1. Develop & Embed “Referral to Enrollment” Service System Savannah-Chatham County Public Schools (social workers, counselors, parent

facilitators, nurses, teachers, principals, athletic trainers), Juvenile Court (probation and intake officers, judges, social workers), Live Oak Public Library System (librarians), Hospital Emergency Department, City and County Recreation Department athletics and summer camp programs

2. Offer multiple Ways to Enroll Personal, one-on-one enrollment assistance Telephone enrollment or by mail Mobile enrollment and Onsite enrollment events at library, schools, sporting events, 3. Train Support, Develop Enrollment Assistance Partner Coalition Cultivate “ownership by partners to embed GaPHIPs enrollment into services4. Board and Targeted Public Awareness Campaign5. Ensure 100% enrollment achieved for eligible families (DFCS feedback loop)
