Chronic obstructive pulmonary disease
• chronic slow progressive disorder characterised by airflow obstructionthat does not change markedly over several months
• although there is some overlap in the features of COPD and asthma, they are separate disorders with different aetiologies, pathologies, natural history and responses to treatment
CHRONIC BRONCHITIS
Definition
• chronic bronchitis is a hypersecretory disorder defined at the presence of cough productive of sputum on most days for at least 3 months of 2 succesive years in a patient in whom other causes of chronic cough have been excluded (e.g. tuberculosis, bronchiectasis)
• the diagnosis is made on the basis of symptoms• the airways of patients with chronic bronchitis show mucous gland hypertrophy
and an increased number of goblet cells

Emphysema
• defined in terms of its pathological features which consists of dilatation of the terminal air spaces of the lung distal to the terminal bronchioles, with destruction of their walls
• physiologically, emphysema is characterised by a reduction in the transfer factor for carbon monoxide and transfer coefficient (TLCO & KCO)
• high-resolution computed tomography (CT) scan can demonstrate the parenchymal lung destruction of emphysema

ETIOLOGIA BPOC
1. Fumatul: data începerii, profunzimea inhalării, număr pachete-ani; mai mulţi bărbaţi, în mediul urban şi în situaţii cu status socio-economic scăzut
2. Poluarea atmosferică
3. Poluarea cu pulberi la locul de muncă: bisinoză la muncitorii din industria bumbacului, astm la muncitorii care vopsesc cu spray, COPD la agricultori; expunerea la praful de cărbune şi la praful de bumbac creşte riscul dezvoltării BPOC
4. Factori din copilărie: expunerea pasivă la fumul de tigară, infecţii respiratorii în copilărie (hiperreactivitate a CA), intervenţia factorilor genetici (ex deficitul de alfa1-antripsină)

EXPLORĂRI ÎN BPOC
1. Teste funcţionale pulmonare – spirometria este mai utilă decât PEF în BPOC. CPT şi VR sunt adesea crescute, semnificând air-trapping şi hiperinflaţie. TLco şi Kco sunt tipic reduse în emfizemul pulmonar
2. Gazometria arterială şi oximetria
3. Examenul radiologic – demontrează hiperinflaţia aerică pulmonară şi exclude NBP, pneumotorax, bronhopneumonia
4. CT – extensia EP şi prezenţa bulelor; nu este de rutină
5. Examenul microbiologic al sputei şi sputoculturi
6. Teste generale – creşterea Hb, GA, tulburări electrolitice, ECG, echocardiografie
