Dear Seniors, Colleagues & Friend's,

This is an occasion to share with you the developmentsand progress made in the year 2005-06.

The inauguration of the twin tower blocks with 1,500 bedsin Hospital was a development in the history of hospital.The number of 154 new beds added in the tower blocks has raisedthe total bed strength of the department to 308. This has created anew record to claim the department as the largest department in aGovernment Medical College in the whole of India. We now haveseparate ortho trauma ward for emergency accident patients, intensive trauma care wardfor poly trauma (multiple injuries) patients and an emergency trauma theatre. The additionof a trauma theatre (with 2 tables) to our orthopaedic theatres (with 4 tables) has madeit possible to provide quicker surgical care for our accident victims.

The bone bank has been allotted space in the new tower block. We are in theprocess of equipping the same. Live donor bone procurement and clinical usage hasalready commenced. Cadaveric donor procurement, processing and storage wil l beshortly started.

I must place a record our thanks to our Dean Dr. Kalavathy Ponniraivan who hasbeen instrumental in raising our department to high standards. She has provided ourdepartment with all that we needed for improvement of patient care. Our special thanksto our Director Medical Education Dr. P. Vijayalakshmi who has given constant supportto our department.

In this Bone and Joint Decade (2000-2010) the Department of OrthopaedicSurgery has taken a great step forward in bringing high quality orthopaedic care withinthe reach of the common man in our society and has set an example that public hospitalscan also be on par with corporate hospitals.

I am indebted to my parents in heaven who have made it possible for me to reachthis position in the world today. I pray to God Almighty, to shower his blessings toone and all.

HISTORY OF THE DEPARTMENTOrthopaedics was recognised as one of the specialities of surgery in the first quarter ofthe 20th Century. In India, the pioneers of orthopaedic surgery started their work invarious parts of the country in the late 1920s. The first Orthopaedic Services inTamil Nadu was started in Govt. General Hospital, Madras in the year 1929 as anoutpatient service under the Department of Surgery. It was in 1931, the inpatient facilitieswere started under the Department of Surgery. In Tamil Nadu, Dr. M.G. Kini was thefirst person to qualify in orthopaedics with M.Ch.Orth. (Liverpool), UK, serving in theDepartment from 1929 to 1931. Dr. N.S. Narasimhan served in Govt. General Hospital,Madras, from 1931 to 1948.

In 1949, a separate Department of Orthopaedics was established and the lateLt. Col. V.R. Thayumanaswamy was appointed as the first Professor of OrthopaedicSurgery. He brought his expertise in General Surgery and the experiences in the secondWorld War for the benefit of Orthopaedics.

In 1957, Prof. M. Natara jan took over as Professor of Orthopaedics, Govt. GeneralHospital and continued till 1976. It was during his period that there was expansion of thedepartment with addition of beds, introduction of postgraduate course in Orthopaedics,establishment of specialities allied to Orthopaedic Surgery such as Physical Medicineand Rehabilitation.

Prof. T.K. Shanmugasundaram (1976-1987) pioneered the starting of the PL 480scheme for Traumatic Paraplegics and the MRC trial for TB Spine. It was due to hisefforts that the paraplegia ward was opened and the PL 480 project was absorbed bythe Government. Prof. S. Soundarapandian (1987-1989) was an excellent teacherand surgeon and improved the quality of patient care in the Department.

Prof. S. Rajagopal (1989-1992) was instrumental in bringing up the new paraplegicblock with contributions from the Round table. Prof. T.S. Xavier (1993-1994) was aperson with pleasant manners and worked towards the upliftment of the Department.

Prof. K. Sriram (1994-1997) was a pioneer in spinal surgery and pediatric orthopaedicsand had a special interest in academic programmes. He had conducted around six suchprogrammes out of which 2 were national conferences. Prof. C.T. Alagappan (1997)was a popular teacher among the undergraduates and post graduates and contributed tothe growth of the Department.

Prof. K. Annamalai (1997-2001) with his untiring efforts was able to obtain a specialgrant of 50 lakhs for the purchase of implants which has made our department self sufficienttill date. The air-conditioned, fully furnished lecture hall of the department stands as atestimony to his efforts.

Prof. M. Subramanian (2001) introduced the concept of uncemented total hipreplacements. He popularised hip and knee replacement surgery in the Department.

Prof. K. Chandran (2001-2003) upgraded cleanliness in the Orthopaedic wards andthe Department was awarded the Best Zone Award during his tenure.

Prof S.N. M o t i l a l (2003-2004) continued the good work of his predecessors andmaintained the high standards of the Department. Prof. R. Dhanapal (2004-2005) wasa specialist in spine and shoulder surgery and was known for his non controversialapproach towards various issues.

THE DEPARTMENT

Prof. Mayil Vahanan Natarajan took over as the Head of the Department from1st March 2005.

The inaugura t ion of the newTower Blocks on 1st July 2005 bythe Govt. of Tamil Nadu was a shot inthe arm for the sustained growth of theDept. of Orthopaedic Surgery.

Two Male Wards with total capacityof 80 beds housed the patients, whowere suffering in the overburdenedwards. One Female Ward with thecapacity of 40 beds eased thecongestion in the existing female ward.The bed strength of the Department has been increased to 308 beds from 145 from1 st July, 2005. This makes the Department the largest one in Government Hospitals inthe whole of India.

The Ortho Trauma Ward was separated from the surgical trauma ward and taken over bythe Dept. of Orthopedic Surgery in the new tower block.This is the landmark event becauseone qualified orthopaedic surgeon, one M.S.Orth. post graduate and two D.Orth.post graduates are present 24 hours a day, 365 days a year to serve the trauma patients.

Emergency Trauma Theatre with state of art facilities has been commissioned whichhas helped the team to treat the compound fractures within 3 hrs of admission.

12 bedded Intensive Trauma Care Unit was started to offer round the clockIntensive Care to the multiply injured patients.

The Dept. of Orthopaedic Surgery has been allotted 3 Operation Theatres with two tableseach. This has increased the number of surgeries performed by the dept. thereby greatlyreducing the waiting time of the patients for surgery. These operation theatres with laminar airflow facilities are suitable for doing modern surgeries like arthroplasty and arthroscopy.

The concept of Bone Bank has been accepted by the Government of Tamilnadu and aG.O.Ms.No. 65 for installing the Bone Bank was passed on 8-3-2005. The Bone Bankhas been inaugurated in Tower block 1 along with the inauguration of the Tower Blocks.Prof. V. Thulasiraman took over as 4th unit chief in December 2005 thereby addingimmense strength to the academic side of the department.

The Bone Bank in Madras Medical College and Govt. General Hospital wasinaugurated by the Hon. Chief Minister of Tamil Nadu, on 1st July 2005 in thesixth floor oj Tower Block 1.

The structural modification of the bone bank has been completed two deep freezersneeded to store the allografts have already been procured and are being used to preservethe bone. The various other equipments such as i. Ultrasonic Bath ii. Bone CuttingMachine iii. Freeze Drier iv. Laminar Air Flow and v. Ethylene Oxide Sterilizer, whichare required for the functioning of the bone bank need to be procured. The requirementhas been sent to the Director of Medical Education and the same has been approved.

The allografts are used in various orthopaedic surgeries likei) Tumour resection and allograft reconstructionii) Curettage of benign tumours and bone grafting

iii) Bone defects in non unions

The allografts which are preserved on the bone bank are the femoral headsfrom the live donors undergoing hemiarthoplasty for fracture neck of femur. We havedone 38 surgeries by using this type of allograft. At present the whole tibia, femurand pelvic allograft is procured from the srilankan bone bank with which we havedone 20 surgeries. All the allografts are preserved and stored at - 80°C afterappropriate screening.

In future we are planning to procure the whole bone allograft in our bone bankonce we get all the equipments.

Public awareness programmes are conducted and will ing donors are beingregistered in the bone bank.

Prof. MAYIL VAHANAN NATARAJANNational Award for the Welfare of Disabled by the Government of India inrecognition of cost effective technological innovation of Custom Mega Prosthesis (2005).

Lemelson Recognition and Mentoring Programme (L-Ramp) Innovation Award by theThe Lemelson Foundation, USA & Indian Institute of Technology, Chennai inrecognition of his effort in innovating the Custom Mega Prosthesis (2005).He was elected as President of Indian Orthopaedic Association in Dec. 2005.The IOA is an academic body of 6,000 Orthopaedic Surgeons from all over India.He is the fourth person from Tamil Nadu to be elected for this highest post.The earlier Presidents are Prof. M. Natarajan, Prof. T.K. Shanmugasundaramand Prof. A. Devadoss.

Prof. K. ANBAZHAGANFirst book in Tamil in Orthopaedics namely 'Elumbu Moottu NalanV was writtenand published by Prof. K. Anbazhagan was released by the past Hon'ble HealthMinister, Tamil Nadu, Thiru. Thalavai Sundaram, in the presence of the thenSpeaker of the Tamil Nadu Assembly, Hon'ble Thiru. Dr. K. Kalimuthu.The Director of Medical Education Dr. Vijayalakshmi presided. The Dean of theMadras Medical College and Govt. General Hospital Dr. Kalavathy Ponniraivan,the former Vice Chancellor, Dr. MGR Medical University Dr. Major Raja felicitatedProf. Anbazhagan.

The same book was awarded first prize as the 'Best Book in Medicine' - 2005 by theTamil Nadu Government and Coimbatore Bharathiar University.

Prof. R.H. GOVARDHANElected as Secretary of Indian Orthopaedic Association for 3 years for 2006-2008.He is the first person from Tamilnadu to be elected for this post.

Prof. V. THULASIRAMANJoined the Department in December 2005.

Dr. R. SELVARAJElected as Secretary - Tamilnadu Orthopaedic Association in February 2006.This is an academic body of 1000 Orthopaedic Surgeons from all over Tamilnadu.

EVENTS ORGANISED IN DEPARTMENT

J & J / R. ROAF Fellows visited the department on 6th January 2006, they observedcustom mega prosthetic replacement, revision total hip arthroplasty being performed

and took part in the scientific session.

Dr. Ilango and Dr. Balakrishnan, Consultant Orthopaedic Surgeons from UKvisited the Department and delivered lectures on revison hip arthroplasty onJuly 2005.

Interactive programme was organized with Mr. Ian Ritchie, Director,Surgical Training (FRCS, Edinburgh) & Consultant Orthopaedic Surgeon on8th August 2005 at 10 am in our Seminar Hal l , which was very helpful for post

graduates who were planning their future studies and work in UK.

The eighth Dr. V.R. Thayumanaswamy Memorial Oration 2005 was organized

on 17th September 2005. Prof. (Major) D. Raja, Former Vice Chancellor ofTamil Nadu Dr. MGR Medical University, presided over the oration. The oration on

"The Evolut ion and Progress of Medical Science" was delivered byProf. T. Dorairajan, Director of Medical Services (Retd.) and Paediatric Surgeon.

Holistic approach to patient care was emphasized by Dr. Ashok Kumar, YogaTherapist. Different asanas its therapeutic application was emphasized. He also

emphasized the important of relaxation techniques for doctors.

Special programmme on body building by state champions led by was organizedto emphasize to the PG student the importance of maintaining good health.

Prof. K. Sriram was kind enough to donate some of the Journal collections andCDs for the department. He also donated some of his spine implant to the department

as an additional to our armamentarium on 31 st October 2005.

Dr. Patrick Kluger and Dr. Joseph Erli, Aachen University, Germanyintroduced the K-Fixator for reduction and fixation of Sondylolisthesis onl l th March 2006.

PARAPLEGIA UNIT - Monthly Conference for planning and review of the treatmentand Rehabilitation procedures was rescheduled to the second Friday of every month

and the 300th paraplegia wards conference was held on I Oth march 2006 between

10 am and 12 noon.
