RETURNING ATHLETES: Sign in to your account & sign up for 19-20 school year

beginning June 1st 2019. Multiple sport athletes: select all of your sports when registering.

NEW ATHLETES: Follow the steps below to register for DSHS Athletics

1. Visit www.athleticclearance.com beginning June 1st 2019.

2. Optional: watch a quick tutorial video

3. Parents register with YOUR valid email and password. We recommend using Gmail or Yahoo email addresses for registration. You will be asked to type in a code to verify you are human. If this step is skipped, your account will not activate.

4. Login using your email address that you registered with in step 3.

5. Select “New Clearance” to start the process.

6. Choose School Year 19-20

7. Choose “DAVIS SR.” for school (even if your student attends DV or a Jr. High)

8. Choose only the sport(s) your child wishes to participate in each season.

9. Complete all required fields for Student Information, Educational History, Medical History, Signature Forms and upload your COMPLETED PHYSICAL (in step #1 –Student Information: ‘choose file’ and click ‘save’)

10. Once you receive the Confirmation Message, you have completed the registra-tion process and are awaiting final clearance.

11. If you would like to register for additional sports, you may check off those sports below the Confirmation Message. Electronic signatures will be applied to the additional sports/activities.

12. All of this data will be electronically filed with the DSHS Athletics Department for review. When the student has been cleared for participation by the Athletics De-partment, an email Clearance Notification will be automatically sent.

13. Print the Clearance Notification email, sign, and give to your Coach on the first day of tryouts/practice.
