Submit request for verification of eligibility of a professional through personal delivery or by postal service (mail).

Verify all verification requests from the PRC database (LERIS) or hard copy of the Master list of Examinees, i.e., name of the profes-sional, date of birth, registration number and date, date of examination, and ratings. Re-encode name of the professional, date of examination and ratings obtained using the preformatted mail verification form then affix-es his/her initials. Review the entries in the verification form then affix initials prior to the signing of the authorized signatory.
