nursing home and assisted living facility in the country. The information includes the facility’s tribal affiliation,
facility name, address, certification, number of beds, and year opened. Additionally, this report outlines
information about the facilities’ Quality Improvement Organizations and Long-Term Care Ombudsman

Long-Term Care Ombudsman
Long-Term Care (LTC) Ombudsman programs help resolve health, safety, and welfare issues and advocate
for the rights of individuals living in LTC facilities, like nursing homes and assisted living facilities. Each state
manages an LTC Ombudsman program. The program is directed by the Older Americans Act to:
• Identify, investigate, and resolve complaints made by or on behalf of residents
• Provide information to residents about long-term services and supports
• Ensure that residents have regular and timely access to ombudsman services
• Represent the interests of residents before governmental agencies
• Seek administrative, legal, and other remedies to protect residents and
• Analyze, comment on, and recommend changes in laws and regulations pertaining to the health,
safety, welfare, and rights of residents

Quality Improvement Organizations
Quality Improvement Organizations (QIOs) aim to improve the effectiveness, efficiency, economy, and quality
of services delivered to Medicare beneficiaries. QIOs are required under Sections 1152-1154 of the Social
Security Act. Their core functions are to:
• Improve quality of care for beneficiaries
• Protect the integrity of the Medicare Trust Fund by ensuring that Medicare pays only for services and
goods that are reasonable and necessary and that are provided in the most appropriate setting and
• Protect beneficiaries by addressing individual complaints, such as provider-based notices of appeal and
violations of the Emergency Medical Treatment and Labor Act
There are two types of QIO programs, Beneficiary and Family Centered Care (BFCC-QIOs) and Quality
Innovation Network (QIN-QIOs). Each type has separate directions, as identified in Table 10.
