SCHEDULE 0.0 INTRODUCTION AND INSTRUCTIONS 1. Tenderer shall complete this Appendix by giving and submitting all details,

information and supporting documents, as required under the various Schedules of this document. Additional sheets may be used as required and attached to the appropriate Schedules as applicable. Responses such as “will be advised” are unacceptable. Failure to submit all information required under the Schedules or failure to comply fully with these instructions in any way may render the Tender null and void.

2. The Schedules submitted under this Appendix constitute Tenderer’s technical

proposal and shall form the basis for the technical evaluation of the Tender. The completed Schedules, amended as may required by and APPROVED by LREDC, shall constitute Appendix D of the CONTRACT document.

SCHEDULE 5.0 PROJECT ORGANIZATION Tenderer shall attach an organization chart proposal, based on its experience with other similar projects and its knowledge of optimal structure requirements, for performing the WORK. The Organization Structure should show Tenderer’s inter-relationships and reporting lines among all disciplines of project team with responsibility matrix, the proposed reporting lines to CONTRACTOR’s main or other offices, Design and other Consultants, QA/QC and safety Functions, and SUBCONTRACTORS, as well as its proposed reporting lines to LREDC. KEY CONTRACTOR PERSONNEL identified and whose CVs are included in Schedule 6.0, should be identified on the organization chart.

SCHEDULE 9.0 SUBCONTRACTORS AND SUPPLIERS

Nominated SUBCONTRACTOR for Land and Fill Transport on Lusail Development

M/s Emdad Equipment Leasing Company of P.O. Box 40088 Doha, Qatar (Tel.+974 44915805, Fax +974 44687181) is the Nominated Subcontractor for Land and Fill Transportation on Lusail Development pursuant to Sub-article 10.4 of the General Conditions of Contract. Tenderer shall confirm that it shall utilize M/s Emdad for Land and Fill Transportation services on this project. Tenderer shall agree rates for these services with M/s Emdad, but such agreed rates shall not exceed the rates below. The Tenderers’s mark-up for Overhead and Profit on any agreed rates shall not exceed 10%.

Tenderer shall list below the names and addresses of all SUBCONTRACTORS including all Suppliers it proposes to employ to carry out any part of or to supply any materials/equipment or personnel in connection with the WORK. Details of the part(s) of the WORK or the type of materials that Tenderer proposes to SUBCONTRACT should also be indicated hereunder. For each proposed SUBCONTRACTOR, including suppliers of equipment or materials, tailed description and full details of the company, its line of business, experience, relevant projects, should be submitted. SUBCONTRACTORS are required to submit details under each schedule of this Appendix D along with details of relevant experience and shall be bound separately for each SUBCONTRACTOR. CONTRACTOR is not permitted to change any SUBCONTRACTOR named in this list without first obtaining LREDC written authority. Acceptance of the Tender as a whole will not imply APPROVAL of any SUBCONTRACTOR listed below. Full technical submissions will be required regarding all work and services proposed to be subcontracted along with details of relevant experience as stipulated in the General Conditions of Contract and the Specifications. SCHEDULE 12.0 WORK IN HAND For each Contract or Work that is currently being carried out by Tenderer, Tenderer shall submit here full details, as required under Schedule 11.0, Additionally, Tenderer shall for each project indicate: The quantity and disciplines of human resources dedicated to the project. The number and type of equipment provided for the project. The percentage of corporate management time and resources dedicated to the project. In the event any of KEY CONTRACTOR PERSONNEL or other resources proposed for the WORK are involved in current projects, Tenderer shall describe how and when they intend to make such KEY CONTRACTOR PERSONNEL or other resources available for the WORK.

SCHEDULE 13.0 FINANCIAL INFORMATION AND INSURANCE

Tenderer shall provide here a description of its financial condition and capability. Tenderer shall also attach a copy of each of the last three (3) year’s Annual Report including Directors’, Company Secretary’s, and Auditor’s reports, including its financial trading profit and loss and funds statements and balance sheets. These statements must also be provided from parent, associated or related company, where the Tenderer is to be supported by such company; and all members of a Joint Venture or a Consortium.

Tenderer shall provide a certification from their statutory auditor certifying that the annual turnover for the last five years for works strictly related to highways, utilities, and infrastructure projects in Qatari Riyals. Additionally, this certificate shall state the infrastructure annual turnover as a percentage of total annual turnover for each of the last five years. Moreover, the said certificate shall be provided individually for each and every joint-venture partner. The certificate shall itemize relevant annual turnover for each of the last years as per the categories below:

(a) Highways and Highways Structures

(b) Telecommunications and ELV Systems

(c) Electrical and cabling Works: HV, MV and LV electrical supply, including 11/0.4kV distribution sub-stations

(d) Piping Works: Stormwater network, Gravity foul-water system, Irrigation system, and Potable water system

SCHEDULE 14.0: QA/QC PROCEDURES CONTRACTOR’s Quality Assurance and Quality Control System dedicated for this Project is to be provided in this Schedule. CONTRACTOR Quality Assurance and Quality Control System dedicated for this CONTRACT shall conform to LREDC Quality System Requirements. This document will be reviewed by LREDC and any comments made shall be incorporated by CONTRACTOR. Final document must be issued three (3) weeks after the EFFECTIVE DATE of CONTRACT. The quality system shall be formalized in a Quality Manual which documents general policies, procedures and practices of the organization covering all quality related elements, addressing as a minimum requirements of LREDC. CONTRACTOR shall ensure that all Consultants and SUBCONTRACTORS comply with CONTRACTOR’s QA/QC Procedures.
