Overview:

The Capacity Building Activity Tracking Form was designed to document

individual capacity building events that are conducted by agencies with WI

AIDS/HIV Program grant-funded capacity building interventions.

The form allows agencies to collect data on their capacity building activities with

parameters that match the EvaluationWeb capacity building data-entry fields.

These parameters also match the objectives defined by agencies in their WI

AIDS/HIV Program approved capacity building intervention plans.

Capacity building events are broken down into two subcategories:

Agency Capacity Building Events: efforts to assist other agencies in

expanding their quantity, quality, and comprehensiveness of HIV

services provided.

Population Capacity Building Events: efforts to assist high-risk,

underserved populations in building HIV services and in increasingcommunity involvement in HIV service development, implementation,

and access.

The Capacity Building Activity Tracking Form has two sections: one to document

agencycapacity building events, one to documentpopulation capacity building

events. Agencies should only use the section that is appropriate for the

documented activity. The remaining section can be skipped.
