Item 3 Any person with a valid interest in the corporation can apply for a revival. According to subsection 209(6) of the CBCA, the applicant must be an interested person, which includes but is not limited to, the following: a) a shareholder, a director, an officer, an employee and a creditor of the dissolved corporation; b) a person who has a contractual relationship with the dissolved corporation; c) a person who was not a person described in paragraph a) at the time of dissolution, but would be such a person if a certificate of

revival is issued; and d) a trustee in bankruptcy or liquidator for the dissolved corporation or other body corporate.

Item 4 If the applicant is a corporation, the name must be the name of the corporation, the address must be that of its registered office, and the articles must be signed by a person authorized by the corporation.

Also Include: • Form 3 - Change of Registered Office Address, if there was a change in the registered office address • Form 6 - Change Regarding Directors, if there was a change regarding the directors • A Nuans name search report, if the corporation has been dissolved for more than two (2) years. A Nuans name search report is not required if the corporation had a numbered name (for example, 123456 Canada Inc.) • Fee payable by credit card (American Express, Visa or MasterCard) or by cheque to the Receiver General for Canada. Item 1 Articles of revival must be accompanied by a Nuans name search report dated not more than 90 days prior to the receipt of the articles by Corporations Canada. A Nuans name search report is not required if the corporation has been dissolved for less than two (2) years or had a numbered name.
