and sign for the corporation is hereby submitted and attached asANNEX “A” ;

3. That also in compliance with the directive of this Honorable Court, acopy of the joint verification/certification on non forum-shopping forMr. Emiliano S. Ching and Ms. Maria Ulit and an Affidavit of Serviceof the Petition are hereby submitted and attached as ANNEX “B” ,and ANNEX “C” , the same were notarized by Atty. Jose Gerardo F.C. 4. Finally, petitioners herein manifest that the full name of individual

petitioner Mr. Emil Ching is Mr. Emiliano S. Ching. Thus, it isrespectfully prayed for that the name “Mr. Emil Ching” in the title ofthe case be changed to “Mr. Emiliano S. Ching”.
