Dr Bradley has prepared a publication based on an international symposium on acute pancreatitis that was conducted in Atlanta in September 1992. Participants in that symposium were invited interna- tional authorities in multidisciplinary as- pects of acute pancreatic disease. The contributions to that symposium have been organized in an effective format ro deliver in-depth and current knowledge about all facets of acute pancreatitis and many of the comp li cations. This text is a h.igh quality publication with extensive illustrations of radiographs, phocomicrographs and clinical exampl es. Chapters arc extensively cabled, and refer- ences arc extensive throughout the seven sections. TI1e organization of the publica- tion flows smoothly from the introductory sections co the reviews of complications. A new feature in this presentation is an international perspective on the varying fom1s of acute pancreatic inflammation and a parallel introduct ion of regional pref- erences for therapies. Contributions from the disciplines of surgery, medicine, basic science and imaging give the reader a broad profile of the content of each section. The first section, entitled 'Introduction to acute pancrcatitis', deals with the pathogenesis of acute pancreatiti s, and evaluates several of the stratification sys- tems for prognosticating severity of acute pancreatitis. Although Bradley's aim was to establish a clear cla ssification of acute pancreatitis and its comp licat ions, one is left with rhe impression char clarity of defi- nition is not feasible with the progress ion from severe pancreatitis through the vari- able septic complications. 1n the second s ection, on severe acute pancreatitis, the concriburors assign spe- cific tools of evaluation (including serol- ogy, imaging and e ndoscopy) and then move smoothly through the spectru m of medical and surgical management with appropriate reviews of indications and effectiveness of non interventional and BOOKS interventional treatments. The third sec- tion reviews the severe form of pancreaticis from an international perspective. The term 'pancreatic abscess' has cre- ated confusion and inappropriate therapy for many years. Section 4 artcmpcs to de- fine this complication of severe pancreati- cis, and presents a separate section on percutaneous treatment by an inLerven- tional radiologist co mpared separately with open surgical management o( this complication. Having dealt with the severe forms of the disease and the relevant septic compli- cations, the editor has chosen to focus the fifth St.--ction on 'pscudocyscs' wherein con tri- butions deal with epidemiology and pa- thology, and expectant therapy compared with percutaneous or surgical dra inage. internatio nal contributions reflect the experience of managing pseudocyscs in Australia, Japan and Sweden. The final clinical section deals with the mild form of acute pancreatitis, and has appropriate contributions on the classical components of epidemiology, pathology, imaging and clinical diagnosi s, and three sections on treatment and variations from international centres.
