Outcomes from my funding request clearly benefit children’s health outcomes and/or the healthcare delivery provided by CHQ.

My funding request aligns with one or more of CHQ’s service directions and strategies as outlined in CHQ’s Children’s Health and Wellbeing Services Plan 2018-2028:

1. Promoting wellbeing and health equity2. Improving health service design and integration3. Evolving service models4. Delivering services closer to home5. Pursuing innovation.

My proposal is for once-off funding and will not be used for operational, recurrent costs. Budget items exclude capital works, infrastructure and indirect costs. Travel, telephone and computer costs have not been included in the budget except if they are directly related to the funding request.

I have only requested funding for an activity or piece of clinical equipment that would not normally be funded by my department’s budget allocation for goods and services.

I am not requesting funding for: • research projects / programs / infrastructure / support• professional development• staff salaries or expenses that fall within the department’s budget• gifts or expenses that would otherwise fall outside of Queensland Government

purchasing guidelines• clinical equipment over $5,000• capital works• educational programs and resources for health care professionals valued at

$20,000 or under (if so, funding should be requested through the CHQ SERTA scheme).

I have attached a letter of support signed by the relevant Divisional or Executive Director. Approval from my line manager to conduct the activity has been obtained, if applicable.

Successful applicants must be located in Australia and at least 80% of the non-research activity must occur in the state of Queensland.

About the Children’s Hospital FoundationThe Children’s Hospital Foundation is the charity of the Queensland Children’s Hospital, the Centre for Children’s Health Research, and for all sick kids throughout Queensland. We work wonders for sick kids by funding life-saving medical research, investing in vital new equipment, and providing comfort, entertainment, family support and care programs for sick kids. Our vision is for every child to have the best possible health care, with access to world-class research and clinical treatment in a healing environment. Our mission is to improve children’s health by supporting and funding world-class research and clinical treatment, and by providing care and entertainment for children and their families. 3. About the Working Wonders Grant SchemeIn consultation with CHQ, the Children’s Hospital Foundation has developed the Working Wonders Grant Scheme to facilitate equitable access to funding to improve health outcomes for children throughout Queensland. In this scheme, funding can be requested for:

• educational programs or resources;

• support services or materials for patients and families;

• clinical projects or programs with no research component;

• salaries for clinical support staff who provide a critical service caring for children, but who cannot be funded by the existing department budget;

• clinical equipment less than $5,000.

Activities and equipment for which funds are requested should augment the standard of care provided by CHQ and must not be covered by a department’s existing budget.

This scheme aligns with the mission and vision of the Children’s Hospital Foundation by aiming to improve child and adolescent health care and medical services and enhance the support provided to patients and their families.

In the 2019-2020 Working Wonders Grant Round, the Children’s Hospital Foundation has up to $1.5 million available for allocation. This is the total funding available to CHQ and other Queensland-based health services caring for children and adolescents.

3.1 Value of the Award

The following limits apply to funding requests in the Working Wonders Grant Scheme:

Applicants who are able to provide a strong justification with supporting evidence and a detailed budget, may request more funding than the limits defined above (with the exception of clinical equipment which must go through the Capital Management Committee process).

3.2 Assessment Process

Alignment to CHQ’s service directions and strategies as outlined in CHQ’s Children’s Health and Wellbeing Services Plan 2018-2028 will be used in the assessment and prioritisation of applications received in the Working Wonders Grant Round. CHQ’s service directions and strategies are as follows:

1. Promoting wellbeing and health equity2. Improving health service design and integration3. Evolving service models4. Delivering services closer to home5. Pursuing innovation

Applications received in this grant round will be approved and prioritised for funding through the CHQ Business and Clinical Services Improvement Committee in consultation with the Children’s Hospital Foundation.

In assessing and prioritising the applications, the following criteria may be considered:

• feasibility

• potential for duplication

4. Award, monitoring and evaluationIf funds are secured for a Working Wonders Grant, the successful applicant will receive a formal Letter of Offer, and the Terms and Conditions of funding. The Terms and Conditions of funding will describe reporting and acquittal requirements that the grant recipient must comply with for the Children’s Hospital Foundation to release funds.

The Terms and Conditions of funding are non-negotiable. Acceptance of Terms and Conditions of funding must be signed by the grant recipient and an authorised representative of CHQ. Electronic copies must be returned to the Children’s Hospital Foundation within 30 business days or the offer of award may lapse.

Applications that fall outside of the available budget will be considered “near misses” and funding will be sought by the Children’s Hospital Foundation’s fundraising staff.

To improve the chances of your application being approved for funding, you should consider your application to be a pitch. Clearly articulate what you are seeking funding for and how the activity / equipment will achieve a significant impact on children’s health outcomes.
