c. Relevant Engineering Experience: Britt is a group and project manager in CALYX’s Roswell office. She has 20 years of experience, 11 years with CALYX, which includes designing and managing transportation and planning projects, managing people and subconsultants, and managing task order services contracts. In her current role, Britt is responsible for quality control, developing cost estimates, maintaining budgets and schedules, developing project scopes, and allocating resources—all to ensure that clients consistently receive the best possible service. In addition to her project management expertise, Britt has engineered several projects from conceptual design through preliminary design to final design and right-of-way. She currently manages CALYX’s GDOT Districts 3, 4, & 5 Environmental On-Call contract, and is responsible for coordination with GDOT OES and GDOT design Project Managers on 12 individual task orders to date, and managing a team of seven different subconsultants to ensure work is completed in accordance with the GDOT Environmental Procedures Manual and GDOT PDP.

Britt will use CALYX’s structured Project Management Plan (PMP), which involves buy-in from all team members. It establishes the expectations of GDOT, clarifies project objectives and deliverables, and confirms individual roles in each project. Our management system relies heavily on regular client interface, involving both team representatives and client staff in project development. As a result, anticipated project performance is fully understood prior to project development, and stumbling blocks are identified and dealt with in a timely fashion throughout the course of the project. Defining roles and expectations of team members ensures all needs and requirements are met within budget and on schedule.

GDOT, Statewide Bridge Replacement On-Call Services Contract (2 of 2), Statewide, GA: Britt was the project manager responsible for managing and designing the approaches for three bridge replacements in Georgia. She managed a team of designers, hydraulic engineers, surveyors, and environmental scientists, and served as the liaison between the team and the prime consultant.

3. GDOT, District 1 Environmental Task Order Services Contract, Various Counties, GA: CALYX provides environmental services in support of the engineering design and development of transportation facilities under limited supervision and on an On-Call basis. This contract consisted of 25 task orders; 15 of which were bridge projects. Britt was responsible for managing the master contract as well as managing the individual scopes, schedules, and budgets for each of these task orders. In addition, she was responsible for coordination with GDOT Office of Program Delivery Project Managers, Office of Environmental Services Subject Matter Experts (SME’s), and the district designers and Preconstruction Engineer. She also managed a team of seven different subconsultants to ensure work was completed in accordance with the GDOT Plan Development Process, Environmental Procedures Manual, and state and federal requirements. A variety of tasks were performed for this contract, including NEPA documentation, history documentation, air and noise studies, ecological assessments, archaeology investigations, protected species surveys, freshwater aquatics, and public involvement activities.

4. GDOT, SR 42 at Echeconnee Creek, Monroe County, GA: Britt was project manager/engineer responsible for the conceptual, preliminary, final, and right-of-way plan design of this 0.5-mile project. She prepared horizontal and vertical design, maintenance of traffic plans, drainage design, and conducted the public meeting for this project to replace a structurally deficient bridge on existing location. Numerous alternatives were designed and reviewed to achieve the alternate that least affected the historical properties in the area.

5. City of Roswell, Big Creek Bridge Road, Roswell, GA: Britt was the NEPA project manager for this City of Roswell project that explores design concepts to construct a new location bridge over SR 400. She was responsible for monitoring the NEPA group’s day-to-day activities, which included background investigation, coordination, and screening of natural and cultural resources in the study area as well as public involvement assistance to the prime consultant. The project is an effort to relieve traffic congestion on Holcomb Bridge Road from SR 9 to Old Alabama Road. The project will provide a new connection across SR 400 so that some traffic can avoid the Holcomb Bridge/SR 400 interchange. e. Relevant Experience Utilizing GDOT Specific Processes, etc: Britt has coordinated project efforts from concept through completion on a variety of projects which has given her the opportunity to stay current with all GDOT policies and procedures. For the past six years, she has managed the District 1 Environmental On-Call Contract, which included 28 different task orders, and the District 3, 4, & 5 On-Call Contract which has a total of 12 tasks to date, two of which include bridge projects and one menu of services task that includes determining habitat for 26 bridges. All work on these contracts was done in accordance with the GDOT PDP and Environmental Procedures Manual. On the District 1 contract, she was able to work with changing GDOT Contract Liaisons (four times), four GDOT contract negotiators, GDOT procurement specialists, eight GDOT Program Delivery PM’s, and eight OES specialists (ecologists, air/noise, historians, and NEPA). c. Relevant Roadway Design Experience: Ken is a project engineer with more than 26 years experience in roadway design. He is experienced in the use of CAiCE, InRoads, and Microstation V7/V8, and in following the GDOT AASHTO and FHWA standards, Plan Presentation Guide, and Plan Development Process. Ken has prepared, and reviewed roadway construction plans from the conceptual phase to preliminary plans to right-of-way plans and through final plan stages. Ken successfully supports a project’s progress, coordinating with GDOT, public officials, consultants, and the general public. While employed with GDOT in Urban Design, he served as a designer on a multitude of projects and served on many committees, including the Department’s Drainage Manual re-write team. Ken’s diverse project experience includes suburban projects, lane widening, intersection improvements, bridge replacement projects, and sidewalk additions. 1. GDOT, SR 74 Widening and Reconstruction, Fayette County, GA: Ken was lead project engineer, responsible for providing design services for the widening and reconstruction of three miles of SR 74 from SR 85 to Crosstown Road, including a river crossing bridge. The heavily congested, two-lane roadway was expanded to four 12-foot lanes with a 24-foot raised median, curb and gutter, and sidewalks. Design services included validation of the existing concept, public meetings, preliminary plans, right-of-way plans, signal, signing and marking plans, and the preparation of final plans. This project is open to traffic. 2. GDOT, Old Alabama Road Corridor Improvements, Fulton County, GA: Ken was project engineer for this project, which was divided into three separate contracts for a combined total of eight miles. The goal of these turnkey projects was to widen and improve eight miles of an existing urban arterial which varies in width from a two-lane to a five-lane road. The project would add turn lanes, sidewalks, a multi-use path in one direction, replace two bridges, and improve safety issues. Ken was in charge of plan production, plan geometry-setting alignments and profiles, and all drainage design. The project was canceled by GDOT due to the local municipalities refusal to continue to participate. 3. GDOT, SR 360 Widening and Reconstruction, Cobb & Paulding Counties, GA: Ken is project manager/engineer, responsible for providing design services for the widening and reconstruction of SR 360/Macland Road from SR 120 in Paulding County to SR 176/New Macland Road in Cobb County. The existing two-lane roadway is being expanded to four 12-foot lanes with a 20-foot raised median, curb and gutter, and sidewalks, for a total length of 6.2 miles. An existing culvert is being replaced by a bridge. Design services include concept development, environmental services, surveys, subsurface utility engineering, public meetings, preliminary plans, right-of-way plans, traffic operation plans, geotechnical investigations, bridge design, and the preparation of final plans.

JOHN McWHORTER, PE a. Education: B.S., Civil Engineering Technology, Southern College of Technology, 1994

Relevant Bridge Design Experience: John has 22 years of experience in many aspects of bridge design ranging from hydraulic and hydrological studies to final bridge design to project management for bridge replacement projects. He has been responsible for the development of over 200 preliminary bridge designs for new location, replacement and widened bridges. He is experienced in modeling complicated bridge hydraulics, including multiple openings, tidal conditions, and the effects of abnormal floodstages as well as meeting FEMA requirements. John has also been responsible for the geometry and final design of multiple structure types, including steel bridge widening, reinforced deck girder bridges as well as prestressed (PSC) beam bridges. He has experience in the design and detailing of multiple foundation types, including pile bents, multi-column bents on pile footings and drilled caissons. This combination of bridge final design and bridge hydraulic design experience, along with extensive discipline coordination with road design, gives John a unique background for bridge replacement projects at stream crossings. His understanding of the hydraulics and bridge related issues and their impact to the other disciplines (survey, environmental, roadway design, etc.), provide him the “big picture” perspective needed to successfully deliver a stream crossing project.SR 17/75, GDOT, Towns County, GA: John was responsible for the final bridge design and plan development for this 197-foot-long, three-span bridge over Hiawassee River in Towns County. The bridge consisted of a 117-foot center span utilizing Bulb Tee PSC beams and two 40-foot Type I mod end spans. The two intermediate bents consisted of 3-feet-square concrete columns with pile footing foundations.SR 44, GDOT, Greene/Putnam Counties, GA: John was responsible for the hydraulic and preliminary bridge design for all six of the stream crossing bridges on this project. He is also overseeing the final bridge design for each of the bridges currently in final design, including single span PSC beam bridges, multi-span steel beam widening, and a two span PSC beam widening.SR 74/85 over Pappy’s Creek, GDOT, Meriwether County, GA: John was the bridge engineer for the H&H study as well as the final design and final bridge plans for this stream crossing project. This bridge was designed on a sharp 45-degree skew and consisted of PSC beams with a single caisson intermediate bent and pile end bents.d. Relevant Experience Utilizing GDOT Specific Processes, etc: John follows GDOT’s Bridge and Structures Design Manual and the Drainage Manual, particularly as it relates to bridges. He is accustomed to using GDOT’s bridge design software as well as the department’s cell library and basic drawings. He is experienced in much of the available hydraulic software, including HEC-RAS, WMS, SMS, FESWMS, HY8, HEC-2 and other software packages such as Geomath and Microstation.

c. Relevant NEPA Experience: Tish is a planning senior project manager. With 25 years of experience, she is an expert in providing environmental planning and feasibility studies, community visioning, NEPA analysis and documentation, and public involvement. She has managed a variety of multi-modal projects in various stages of the planning and environmental processes, and has authored more than 70 environmental documents for transportation as well as non-transportation projects. Her core strengths are her ability to lead large, multi-disciplinary teams, and her extensive NEPA experience for federal, state, and local clients.GDOT District One, On-Call Contract for Environmental Work, Task Orders 9, 10, 11 and 12, Various Locations, GA: Tish was environmental manager for this multi-year, $2-million On-Call environmental planning contract for GDOT. These task orders all involved bridge widening and replacements with off-site detours, bat and aquatic surveys, 404 permits, stream buffer variances, and public involvement. She was the primary point of contact for the project team and the client, facilitating communication, workflow, and completion of tasks and deliverables. She has been principal author or primary quality control reviewer of each environmental document completed under these task orders. GDOT, SR 120 Widening, Gwinnett & Fulton Counties, GA: The project will widen SR 120 from Medlock Bridge Road (SR 141) to Peachtree Industrial Boulevard from two through lanes to four through lanes. The total project length is approximately 2.5 miles. Side roads will be improved approaching SR 120 in all directions, including Parsons Road, Boles Road, Peachtree Industrial Boulevard, and Medlock Bridge Road. The existing bridge will be widened and reconstructed across the Chattahoochee River. Pedestrian and bicycle access is being studied. Public involvement plays a big role on this project with extensive meetings with adjoining neighborhoods, NPS, the multi-lingual community as well as Johns Creek, Duluth, and Gwinnett County. Tish serves as environmental task lead for this project. She was primary author of the public involvement plan, project justification statement, and logical termini form, and will serve as principal-author of the environmental assessment and Section 4(f) documentation. SR 20 from Samples Road to Peachtree Industrial Boulevard, across the Chattahoochee River, Gwinnett & Forsyth Counties, GA: Tish was the environmental lead for the client and project team, facilitating all communication, and workflow. In addition, she was completion manager and co-author of the EA, Section 4(f) and reevaluation for this project which included the widening of the existing bridge to a four-lane facility over the Chattahoochee River. Tish coordinated the subconsultant special studies and reviewed all study reports.d. Relevant Experience Utilizing GDOT Specific Processes, etc: Tish is very familiar with GDOT’s Plan Development Process. Recently, Tish served as environmental manager for the GDOT District 1 task order services contract for environmental work. SR 74 CORRIDOR IMPROVEMENTS (PI#322357) a. GDOT l Fayette County, GA l 2005-2012b. CALYX was the primary consultant for this project and was responsible for the overall design and management. The scope of work consisted of concept validation, public involvement, preliminary plans, right-of-way plans, final plans, shop drawings, and limited construction services. CALYX conducted the public involvement activities, prepared the Concept Validation Report, the preliminary, right-of-way, and final plans, and administered QC/QA for the roadway and bridge plans. CALYX surveying crews performed the right-of-way staking for the project. This project consisted of 3.3 miles of widening and improvements of SR 74 in Fayette County. The improvements included widening the existing two-lane roadway to a four-lane roadway with a 24-foot raised median, two intersection re-alignments, and construction of a new bridge over Flat Creek. The improvements enhanced the safety and the mobility for the traveling public and accommodated the growth and development in the area. SR 360 WIDENING AND RECONSTRUCTION (PI#0006049) a. GDOT l Cobb and Paulding Counties, GA l 2006-2016b. CALYX is the primary consultant providing overall design and management for this 6.2-mile roadway. The proposed typical section includes four 12-foot lanes (two lanes in each direction) with curb and gutter and sidewalks (urban shoulders) and a 20-foot raised median. In addition, an existing triple box culvert (10’W X 9’H) will be replaced with a bridge. CALYX completed the environmental document and right-of-way plans were approved and authorized in 2012, including 267 right-of-way parcels. OLD ALABAMA ROAD IMPROVEMENTa. GDOT l Fulton County, GA l 2006-2012b. The corridor was divided into three projects with two bridges: from Holcomb Bridge to Jones Bridge, from Jones Bridge to Buice Road, and the intersection with Old Alabama Connector. CALYX prepared approved concept reports, performed all surveys and data research, prepared the Environmental Assessment for the corridor and the approved categorical exclusion for the intersection, prepared preliminary plans for the entire corridor and intersection, and right-of-way and final plans for the intersection project. During concept development, we prepared alternatives that ranged from configurations of a traditional signal design to a combination of dual-lane roundabouts at two closely spaced intersections. We introduced creative alternatives for single and multi-lane roundabouts with slip ramps at 13 intersections, a pedestrian tunnel, a “Florida T” intersection, bike paths, and shoulder and median treatments. SC 5 BRIDGES OVER THE CATAWBA RIVER & OVERFLOW STRUCTURE a. SCDOT l York and Lancaster Counties, SC l 2008-2013 b. CALYX provided the SCDOT with survey, planning, design, and environmental engineering services for replacing the existing SC 5 bridge over the Catawba River and S.A.L. Railroad as well as the SC 5 overflow relief bridge in Lancaster and York Counties for which we were engineer of record. This two-mile long project was a top priority bridge replacement in 2010 and CALYX was able to meet the compressed production schedule using our turn-key delivery approach. Our team was responsible for inventorying and assessing the impacts associated with both the natural and human environment. We addressed several major environmental issues, including water quality (Catawba River), cultural resources, tribal coordination, major utility conflicts, and constructability. We prepared and obtained approvals for the environmental inventory, alternative evaluation, NEPA documentation, and permitting in accordance with federal funding requirements. CALYX was also responsible for maintaining the agency and public involvement coordination throughout the project. The project followed the state and federal requirements, guidelines, and regulations. The bridge used steel pile foundations for both the end bents and interior bents. SR 120 WIDENING (PI#721000) a. GDOT l Gwinnett and Fulton Counties, GA l 2013-ongoingb. This project will widen SR 120 from Medlock Bridge Road (SR 141) to Peachtree Industrial Blvd. from two through lanes to four through lanes. The total project length is approximately 2.5 miles. Side roads will be improved approaching SR 120 in all directions, including Parsons Road, Boles Road, Peachtree Industrial Blvd., and Medlock Bridge Road. The existing bridge will be widened/reconstructed across the Chattahoochee River. Pedestrian and bicycle access is being studied. We anticipate, at a minimum, sidewalks on one side with the potential for enhanced sidewalks or additional sidewalks on the other side. Public involvement plays a big role on this project and includes extensive meetings with adjoining neighborhoods, NPS, the multi-lingual community as well as Johns Creek, Duluth, and Gwinnett County. All signalized intersections will be modified along the corridor. (1.b) Primary Office The majority of work for this project will be completed and managed from our Roswell or Columbus, Georgia office. Total Staff: 26 transportation (7), traffic services (2), environmental/planning (9), survey/SUE (7), and administrative (1). Bridge design (4) will be completed in our Raleigh, NC office and bridge hydrology (8) will be completed in our Charlotte, NC office.Including our Georgia offices, CALYX employs a total of 145.

(1.c) Additional Resource Areas and AbilityOver the last 23 years, CALYX has developed a reputation as a multi-disciplined, quality service provider in roadway and bridge design, bridge hydrology and drainage design, traffic engineering, transportation planning, environmental services, land surveying, and subsurface utility engineering. Each of these disciplines complement and support one another, resulting in better outcomes for our clients. Bridge replacement projects such as these need an experienced and smart team that has worked together routinely through the years. CALYX staffs our projects with highly qualified team members that understand the intricacies of bridge replacement projects and know how to ensure successful outcomes. Britt Hennessey, PE, will lead the CALYX team as project manager. She has extensive experience managing various task order contracts, especially bridge replacement projects. She currently manages CALYX’s GDOT Districts 3,4, & 5 Environmental On-Call contract. Since 2002, Britt has worked with team members Ken McDuff and John McWhorter on many bridge replacement projects — Britt as the project manager, Ken as the roadway lead and John as the bridge hydraulic engineer. They know how to work closely together to get these projects done quickly, smoothly, on time, and within budget. This team knows each other extremely well which allows for a seamless working environment. Add to the engineering team Tish Stultz, a seasoned CALYX environmental project manager with many bridge replacement projects under her belt, and we have a tight knit team that works well together, that knows and understands the common goal of the project, understands the PDP process, and has availability to dedicate to this project. Since this contract contains five bridges, we plan to split the work into two groups, each with its own roadway design, bridge design, survey, and bridge hydro members. Team one will handle three bridge projects and include CALYX staff and team two, handling the other two projects, will be compromised of TranSystem staff. CALYX, VHB, and Ecological Solutions will provide environmental services for all five projects. NOVA, likewise will provide

geotech for all projects. We are confident that splitting the projects up in this manner will allow the schedules to be met in an efficient manner.In addition to our key team leaders, the following individuals will play an important role in making this a successful project for GDOT. Junior Tunnell, RLS: Junior has over 30 years of experience on a variety of surveying and SUE projects, including dozens of bridge replacement projects. Junior’s has enough staff and availability to work on several bridge projects at a time. Matt McDaniel: Matt is a senior architectural historian with extensive nationwide experience planning for the consideration of historic properties within the context of major transportation, civil engineering, and public works projects. Matt is currently working on the GDOT Georgia Historic Bridge Survey and is extremely familiar with bridge replacement projects.Geotechnical investigation is very important to the project and will be handled by our close partner, NOVA, who has extensive direct GDOT experience. CALYX has used the expertise of NOVA on many projects and have always been very pleased with the results.The team has an excellent track record for being responsive, available, and having the ability to provide quality services to GDOT.
