Voting Instructions

1. With a ball point pen or pencil, mark the ballots with your choice(s). Theballot indicates the maximum number of candidate(s) that can be voted for.You may vote for less than the maximum number, but not for more.

2. Place all ballots, marked or not, in Special (Mail-in) Ballot Envelope A andseal.

3. Place Special (Mail-in) Ballot Envelope A in the Special (Mail-in) BallotCertificate Envelope B and seal.

4. Complete and sign Part 1 of the Special (Mail-in) Ballot Certificate Envelope B.

5. Attach a copy of your identification to the OUTSIDE of Certificate Envelope B (see reverse for a listof authorized ID).

6. Place your identification and Special (Mail-in) Ballot Certificate Envelope B into Return Envelope Cand seal.

Submit Your Package The Return Envelope C and contents must arrive at the address below or be placed into a Mail-in (Special) Ballot drop-box on or before 4 p.m. Monday, October 18, 2021. Please visit www.calgary.ca/mailinballot for more information on drop-box locations. Failure to complete any of these steps may invalidate your ballot.

By submitting this package to Elections Calgary, you are

confirming: I am eligible to request a mail-in (Special) ballot by

• Being physically unable to attend a voting station;• Absence from the city on Election Day; or• Working on Election Day in a voting station that is not my

designated voting station for Elections Calgary, or as acandidate, official agent or scrutineer.

General Election Date: October 18, 2021

Authorized Identification In accordance with Section 53 of the Local Authorities Election Act, all electors must present one of the forms of identification as proof of eligibility to vote. The authorized identification must verify name and current residential address. Picture ID is not a requirement.

The following is a list of acceptable identification. Only identification listed below will be accepted.

Proof of identity and current residence is required to be able to vote in municipal elections. Any of the following documents that has the voter’s name and address on it will be accepted as valid proof: • Identification issued by a Canadian

government, whether federal, provincial orlocal, or an agency of that government, thatcontains a photograph of the elector and theirname and current address.

• Bank or credit card statement or personalcheque.

• Government cheque or cheque stub.• Income or property tax assessment notice.

• Insurance policy or coverage card.

• Letter from a public curator, public guardian orpublic trustee.

• Pension plan statement of benefits,contributions or participation.

• Residential lease or mortgage statement.

• Statement of government benefits (forexample, employment insurance, old-agesecurity, social assistance, disability support orchild tax benefit).

• Utility bill (for example, telephone, publicutilities commission, television, hydro, gas orwater).

• Vehicle ownership, registration or insurancecertificate.

The attestation form confirming that the person lives at the stated address will also be accepted as valid proof. Please visit the Elections Calgary website to obtain the attestation form. The form can be signed prior to the vote by any of the following: • Authorized representative of a commercial

property management company;

• Authorized representative of a correctionalinstitution;

• Authorized representative of a First Nationsband or reserve;

• Authorized representative of a post-secondaryinstitution;

• Authorized representative of a facility thatprovides services to the homeless; or

• Authorized representative of a supportive livingfacility or treatment centre.
