Ventricular shunt

malfunction outcome

measure

This quality improvement effectiveness measure, implemented at the Boston

Children’s Hospital, has influenced the care received by children with

hydrocephalus requiring cerebrospinal fluid shunt diversion. The measure

received full endorsement by the National Quality Forum in 2012.

State of the Universe

The state of the universe (SOU) is an extensive database of the universe of

pediatric and adult quality improvement metrics complied from national,

regional and local sources. Information abstracted from SOU has informed

national pediatric quality improvement initiatives, including the Institute for

Healthcare Improvement’s Improvement Map.

Care Coordination View

The Care Coordination View is an interactive page within the Children’s

Hospital electronic health record which displays important diagnosis,

medication, equipment, and care plan information for children with medical

complexity. The view is being used to populate a personally-controlled health

record so that patients, families, outpatient and community providers may

access, view and share this health information.

Complex Care

Home Visit Program

I directed the development and implementation of a clinical program in which

hospital nurses make home visits to children with medical complexity soon

after their hospital discharge. During the visit, a standardized process occurs to

assess and address post-discharge issues, including problems with medications,

durable medical equipment, follow-up appointments, discharge instructions,

etc.

Narrative

My clinical, research, and quality improvement activities center on optimizing the health and wellbeing of

children with medical complexity (CMC). CMC are a population of children with life-limiting complex

chronic conditions and mult-morbidities who often rely on medical technology (e.g., tracheostomy) to

overcome significant functional impairment. I have had a major affinity to help these children and their

families since medical school. I am grateful to have worked over the last 10 years with one of the first

general pediatrics, structured clinical programs designed specifically for CMC, the Complex Care Service

at Boston Children’s Hospital. I am also grateful to have completed the Harvard Pediatric Health

Services Research Fellowship program, which enabled me to develop the research skills necessary to

conduct investigations on CMC.

In 2008, with the help of the Children’s Hospital Association, I founded the Complex Care Quality

Improvement and Research Collaborative (CC-QIRC), a multi-institutional collaborative of health

services researchers, quality improvement specialists, and researchers clinicians with a strong interest in

improving care for CMC. This group set out to understand the utilization of hospital resources by

children with medical complexity and how this utilization could be reduced with better care coordination

and better care planning in the community. I led the group through the first national study demonstrating

that a small group of children with medical complexity accounted for a large proportion of hospital

resources because they were being re-hospitalized over and over again in a short amount of time. This

work, published in JAMA, has spurred quality improvement initiatives in hospitals and communities

across the country to monitor and reduce this phenomenon. It has been a catalyst for the development of a

national coalition of children’s hospitals to incentivize the reduction of repeat hospitalizations in children

with medical complexity through Medicaid payment reform. Since its inception, CC-QIRC has secured

federal funding from the Agency for Healthcare Research and Quality and the Maternal and Child Health

Bureau. The collaborative has published an array of health services projects on CMC in Pediatrics,

JAMA Pediatrics, and Health Affairs. It also founded the Complex Care Special Interest Group sponsored

Academic Pediatrics Association. The body of work conducted by the collaborative contributed to the

development of federal legislation (Advancing Care for Exceptional Kids Act) to reform the system of

care for CMC.

I am thankful for the opportunities that have led me to develop a strong national and international

presence for CMC. This presence has enabled me to participate in national expert panels on CMC; advise

policymakers about CMC on Capitol Hill in Washington D.C.; and work with hospital leaders across the

U.S. as well as in Canada, Ireland, the United Kingdom, and Australia about their ongoing efforts to

systematically improve the quality of health services for CMC. This experience led me to become the

U.S. representative on the first ever international health systems project on CMC, funded by the European

Commission.

I plan to keep going with my attempts to leverage my health services research background to answer the

clinical questions that will have the greatest impact on the health and lives of CMC. There is a lot of

work to be done for these children and I’m thankful to be involved in it.
