67-year old male presents with swelling of lower leg and progressive heaviness and discomfort.

On US, the patient has SSV insufficiencies with multiple varicosities in the popliteal fossa. These varicosities are located about a 1cm

proximal to the SSV termination into the popliteal vein. The popliteal vein does not reflux.

Our plan is to do endovenous ablation of the SSV. This will begin about 2-3 cm from the popliteal vein (Straight rule). The multiple varicosities may thrombose spontaneously after the ablation, if not, foam sclerotherapy will need to be done to prevent further vein formation.

Notice the location of the tibial nerve.
