RELEASE, WAIVER & QUITCLAIM

We, ___________________________ and ____________________________________, of legal age, [married], Filipino, with residence at ______________________________________________________, as [parents/legal guardian] of ____________________________________, do hereby freely and voluntarily give our consent for our [child/ward] to be a participant to the 10th Citi Junior Bankers Congress, a live-in workshop scheduled on January 18-19, 2013, Baguio City. We, together with our [child/ward], further acknowledge and agree that as a participant, our [child/ward] shall take part in various activities under the Congress, including those held outside of the aforementioned venue, where our [child/ward] may be brought using vehicles provided/contracted by FINEX and CITI (collectively, “ORGANIZERS”).

In view of the foregoing, we do hereby voluntarily and unconditionally waive and relinquish any and all rights, demands, claims and causes of action of whatever kind or nature against the ORGANIZERS arising out of, or by reason of, any matter, cause, damage or loss relating to the participation of our [child/ward] in the Congress, the intention hereof being to completely release the ORGANIZERS from any and all liabilities arising in connection with said Congress.

This Release, Waiver and Quitclaim shall operate as a bar to any action or suit or any other proceeding, judicial or extrajudicial against the ORGANIZERS, and shall extend to, and be binding upon, our heirs, administrators, executors, assigns and successors-in-interest.

We finally declare that we have read this document, which we fully understand, and the release and waiver hereby given are made willingly, voluntarily and irrevocably.
