Introduction
The Whole Family Connection™ consists of two web-based programs that help you discover what services may benefit you and your entire family.
The Whole Senior Connection™ enables seniors to contact the providers of those services that you decide are best for you, conveniently and anonymously.
The Whole Child Connection is used by children and their families.
There are only four steps to follow:
1. Complete a Profile,
2. Review & Select Services & Providers,
3. Create a Success Plan,
4. Get Connected.
