Introduction This directory provides a list of Bright Health Plan Homelink Hearing Healthcare hearing services providers. To get detailed information about your healthcare coverage, please see your Evidence of Coverage.

You can find a list of in-network hearing services providers online at BrightHealthPlan.com/Medicare. As a Bright Health Medicare Advantage plan member, you may go to any of the network providers listed in this directory for your hearing exam and hearing aid fitting. To make an appointment, just call your chosen in-network provider’s office and let them know you’re a member of Bright Health.

Keep in mind, if you choose to get care outside of the plan’s network, you will pay for the full cost of the service. If you obtain routine care from out-of-network providers, neither Medicare nor Bright Health will be responsible for the costs. If you have a medical emergency, get help as quickly as possible. Call 911 for help or go to the nearest emergency room or hospital.

If you need urgent care, call your doctor’s office during the day to make sure you don’t incur unnecessary expenses

Bright Health plans are HMOs and PPOs with a Medicare contract. Bright Health’s New York D-SNP plan is an HMO with a Medicare contract and a Coordination of Benefits Agreement with New York State Department of Health. Our plans are issued through Bright Health Insurance Company or one of its affiliates. Bright Health Insurance Company is a Colorado Life and Health company that issues indemnity products, including EPOs offered through Medicare Advantage. An EPO is an exclusive provider organization plan that may be written on an HMO license in some states and on a Life and Health license in some states, including Colorado.
