CELL PHONE USER AGREEMENT

Mr. /Ms. / Mrs. ________________________________________, selected to by company for receive Cell Phone Form Company, this phone is to be used for company purpose only, with following terms and conditions:

Text MassageThese are billed on a per message basis. Company does not carry a texting plan, so any text messages will be the responsibility of the phone user.

Application DownloadAny costs due to application downloads to your phone will the responsibility of the phone user.

Internet SurfingInternet are not allowed to used in cell phone, in case of used company will be terminate phone connection, in this case all amount of bill are payable to user accounts. (Web mail account (Only) users from cell phone user must have permitted to surf; permitted users are free from this agreement point).

Illegal use Illegal used of phone and numbers are not acceptable in term of

legal actions can be possible on user as subject to activity. Fun through calls is not acceptable.

Lost / DamageIf a phone is lost or abused to the point of being unusable, the user will have to replace it with the same or a comparable phone. If a phone wears out through normal use it will be the responsibility of the company to replace the phone.
