women’s Health Initiative
(WHI), a large prospective cohort study in the US.
