ประวต

อาการไอ แนนหนาอก หายใจมเสยงหวดเปนๆหายๆ

อาการเปนมากในชวงอากาศเยน ตอนกลางคนหรอไดรบสงกระตน

อาจมอาการรวมกบโรคภมแพ หรอมประวตในครอบครว

การตรวจรางกาย

ถาขณะมอาการอาจฟงไดเสยงหวด แตอาจตรวจไมพบความผดปกตกได

ควรแยกโรคอนทอาจมอาการคลายๆกน

การวนจฉยโรคหด

การตรวจทางหองปฏบตการ

- spirometry : revesibility FEV1 > 12%( 200 ml)

- peak flow reversibility > 20 %

- peak flow variability > 20 %

- methacholine challenge test

การวนจฉยโรคหดในเดก

เปาหมายของการรกษาโรคหด

1. ควบคมอาการของโรคใหสงบ

2. ปองกนการก าเรบของโรค ใหผปวยมสมรรถภาพปอดทดทสด

3. สามารถด ารงชวตไดเชนเดยวกบคนปกต

4. หลกเลยงภาวะแทรกซอนจากยารกษา

5. ปองกนการเสยชวตจากโรคหด

การด าเนนการเพอไปสเปาหมายตามguideline

1.ใหความรแกผปวย ญาต และ ผใกลชด 2. หลกเลยง หรอ ก าจดสงกระตนภมแพ หรอ อาการหอบ ดวยวธท

สามารถท าไดจรง 3. ประเมนล าดบความรนแรงกอนการรกษา 4. ประเมนการควบคมโรค และ วางแผนรกษาระยะยาว 5. วางแผนการรกษาอาการหดก าเรบเฉยบพลน 6. จดระบบใหมการรกษาตอเนอง อยางมประสทธภาพ 7. มแผนการรกษาผปวยหดในกรณพเศษ

หลงรกษา หากยงไมสามารถควบคมอาการได ใหเพมการรกษาไปอก 1 step

= เพมขนาด ICS หรอ เพม Controller ชนดอน โดยเฉพาะ ICS + LABA ถายงไมเคยได อาจใช oral steroid ชวงสนๆ แบบ acute exacerbation

ถาสามารถควบคมอาการได ใหใชยาตามเดมตอไประยะหนง อยางนอย 3 เดอน

การรกษาหลงเกด acute exacerbation หลงควบคมอาการไดดแลว อาจกลบไปรกษาตามขนเดมของผปวย หรอ ปรบเพมขน 1 step

การรกษาตาม step 4 และ step 5 ควรอยในการดแลของผเชยวชาญ

การปรบลดยา ขอมลเชงประจกษปจจบนยงไมมากพอ ในดาน ** ระยะเวลาทเหมาะสมในการเรมลดยา ** ล าดบขนตอนในการลดยา ** ปรมาณยาทปรบลดในแตละครง

** ปรบลดขนาดของ ICS กอน ในกรณใช ICSขนาดปานกลางหรอสง โดยลดลงประมาณครงหนงของขนาดเดม

** ปรบลดทก 3 เดอน (หรอมากกวา?) โดยไมท าใหอาการก าเรบ ** ถาใช ICS รวมกบ controller ชนดอน ปรบ ICS กอน จนได

• เปนโรคทปองกนไดและรกษาได

• โดยมลกษณะเปน progressive, not fully reversible airflow

limitation

• ซงเปนผลจากการระคายเคองเรอรงตอปอด จากฝนและกาซพษ ทส าคญทสดไดแก ควนบหร ท าใหเกด abnormal inflammatory response ทงในปอดและระบบอน ๆ ของรางกาย (multicomponent disease)

• โดยทวไปมกหมายรวมถงโรค 2 โรค คอโรคหลอดลมอกเสบเรอรง (chronic

bronchitis) และโรคถงลมโปงพอง (pulmonary emphysema)

จากการตรวจ spirometry : FEV1/FVC หลงการใชยาขยายหลอดลม < 70%

การแบงความรนแรงของโรค ใชคา FEV1 รวมกบอาการทางคลนก

The characteristic symptoms of COPD are chronic and progressive dyspnea, cough, and sputum production.

Dyspnea: Progressive, persistent and characteristically worse with exercise.

Chronic cough: May be intermittent and may be unproductive.

Chronic sputum production: COPD patients commonly cough up sputum.

Symptoms of COPD

ขณะน อาการเหนอยหอบของคณเปนอยางไรบาง 0. ไมมอาการเหนอย เพยงแครสกหายใจหอบเฉพาะ ขณะออกก าลงกายอยางหนกเทานน

1.หายใจหอบ เมอเดนอยางเรงรบบนพนราบ หรอเมอเดนขนทสงชน 2.เดนบนพนราบไดชากวาคนอนทอยในวยเดยวกน เพราะหายใจหอบ หรอตองหยดเพอหายใจเมอเดนปกตบนพนราบ

3.ตองหยดเพอหายใจหลงจากเดนไดประมาณ 100 เมตร หรอหลงจากเดนไดสกพกบนพนราบ

4.หายใจหอบมากเกนกวาทจะออกจากบาน หรอหอบมากขณะแตงตว หรอเปลยนเครองแตงตว
