Soil undermining of the gas piping due to adjacent water facility leakage had caused sag in the piping. 36” inches of paving material from multiple road patches were covering the leak, instead of normal 18”.

It was found that the sewer undermining, road weighting and the failure of the “poorly completed, poorly beaded” gas fusion were co-contributing factors of the explosion.

Review of all fuser qualification records uncovered many fusers had lapsed qualifications of up to a year. The fuser in this case was lapsed by 1 month and inspector lapsed by 6 months.

Mapping for that section of piping inadequately identified the pipe material.

Leak survey records could not be produced for that section of piping for 2013-14.

PE fusion investigation of all NY utilities launched 6/14 identified widespread non-compliance with qualification protocols.

A new PSC order was published 05/15. “Second person” qualified inspections were required starting 10/15 and work fully documented. Random samples of existing fusions were inspected across all NY utilities beginning immediately. Extensive training/re-qualification of personnel also took place in this period.

Beginning on 01/16, the final step was to have “an auditable, electronic database must be kept correlating the GPS location to the fusion and result of its’ inspection. This includes newly constructed fusions and any that are remediated. Fusion records and failures are to be submitted to PSC, including fuser and inspector information.” The plan to accomplish this was due 09/15.

Short time to plan (3+ months) and implement (6+ months) an electronic data capture solution.

Determined it was best to leverageour existing GIS system and fieldcollection, as used in our ElectricStorm Damage AssessmentProgram, which had been inproduction for approximately1 year.

Electric Storm Damage Assessment was using 300 Apple iPads with ESRI Collector dedicated for major outage events. It was decided that Gas would get 300 for their own use.
