Forms to Complete During Open Enrollment

Forms submitted during Open Enrollment are subject to the terms of your collective bargaining agreement.

Please refer to the benefits booklet sent to you via email and to your school or other district location. Within the booklet, you will see a benefits forms grid. The grid is a clear outline of what forms are applicable to your job classification and when each form need to be completed.

Qualifying Life Events

If you experience a qualifying event and desire to make changes to your benefits enrollment, you have 31-days from qualifying event date to do so.

Employee and Dependent Benefits Eligibility

• Employees working at least 30 hours per week who have met District requirements and the obligations of their Collective Bargaining Agreement

• Legally married spouses

• Medical- Children up to age 26 and children who are over 26 but are permanently disabled

• Dental & Vision- Children up to age 19 or up to age 23 if full-time student

Flexible Spending Account (FSA)

The District offers both a Health Care and Dependent Care FSA.

• 2018 FSA maximum/minimum contribution limits:• Health Care- $2,650/$120• Dependent Care- $5,000/no minimum

• Carryover provision• Allows you to carryover amounts from $60 to $500 of unused

funds from your health care FSA at the end of each plan year. The carryover does NOT apply to the Dependent Care FSA.
