Ifound working in a non clinical role a starkcontrast to what I was familiar with and thismeant having to adapt my style of working tomeet the demands of the job.

I am currently a ST5 trainee in General AdultPsychiatry and my present job is working withOlder Adults.

I enjoy working in Psychiatry because of thediverse patient groups. It provides numerousopportunities to work in both hospital andcommunity settings helping people and theirfamilies. Outside of work I enjoy the cinema, reading and international travel.

Looking at streamlining appropriate care toappropriate clients using a traffic lightssystem to highlight key worker caseloadweighting and risks attached to theirpatients. The idea is simple – it is to makesure work load is spread evenly and tohighlight patients who need more intensiveintervention sooner so that resources can beallocated earlier before the point of crisis. The traffic light tool is borrowed from apilot done elsewhere and is being simplifiedtaking into account frequency of patientcontact and level of predicted risk. It ishoped this tool will guide efficiency in teammeetings, reduce the frequency of heavycaseloads occurring and catch elevating riskbefore it manifests in more serious ways

Project

I am an ST4 Trainee working for the central eastcommunity mental health team in Amersham. Isee a wide variety of patients that keep me onmy toes and interested in my field fromdebating delusionals to hair eating compulsionsand suicidal or high risk patients to suchenigmas of psychopathology that you cannotget bored. I am particularly curious about themedical-psychiatric interface with a keenness tohelp those with psychiatric sequelae to medicalconditions like thyroid disorders orHuntington’s disease.

My current special interests include workingwith post traumatic stress, eye movementdesensitisation and reprocessing andsupporting survivors of torture with the“freedom from torture” organisation. Inaddition to this I have eagerly followed a youthhelpline that I helped set up a number of yearsago and am also involved in a number ofcommunity projects.

Aside from my clinical work I have an interest inresearch and education. I am actively involvedin tutoring oxford medical school students andam an investigator for clinical trials at theUniversity of Oxford. This year I am also doing apost graduate diploma in education in Oxford.

To undertake a feasibility study into clinicalphotography triage of dermatology referralsto secondary care with the aim to reduceoutpatient demand and improve patientoutcomes

Project

I trained in general photography gaining aNational Diploma and then a Higher NationalDiploma after which I undertook generalphotographic employment before, in 1999,being employed in the NHS as a clinicalphotographer at the Ipswich Hospital.

After 21/2 years at Ipswich Hospital, developingspecialist clinical photography and healthcarescience skills, I had the opportunity to helpdevelop and set up a diabetic retinal screeningservice for Suffolk run by the West SuffolkHospital. This was a huge challenge and was agreat learning experience. Once this service wasrunning and successful I was asked to take onthe role of developing a full time clinicalphotography service at the West Suffolk NHSFoundation Trust.

Since 2006 I have been working to develop thisservice which has proved to be an even biggerchallenge than setting up a PCT wide screeningservice! In the past six years I have undertakena specialised graduate certificate qualification inClinical Photography and been regional finalistin the Health and Social Care Awards.

Being accepted onto the Clinical LeadershipFellowship programme has refreshed my driveto improve services and patient care. I alsohope to raise the profile and the valuablecontribution that Clinical Photographers makein the patient care pathway.

I am currently working aspart of an integrated health and social carecommunity team for adults with learningdisabilities in Somerset. People with learningdisabilities are among the most vulnerablepeople in society, however, increasingly, theirhealth needs are being recognised andaddressed.

Providing a responsive service that delivers highquality care has always been my priority. Inrecent years I have had the opportunity tospecialise in the area of dysphagia (eating, drinking and swallowing difficulties). Dysphagiacan significantly impact on a person’s healthand it is an area that carers may findparticularly difficult to manage. Currently theevidence base relating to dysphagia and adultswith learning disabilities is fairly limited. However, best practice suggests that reducingthe risks of dysphagia to an individual benefitsfrom a multidisciplinary approach. I believe thatthe clinical leadership fellows programme willhelp to enhance my skills and understanding tocollaborate with health and social carecolleagues with the aim of developing adysphagia service within adult learningdisability services that is equitable across therural county of Somerset and that reduces risksto health.
