Dear UAB Employee:

For the seventeenth consecutive year, we are pleased to have the opportunity

to serve UAB employees through the VIVA UAB product. As the largest

health plan for UAB employees, we pride ourselves on offering our customers

comprehensive benefits, outstanding value, and quality providers.

If you are a current VIVA UAB member, we hope you decide to continue with

our plan. If you will be enrolling with VIVA UAB for the first time, please

remember to choose a Primary Care Physician (PCP). You will find a full VIVA

UAB provider directory at www.vivahealth.com/VIVAPhysDirectory.html

(be sure to click on the VIVA UAB link).

As a member of VIVA UAB, you have access to the world-renowned UAB

Health System for primary care, OB/GYN and other health care services.

For OB/GYN services, you also have access to providers at Medical West.

You have access to our entire network of optometrists and ophthalmologists.

VIVA UAB members under the age of 18 have access to VIVA Health’s entire

pediatric network. Please keep in mind that VIVA UAB members are not

required to obtain a PCP referral to see participating OB/GYNs at UAB,

optometrists or ophthalmologists.

If you have any questions, we invite you to contact our customer

service department at (205) 558-7474, (800) 294-7780, or by e-mail at

[email protected]. You will also find valuable information

on our website at www.vivahealth.com.

Thank you for considering VIVA UAB as your health plan in 2012. We look

forward to serving you.

Sincerely,

Brad Rollow CEO/President

Worldwide Emergency and Urgent Care CoverageIf you are outside the service area and have an urgent and unforeseen need for care that can’t wait until you return home, you are covered. This includes care in a physician’s office for an unexpected illness or injury that would not be classified as an emergency, but does require immediate attention. Of course this does not include routine or elective medical services, and you must return to the service area for any follow-up care. Still, if you’re on a weekend trip or an extended vacation, you can relax knowing that you are covered. Care in an Emergency Room is only covered for treatment of emergency medical conditions.

OB/GYN NetworkVIVA UAB members must see providers at UAB Hospital or Medical West for OB/GYN related services. OB/GYN services outside of the UAB Health System network will not be covered. No referrals are required for OB/GYN services. Please refer to the website www.vivahealth.com/VIVAPhysDirectory.html for a listing of OB/GYN providers for VIVA UAB members.

Dependent Student RiderServices to treat an illness or injury for Covered Dependents will be covered while they are full-time students at an accredited educational institution located out of the Service Area. These services are subject to the Copayments described herein and are available at a $1,500 maximum benefit per calendar year. Emergencies and in-area care are covered under the appropriate sections set forth in the Certificate of Coverage.

Sabbatical Leave RiderSabbatical Leave is a period of paid leave granted to faculty members by the Employer to pursue professional development, a program of investigation, creative writing, artistry and the like. Services to treat an illness or injury for Subscribers and Covered Dependents on Sabbatical Leave will be covered while they are out of the Service Area, and are subject to the Copayments described herein and are available at a $1,500 maximum benefit per calendar year.

Grandfathered Health PlanVIVA Health believes this health plan is a “grandfathered health plan” under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted. Being a grandfathered health plan means that your plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, such as the requirement for the provision of preventive health services without any cost sharing. However, grandfathered health plans must comply with certain other consumer protections in the Affordable Care Act such as the elimination of lifetime limits on the dollar value of essential health benefits.

Questions regarding which protections apply and which protections do not apply to a grandfathered health plan and what might cause a plan to change from grandfathered health plan status can be directed to VIVA Health Customer Service at (205) 558-7474 or 1-800-294-7780. You may also contact the U.S. Department of Health and Human Services at www.healthcare.gov.

Don’t Forget…• Adult members (age 18 and over) must select a UAB doctor as their Primary

Care Physician (PCP). When you need specialty care, your PCP will refer you to UAB Health System providers (no referral required for OB/GYN and vision care providers). When you need hospital care, you will be admitted to UAB.

• All children (under age 18) enrolled in VIVA UAB must select a pediatric doctor in the VIVA Health network as their PCP.

• Female members enrolled in VIVA UAB must choose an OB/GYN provider at UAB or Medical West.

VIVA UAB provides UAB employees with outstanding benefits at an exceptional value. For the seventeenth consecutive year, we are pleased to offer a plan designed with UAB Employees and their families in mind. As a VIVA UAB member, you have access to the world-renowned UAB Health System.
