All students entering ninth grade, shall show proof of receiving one dose of Tdap (defined as tetanus, diphtheria, acellular pertussis) vaccine regardless of the interval since the last Dtap, DT or Td dose.

All forms must be completed and returned to the school nurse no later than August 14, 2013. Students that do not meet these requirements will not be allowed to attend school until these requirements have been met.
