The Foundation for Anesthesia Education and Research aims to advance medicine through anesthesiology research and education. Our grants and programs are designed to drive the medical specialty of anesthesiology forward and positively impact the careers of individuals who will improve patient care and safety, and who will find solutions to current issues. The early career funding that FAER is able to provide because of the generosity of the entire anesthesiology community not only influences individual careers but also changes the future of anesthesiology for the better.

In 2012, in order to support more than 130 anesthesiologists, residents and medical students, FAER committed $1.875 million to funding research grants and more than $300,000 to programs. These numbers show the investment we make each year in research and education, and the highlights within this annual report help illustrate the outcomes.

Putting Academic Anesthesiology in Focus: FAER Resident Scholar ProgramThe FAER Resident Scholar Program is an educational opportunity for residents that encourages careers in research and academic anesthesiology. The program sends approximately 60 residents to the ASA annual meeting each year. By attending workshops and plenary sessions, as well as the ASA House of Delegates, Resident Scholars learn about the latest advances in anesthesiology, and identify scientific, educational and political issues facing the specialty.

As a CA-2 resident at the University of Virginia, Kevin Beers, MD, attended ANESTHESIOLOGY™ 2012 as a FAER Resident Scholar. He says of his experience, “I am grateful to have been chosen by the faculty at UVA to attend the meeting as a FAER Resident Scholar. The educational opportunities were seemingly endless. Not only was I able to learn of several advances and changes in our specialty, but I also was able to network and interact directly with leaders in anesthesiology. … Being a FAER Resident Scholar has put me on a path toward future involvement in the ASA and beyond.”

Abbott Laboratories provided a generous educational grant to support the 2012 FAER Resident Scholar Program.

After Research Fellowship, Eyes on Residency: FAER MSARF ProgramThe FAER Medical Student Anesthesia Research Fellowship (MSARF) program encourages medical students to enter careers in academic anesthesiology. The program pairs medical students with anesthesiology departments for summer fellowships where they conduct projects, learn scientific methods and participate in clinical anesthesia activities. Students present their research at the FAER MSARF Symposium at the ASA annual meeting each fall.

Fifty-eight students participated in the MSARF program in 2012, one of whom was Michelle Mahanian from University of California Los Angeles. Mahanian visited Mount Sinai School of Medicine for her MSARF experience, where she worked alongside Stacie Deiner, MD, on a project titled “Predictors of postoperative delirium in non-cardiac surgery patients.”

“My research experience through FAER taught me to look for broader connections among clinical scenarios that may benefit future patients through research,” Mahanian says. “Participating in a project from start to finish has taught me a great deal about formulating proper hypothesis, integrating relevant patient data into what is already in scientific literature, and analyzing my results into a clinically-relevant conclusion.” Mahanian will graduate next year and plans to pursue her residency in anesthesiology.

The 2012 MSARF program was supported by the Ronald L. Katz Family Foundation, Valley Anesthesiology Foundation and B. Braun Medical Inc., as well as many individuals and anesthesiology departments.

FAER Panel at ANESTHESIOLOGY 2012™

Leaders in quality research discussed how to improve quality in anesthesia at the 2012 FAER Panel, “The Science of Quality Improvement: What Leads to Improvement.” Lee A. FAER Academy of Research Mentors in Anesthesiology WorkshopDuring the workshop “Early Grants: What You Need to Do,” a group of experienced physician-scientists, mentors and leaders in anesthesiology led panel discussions aimed at educating others on how to submit successful NIH grant applications. The workshop featured information on the personal experience of study section chairs, a mock study section, and a panel of physician-scientists who

have received NIH awards. Faculty members who led the panels included: Roderic G. Eckenhoff, MD, University of Pennsylvania; Evan D. Kharasch, MD, PhD, Washington University in St. Louis; William L. Young, MD, University of California San Francisco; and David C. Warltier, MD, PhD, Medical College of Wisconsin.

12th Annual FAER Honorary Research LectureJudy R. Kersten, MD, professor and senior vice chair of anesthesiology at Medical College of Wisconsin was the 12th annual FAER Honorary Research Lecturer. During her lecture, “What’s so bad about being sweet?” she discussed how diabetes and hyperglycemia contribute to increasing perioperative risk through adverse effects on the coronary circulation, nitric oxide regulation, and cardioprotective signaling using evidence from both basic science and clinical research.

BOLD DEVELOPMENTSResearchWith the urgency to transform healthcare and the triple aim of improving health for the individual, improving health for the population and reducing costs, it is critical to train anesthesiologists in the science of health services research. This is why, in 2012, FAER partnered with

the Anesthesia Quality Institute (AQI) to create a pilot mentored grant for early-career investigators in the field of health services research. The first FAER-AQI Mentored Research Training Grants – Health Services Research (two-year, $175,000 grants) will begin in 2013.

EducationIn spring 2012, the FAER Board of Directors established the Academy of Education Mentors in Anesthesiology, as a companion to the Academy of Research Mentors in Anesthesiology. The Education Mentor Academy initiates activities to improve education mentorship in anesthesiology. It recognizes educators and mentors who have made significant contributions to the advancement of academic anesthesiologists.

InnovationOver the past few decades, anesthesiologists have distinguished themselves among physician specialists as being leaders in advancing patient care through innovations in medical technologies.

Although many advances have been made, much work remains. In 2012, FAER launched an educational initiative focused on translational medical research, innovation and entrepreneurship. It took form in an inaugural FAER panel at the Society for Technology in Anesthesia annual meeting.

The educational session answered two questions: “What are the basic approaches for an anesthesiologist to bring an idea to clinical practice?” and “Why are physician-industry relationships important for successful TMR?”

FAER will continue to facilitate educational opportunities related to pharmaceutical and device product development and commercialization through the first-

ever Anesthesiology Conference on Innovation and Entrepreneurship, which will be held January 18, 2014 in Orlando, Fla.

Mission MomentGROWING THE SKILLS OF PRACTICING ANESTHESIOLOGISTSFor anesthesiologists pursuing training in advanced ultrasound-guided regional anesthesia, the efficacy of available teaching methods, learning curve, and optimal duration of training in this population remains unknown. This is why Edward Mariano, MD, MAS, proposed to study the effectiveness of conventional methods (e.g., didactic lectures and model scanning) and immersive learning (e.g., hybrid simulation) in teaching ultrasound-guided perineural catheter insertion to anesthesiologists in practice without prior formal regional anesthesia training.

Dr. Mariano, who is chief of anesthesiology and perioperative care service at VA Palo Alto Health Care System, and associate professor of anesthesiology, perioperative and pain medicine at Stanford University, received a 2012 FAER Research in Education Grant, a two-year, $100,000 award.

“This project is the first to specifically study the acquisition and implementation of a new skill in a cohort of practicing anesthesiologists,” he says. “Since ultrasound guidance in regional anesthesia is relatively new and rapidly increasing in popularity, this is an ideal model to study. The results can be potentially applied to other skills or populations.”

In his career, Dr. Mariano has focused on clinical studies related to perioperative pain management and short-term outcomes. “With direct access to a world-class simulation center and staff, and the largest networked healthcare system in the country [Veterans Health Administration],” he says, “our FAER-funded study marks the start of an exciting path to implementing system change and increasing our veterans’ access to the highest quality of perioperative pain control.”

Alongside his mentor, Steven Howard, MD, Dr. Mariano will continue his work on this FAER-funded project throughout the 2013-14 academic year.

Epitome of ExcellenceEARLY CAREER ACCOMPLISHMENTS RECOGNIZED WITH ASA AWARDOne of the goals of FAER’s grants is to provide the funding, training and mentorship to early career anesthesiologists so they can develop the skills and preliminary data needed to become successful physician-scientists. Peter Nagele, MD, MSc, who received a Mentored Research Training Grant from FAER in 2007 to study the influence of nitrous oxide on postoperative myocardial infarction, has done just that. He received the 2012 ASA Presidential Scholar Award, recognizing his research accomplishments and his dedication to a career as a physician-scientist.

Dr. Nagele is currently assistant professor in the department of anesthesiology at Washington University in St. Louis, where he has been since 2005. After his FAER award in 2007, he received a K23 from the NIH, and has received subsequent funding from the NIH and other foundations since that time.

Dr. Nagele joins several other former FAER grant recipients and Resident Scholars who have received ASA Presidential Scholar Awards, which began in 2003. Mission MomentA STEPPING STONE TO LONG-TERM ACHIEVEMENTSince receiving a FAER Mentored Research Training Grant in 2011 for his project “Anesthesia Technique and Outcomes After Hip Fracture: Instrumental Variable Analysis,” Mark Neuman, MD, MSc, has conducted a number of analyses of a large database with anesthesia care and clinical outcomes for more than 50,000 patients treated for acute hip fracture in New York State. Dr. Neuman’s main goal has been to understand the impact of differing anesthesia types, specifically regional versus general anesthesia, on key patient outcomes.

Based on this work, Dr. Neuman, assistant professor of anesthesiology and critical care at University of Pennsylvania, published a paper in Anesthesiology in July 2012 that showed marked reductions in in-hospital mortality and pulmonary complications among patients receiving regional

anesthesia. And fewer than two years after his award, nearing the completion of his FAER-funded project, he received a grant from the National Institute on Aging (an NIH K08) to continue his research.

“FAER’s support has been vital to my development as a researcher within anesthesiology; it has allowed me to complete research that has potential to impact clinical practice and to secure further funding from the National Institutes of Health,” he says. “Obtaining a FAER Mentored Research Training Grant has been a key early step in achieving my long-term goal of building a successful career in academic anesthesiology and improving patient health through my research.”

Mission MomentANSWERING QUESTIONS AND ADVANCING SKILLS

Anna Woodbury, MD, an anesthesiology resident at Emory University, received a FAER Research Fellowship Grant for her project, “Neonatal Pain Results in Neuro-Psychiatric Disorders: Honokiol’s Role in Treatment and Prevention,” with the goal of optimizing neuroprotection and analgesia in the neonatal population and prevent long-term consequences of early pain.

For her project, Dr. Woodbury used behavioral testing and immunohistochemistry to evaluate the analgesic, neuroprotective and neuro-regenerative effects of honokiol on behavior and brain development in a neonatal rat model of formalin-induced peripheral inflammatory pain. Honokiol is a naturally occurring compound traditionally used throughout Asia as an analgesic and anti-inflammatory agent.

Now nearing the completion of her grant, Dr. Woodbury’s results show that honokiol prevents the development of chronic pain responses and attenuates anxiety-related behavioral changes in a rat model of neonatal pain. “We have taken another step toward understanding pain in neonates,” she says. “Hopefully, we have found a neuroprotective, analgesic agent with minimal side effects that will help prevent the chronic, detrimental effects of pain and/or anesthetics from taking hold in the most vulnerable periods of the developing neonatal brain.”

Dr. Woodbury has had a long-standing interest in research. She says, “The FAER Research Fellowship Grant allowed me the opportunity to apply my clinical knowledge to target an area of medicine I felt needed further elucidation while giving me the chance to learn, in depth, basic science skills that I can apply to pain and neuroscience research for the rest of my academic career.”

Dr. Woodbury’s Research Fellowship Grant was named the 2012 FAER/Gertie Marx Research and Education Award, made possible through the generosity of Gertie F. Marx, MD, a pioneer in obstetric anesthesiology who remembered FAER in her estate.
